Aim. To evaluate the expression level and prognostic significance of androgen in primary operable luminal breast cancer without overexpression of Her2/neu in postmenopausal women.
Methods. We analyzed treatment outcomes of 60 cases of primary operable (T1-2N0M0) luminal breast cancer without overexpression of Her2/neu in postmenopausal women. The follow-up period was 5 years. All cases were divided by immunohistochemical method into luminal A (20 females) and luminal B (40 patients) subtypes. Along with the standard panel of immunohistochemical markers, expression of nuclear androgen receptors was measured in tumor tissues of all patients. Depending on the expression levels, patients with luminal A and B subtypes were divided into three groups: (1) with high, (2) moderate and (3) low or negative expression.
Results. Mean levels of androgen receptor expression in the nuclei of tumor cells in patients with luminal A subtype (57.3±5.9%) were higher than those of luminal B subtype (21.4±4.04%) by 62.7% (Mann-Whitney test, р=0.0026). In patients with luminal A subtype, the maximal accumulation of androgen receptors in the nuclei of tumor cells was 2.7 times higher (р=0.0023) than in patients with luminal B subtype. All cases diagnosed with the disease progression were characterized by low or negative level of nuclear androgen receptor expression.
Conclusion. Negative and low levels of androgen receptor expression in tumor tissues of postmenopausal patients with luminal primary operable breast cancer without overexpression of Her2/neu might be an independent factor associated with poor prognosis.

У 40 % больных РМЖ встречается люминальный А-подтип опухоли, для которого характерны высокий уровень экспрес-сии рецепторов стероидных гормонов, низкая сте-пень злокачественности, низкая пролиферативная активность. Именно этот подтип ассоциируется с наи более благоприятным прогнозом. Люминальный В-подтип РМЖ встречается в 20 % случаев и харак-теризуется менее выраженной экспрессией рецепто-ров эстрогена (РЭ) и прогестерона (РП), повышен-ной пролиферативной активностью и худшим прогнозом по сравнению с люминальным А-подти-пом [2]. С учетом дороговизны и трудоемкости гене-тического профилирования опухолей в клинической практике для разделения РМЖ на молекулярно-би-ологические подтипы чаще всего используется имму-ногистохимическая характеристика новообразования [3]. Обычно с помощью иммуногистохимического метода определяются экспрессия РЭ и РП, индекс пролиферативной активности (Ki-67), наличие гиперэкспрессии HER2 / neu.На сегодняшний день обязательным компонентом системного лечения люминальных подтипов РМЖ является гормональная терапия (ГТ). Добавление адъю вантной химиотерапии (АХТ) к ГТ у пациенток c люминальным А-подтипом РМЖ в постменопаузе даже при положительных лимфатических узлах не приводит к существенному улучшению результатов лечения [4], однако принято считать, что у пациенток с люминальным В-подтипом добавление АХТ может принести дополнительную пользу и улучшить резуль-таты лечения [5].Чаще всего РМЖ диагностируют у женщин в воз-расте от 55 до 64 лет [6]. С учетом того, что у пациен-ток пожилого возраста происходит накопление зна-чимых сопутствующих патологий, проводимая им системная терапия может вызывать большее число осложнений по сравнению с терапией у более моло-дых пациенток [7]. Наиболее сложно выбрать опти-мальную тактику АХТ при первично-операбельном люминальном В-подтипе РМЖ без гиперэкспресии HER2 / neu у пациенток в постменопаузе. В таких случаях решение о добавлении химиотерапии к ГТ принимается, как правило, по усмотрению лечащего врача.Цель настоящего исследования -оценка результа-тов комплексного лечения первично-операбельного люминального РМЖ без гиперэкспрессии HER2 / neu у пациенток в постменопаузе в зависимости от прове-денной АХТ.

