Рак почки Введение В России в 2015 г. на учете в онкологических учреж-дениях находились 150 016 больных раком поч ки, вы-явлены 20 365 пациентов со злокачественными новоо-бразованиями почки [1]. По данным W. B. Jacobs, R. G. Perrin, метастазы в позвоночник определяются более чем у 70 % онкологических больных, у 10 % из них развивается компрессия нервных структур [2]. Несмотря на сообщения об эффективности современ-ных таргетных средств (R. J. Motzer и соавт.), прогноз для пациентов с метастазами почечно-клеточного рака (ПКР) остается неудовлетворительным [3].Лечение больных ПКР с метастазами в позвоноч-ник представляет трудности, поскольку последние вызывают агрессивный литический процесс в кости, являются гиперваскуляризованными образованиями, кроме того метастазы ПКР обладают радиорезистент-ностью.Особое место в хирургическом лечении метастазов рака почки в кости занимают операции при пораже-нии позвоночника. Это обусловлено наличием интен-сивного болевого синдрома, резистентного к консер-вативным способам лечения; прогрессирующими неврологическими нарушениями; отсутствием ответа опухоли к лучевой и лекарственной терапии; грубой нестабильностью позвоночника или значительными разрушениями его костной структуры, что и является показаниями к оперативному лечению пациентов
ель исследования. Изучить возможности позитронно-эмиссионной томо-графии, совмещенной с компьютерной томографией (ПЭТ/КТ) с 11 С-Холином у больных РПЖ с низкими значениями ПСА (<2,0 нг/мл) после радикальной простатэктомии.Материалы и методы. Проанализированы результаты 63 ПЭТ/КТ исследований с 11 С-Холином; среднее значение ПСА составило 0,94±0,51 (0,21-1,98) нг/мл, медиана -0,93 нг/мл. Сканирование выполнялось на гибридной системе «Biograph-64» True Point PET/CT (Siemens) через 10 минут после внутривенного введения 11С-Холина (700-950 МБк).Результаты. По результатам ПЭТ/КТ рецидив РПЖ при ПСА<2,0 нг/мл диагно-стирован в 30% (19/63) случаев. Группы больных с ПЭТ-позитивными (n=19) и ПЭТ-негативными (n=44) результатами значимо не отличались по возрасту, времени между первичным лечением и развитием биохимического рецидива, индексу Глисона и полу-чению гормонотерапии на момент выполнения ПЭТ/КТ.Выявлена прямая корреляция (р=0,007) между получением ПЭТ-позитивного ре-зультата и уровнем ПСА: при ПСА менее 2,0 нг/мл, 1,5 нг/мл, 1,0 нг/мл и 0,5 нг/мл ре-цидив РПЖ диагностирован в 30% (19/63), в 23% (12/53), в 17% (6/36) и в 20% (3/15) случаев соответственно.Статистически значимая взаимосвязь была также получена для следующих па-раметров: времени удвоения ПСА<6 месяцев и категорий опухоли >T3 и N1 по TNM (р<0,05 для всех). При этом все эти параметры являлись достоверными предикторами получения ПЭТ-позитивного результата. По результатам множественного регрессионно-го анализа, сочетание перечисленных предикторов, за исключением N1, повышало ве-роятность диагностики рецидива РПЖ по данным ПЭТ/КТ.По локализации рецидива выявлено преобладание местнораспространенных ре-цидивов в 63% (12/19) случаев, в 26% (5/19) диагностировано изолированное отдален-ное метастазирование, в 11% (2/19) -сочетанный рецидив.Выводы. У больных с низкими значениями ПСА (< 2,0 нг/мл) результаты ПЭТ/КТ с 11 С-Холином позволили диагностировать рецидив заболевания (как локореги-онарный, так и генерализованный) в 30% случаев, что явилось определяющим в выборе дальнейшего лечения. Вероятность диагностики рецидива РПЖ по данным ПЭТ/КТ выше у больных с исходно неблагоприятной стадией опухоли (>T3, N1), ростом ПСА и быстрой его кинетикой, что следует учитывать при отборе пациентов на исследование.
Objective. To evaluate the impact of surgical intervention and targeted therapy on the results of treatment and survival of patients with metastases of renal cell carcinoma to the spine. Material and Methods. Retrospective analysis of 100 patients (76 men, 24 women, mean age 58.4 years) with renal cell carcinoma metastases to the spine was carried out. Metastasectomy (en block resection) was performed in 39 patients, palliative decompression and stabilization-in 61. Twenty six patients received adjuvant targeted therapy (7 with metastasectomy, 19 with palliative decompression). The pain syndrome (VAS), neurological status (Frankel scale), and survival time (from the moment of surgery till the lethal outcome or the last follow-up examination) were assessed. The Kaplan-Meier survival analysis and Log-rank test were performed. A p-value < 0.05 was considered significant. Results. All patients demonstrated restoration of neurologic function and reduction of pain syndrome. There was no significant difference in survival time in patients with metastasectomy and palliative decompression (p = 0.47). Statistically significant survival benefit was observed in patients who underwent targeted therapy (p = 0.0019). Conclusion. Targeted therapy increases survival time in patients with renal cell carcinoma metastases to the spine. Metastasectomy is advisable with additional targeted therapy.
