К двум годам приблизительно 50% детей переносят РСВИ дважды. В развитых индустриальных странах у детей в возрасте до 3 лет, госпитализированных с острыми респиратор-ными инфекциями (ОРИ) с поражением нижних отделов дыхательного тракта, частота обнаружения РСВИ дости- Respiratory syncytial virus (RSV) is a most important cause of viral acute respiratory tract illness especially in infants and young children. In industrialized countries RSV infections account for 42-63% of hospital admissions with lower respiratory tract infections in children younger than 3 years of age. Risk factors for increased mortality and morbidity during RSV infection were prematurity, young age below 12 months as well old age, blood and cardiopulmonary diseases, immunocompromised conditions. In children hospitalized with RSV infections mortality is estimated to be approximately 1%, although it is increased up to 37% in patients with burdened anamnesis. Among respiratory viruses, RSV is the most common cause of airway obstruction, particularly in children below 1 year of age. RSVassociated infection in infancy is associated with distant harmful consequences: increased risk for developing bronchial obstruction during following respiratory diseases, systemic allergic sensitization and asthma in later life. Despite its importance, there is no safe vaccine as well as no effective medicines to protect against RSV infection. This brief report describes the mechanisms contributing to immunopathology of RSV infection and present status of treatment and prevention of this disease.
The aim of this study was to compare the results of the hemagglutination inhibition test (HI-test) and microneutralization reaction in detection of antibodies to influenza A(H1N1), A(H1N1)pdm09, and A(H3N2) viruses in human sera, as well optimize microneutralization reaction conditions. The proposed variant of microneutralization reaction is based on detecting decreased influenza virus reproduction in infected MDCK cells in the presence of virus-specific serum antibodies. Virus reproduction was evaluated by enzyme-linked immunosorbent assay in 96-well cell culture plates using type-specific anti-influenza A viruse NP-protein monoclonal antibodies. In parallel, in microneutralization reaction and HI-test paired sera collected from 205 volunteers inoculated with inactivated seasonal influenza vaccines were analyzed, as well as from 117 adult patients with laboratory-confirmed influenza. The rationale for treatment of human serum with receptor-destroying enzyme (RDE) was proved not only for HI-test, but also for microneutralization reaction. Compared to HI-assay, the microneutralization reaction displayed higher sensitivity. According to microneutralization data, seroconversion rates and increase in antibody titer against influenza A viruses in both vaccinated and infected persons were superior to HI-test data by 1.4–2.5-fold. Moreover, higher sensitivity of this method was of great importance for the diagnostics of disease caused by new pathogens. The efficacy of influenza A(H1N1)pdm09 serodiagnostics in PCRpositive patients was 1.5 times higher based on microneutralization reaction vs. HI-assay data. According to the data from early studies, it is commonly believed that 1/40 titer of flu-specific antibodies detected by HI-test is set as protective. However, a consensus on protective level for virus-specific antibodies in neutralization reaction has not been established yet. It was found that serum antibody levels detected by the proposed version of microneutralization reaction were significantly higher than those in HI-assay. In the post vaccination sera collected from vaccinated volunteers, average titers of virus neutralizing antibodies corresponding to 1/40 in HI-test were 1/195, 1/203, and 1/ 426–1/430 for influenza A(H1N1), A(H1N1)pdm09 and A(H3N2), respectively, whereas in influenza patients they were 1/285, 1/215 and 1/488, respectively. Thus, it was suggested to consider a threshold value for “conditionally protective” level of neutralizing antibodies in adult vaccinated volunteers or infected patients, an average titer 1/160 for A(H1N1) and A(H1N1)pdm09 viruses, as well as 1/320 — for A(H3N2) virus, which agree with data published elsewhere.

Рассмотрен вопрос о регуляторных и протективных свойствах аутореактивных нормальных антител, а так-же их роли в формировании эффективного адаптивного противовирусного иммунного ответа. Представления о противовирусном иммунном ответе дополнились аутоиммунной составляющей, включающей антитела (АТ) к ан-тигенам (АГ) хозяина.У человека ранний гуморальный адаптивный иммунный ответ на чужеродные АГ наблюдает-ся со стороны IgМ, который является наиболее эволюционно консервативным классом имму-ноглобулинов (Ig), доставшимся млекопитаю-щим в наследство от хрящевых рыб. После син-теза IgМ, происходит переключение на синтез АГ-специфичных АТ классов G, A и E, что со-провождается их аффинным созреванием в ре-зультате соматических гипермутаций в генах Ig В-лимфоцитов. Помимо АТ, синтезирован-ных в процессе адаптивного иммунного ответа, в сыворотках людей изначально присутству-ют «нормальные» (natural) АТ, являющиеся не-отъемлемой составляющей врожденного имму-нитета и взаимодействующие с чужеродными АГ даже в тех случаях, когда организм никогда ранее не подвергался их воздействию. Нормаль-ные АТ секретируются главным образом долго-живущими В-лимфоцитами субпопуляции В1 (CD19 + CD5 + ), которые дифференцируются в те-чение внутриутробного и неонатального разви-тия и локализуются у взрослых людей в плевраль-ной и перитонеальной полостях. В-лимфоциты, обеспечивающие адаптивный иммунный ответ на чужеродные АГ, включая инфекционные аген-ты, относятся к субпопуляции В2 (CD19 + CD5 -). В1-и В2-лимфоциты отличаются филогенети-чески, фенотипически и функционально. Лим-фопоэз В2-клеток происходит в костном мозге, а иммуногенез -в фолликулах периферических лимфоидных органов. В2-лимфоциты проду-цируют высокоаффинные гипермутированные монореактивные АГ-специфичные АТ, синтез которых носит в основном Т-зависимый харак-тер. В отличие от них В1-клетки редко достига-ют герминативных центров, продуцируют АТ в основном Т-независимым путем, а их геном, как правило, не претерпева...
Разработан чувствительный вариант микрокультурального ИФА (cell-ELISA) для субтиповой дифференциации современных вирусов гриппа А(Н1N1)pdm09 и А(Н3N2), циркулирующих в человеческой популяции. Метод основан на оценке репродукции вируса в инфицированной культуре клеток MDCK c использованием на стадии детекции cубтип-специфичных моноклональных антител (mAb), взаимодействующих с гемагглютинином (HA) вируса гриппа. При отработке метода использованы штаммы вирусов гриппа A, выделенные из клинических образцов в эпидемический сезон 2014 года. Показано, что при использовании mAb #1/#2 или #4 в концентрации 10–15 мкг/мл разработанный вариант cell-ELISA позволяет детектировать синтезируемый в зараженных клетках белок HA соответственно вирусов гриппа А(H3N2) или A(H1N1)pdm09. Разработанный метод может быть использован для идентификации современных штаммов вируса гриппа А в процессе их репродукции в клеточной культуре MDCK, что позволяет проводить субтипирование вирусов с низкой гемагглютинирующей активностью, когда невозможно использовать общепринятый метод – реакцию торможения гемагглютинации (РТГА).