e12065 Background: Assessment of proliferative activity is important in terms of tumor prognosis. Topoisomerase IIα (TopIIα) and ki-67 are common markers of breast cancer (BC) cell proliferation. The purpose of the study was to assess the correlation between TopIIα and ki-67 expression as a prognostic factor in luminal, primary operable breast cancer without Her 2 neu overexpression in postmenopausal women. Methods: 60 postmenopausal patients with BC (invasive ductal carcinoma) were equally divided into 3 groups: I –luminal A BC, surgical treatment with subsequent radiotherapy and hormonal therapy; II – luminal B BC, surgical treatment with subsequent radiotherapy and hormonal therapy; III – luminal B BC, surgical treatment with subsequent radiotherapy, polychemotherapy and hormonal therapy. Mean age of patients: I – 76.16±3.14 years, II – 65.0±3.91, III – 74.0±2.54 years. Immunohistochemical study was performed with mouse monoclonal ki-67 (Thermo scientific) and TopIIα (Santa Cruz Biotechnology) antibodies using the Reveal Polyvalent HRP-DAB Detection System. Results were analyzed using the STATISTICA 10.0 program (StatSoftInc., USA). Results: The average expression levels of the ki-67 marker in the groups were: I - 12.3±1.4%, II - 15.1±3.1%, III - 37.0±7.1%; TopIIα expression – 3.5±1.6%, 16.0±5.9%, 21.7±6.4%, respectively. The statistically significant Spearman correlation between TopIIα and ki-67 was observed both as general in groups (r = 0.787 at p < 0.001) and in groups with luminal B subtype (groups II and III). Strong positive correlation between TopIIα and ki-67 was registered in groups II (r = 0.941 at p < 0.01) and III (r = 0.864 at p < 0.001) with luminal B subtype. Weak correlation was observed in group I with luminal A subtype (r = 0.477 at p≥0.05). Conclusions: The study revealed a correlative increase in the level of TopIIα with simultaneous overexpression of the ki-67 protein in the studied groups. This indicator can be considered predictive for breast cancer.
e12558 Background: Breast cancer (BC) is associated with miRNAs, the qualitative and quantitative analysis of which shows significant differences between molecular subtypes, as well as between miRNAs circulating in the bloodstream and detected in tumor tissue. Since there is no clear characteristics for markers of luminal B primary operable BC without Her 2 neu overexpression, our purpose was an evaluation of prognostic significance of miRNA-21, 221, 20a, 200a, 196a in such postmenopausal patients in order to create a diagnostic panel. Methods: The study included 120 samples of healthy tissues of patients with luminal A BC (group 1, mean age 76.16±3.14 years) and tumor tissues of patients with luminal B BC (group 2, mean age 69.5±3.2 years); invasive ductal carcinoma in all patients. The expression status was analyzed using 5 mioRNA markers associated, according to the literature, with various molecular subtypes of BC (miRNA-21, 221, 20a, 200a, 196a). The Thermo Scientific PureLink RNA Mini Kit was used for the total RNA isolation. The reverse transcription reaction was performed using the ImProm-II Reverse Transcriptase kit (Promega). The Livak/2 method was used to assess the relative expression of miRNAs. Results: The average relative expression was: miRNA-21 in group 1 – 0.2±0.01, group 2 – 1.5±0.05; miRNA- 221 in group 1 – 0.19±0.02, group 2 – 0.43±0.04; miRNA -20a in group 1 – 0.12±0.01, group 2 – 0.13±0.01, miRNA -200a in group 1 – 0.22±0.02, group 2 – 0.96±0.06; miRNA- 196a in group 1 – 1.06±0.02, group 2 – 1.56±0.04. The differences between the groups were statistically significant (p = 0.0001); the difference was statistically insignificant only for miRNA -20a (p = 0.159955). Conclusions: Results of the study allow considering the overexpression of miR-21, 221, 200a and 196a and the relatively low expression of miR-20a as diagnostic markers for a diagnostic panel for luminal B BC without overexpression of HER2/neu.