Purpose: To evaluate the usefulness of 11C-Choline PET/CT in the detection of recurrent prostate cancer (PCa) in patients with biochemical relapse after radical treatment. Materials and methods:This retrospective study included 217 PCa patients who underwent 11C-Choline PET/CT in the Department of Nuclear Medicine of Bakoulev Scientific Centre. All patients had biochemical relapse 3±2 years after radical treatment for locally advanced PCa (T1-3 N0-1 M0): radical prostatectomy (n = 159) and radiation therapy (n = 58). The mean PSA value in the group was 2.1±2.5 (0.2-9.7) ng/ml, median -1.9 ng/ml. Imaging was performed on PET/CT scanner (Biograph-64, Siemens) 10 min after injection of 11 C-Choline (400-550 Mbq). Results:Overall, according to 11C-Choline PET/CT results PCa relapse was detected in 56% (121/217) of cases: in 50% (80/159) after radical prostatectomy and in 71% (41/58) after radiation therapy.The mean PSA value in PET-positive cases was 3.1±2.2 (0.2-9.7) ng/ml, while in PETnegative cases -1.8±1.7 (0.2-4.6) ng/ml. The majority -68% (65/96) patients with PET-negative scan had low PSA levels (< 2 ng/ml).PET/CT results were positive in 43% (50/115) patients with PSA of < 2 ng/ml, in 63%(45/72) with PSA of 2 to 5 ng/ml, and in 87% (26/30) with PSA of > 5 ng/ml.Local relapse was detected in 51% (62/121) patients, distant metastases -in 28%(34/121) of cases, both local and distant metastases -in 21% (25/121) of cases.Lymph node metastases were detected in 38% (86/217) of all patients included in the analysis, of which 28% (24/86) had lesions in lymph node of normal size (median 7 mm). Importantly, that 27% (11/40) of PETpositive patients with bone metastases had no structural abnormalities on CT images (CT-negative cases), corresponding to isolated involvement of bone marrow. And half of these CT-negative patients (5/11) had single lesions. The mean PSA value in patients with bone metastases was 5.0±3.7 (0.4-9.1) ng/ml, median -3.8 ng/ml.According to 11C-Choline PET/CT results oligometastatic PCa recurrence was revealed in 38% (82/217) of all patients, of which 62% (51/82) had local relapse only. Distant oligometastatic lesions were detected in 38% (31/82), of which 13% (4/31) were presented by normal-size lymph nodes and 19% (6/31) -by early bone marrow metastases.48% (58/121) of PET-positive results were confirmed by data of repeated PET/CT examinations.Conclusion: 11C-Choline PET/CT has been shown to be a single noninvasive accurate technique for detection of recurrent PCa in patients with rising PSA after radical treatment, which allows to differentiate patients with local and distant metastases in one study, as well as identify oligometastatic process, and therefore was useful in determining the further personalized therapeutic approach.

The key events of carcinogenesis are genetic alterations including chromosomal aberrations and point mutations in proto-oncogenes and tumor suppressor genes. This review describes cytogenetic aberrations in the context of nccRC diversity according to the current ISUP classification. Translocation variants of nccRC (MiT-RC) were characterized separately as particular cases of the chromosome rearrangements involving MiT gene family (TFE3, TFEB, MITF). In addition, the main nccRC hereditary forms caused by germinal mutations in the genes FLCN, FH, and MET, as well as recent studies of sporadic tumors with using the next generation sequencing techniques were reviewed. These experiments were designed to search for somatic mutations throughout the tumor genome or exom and revealed the different mutational profiles of I/II papillary RC subtypes, chromophobe carcinoma versus oncocytoma. The review may be informative for oncologists, urologists, geneticists and specialists in related sciences. Среди мужчин в возрасте 30-59 лет рак почки (РП) занимает 4-е место по распространен-ности после рака легкого, кожи и желудка [1]. Около 95 % всех случаев РП представлены почечно-кле-точными карциномами (далее сокращение РП будет обозначать именно эти опухоли), остальные 5 % -опу холями почечной лоханки и мочеточника. РП раз-вивается из эпителия почечных канальцев, но, несмо-тря на общность происхождения, представляет собой морфологически гетерогенную группу опухолей.
