Резюме. Корь остается одной из основных причин детской смертности в развивающихся странах и периодически приводит к возникновению крупных вспышек в разных странах. Эта проблема стала особенно актуальна после принятия ВОЗ стратегического плана по борьбе с корью. Согласно плану поставлена цель снижения кори в глобальном масштабе. В 2010-2011 гг. крупные вспышки кори регистрировались на Африканском континенте: в Демократической Республике Конго, на юге Африки, в Нигерии и в ряде других африканских стран. В Гвинейской Республике вакцинация против кори проводится однократно детям в возрасте 9 месяцев. В 2014-2015 гг. отмечен рост заболеваемости корью. Материалы и методы. В ИФА исследованы 22 сыворотки крови здоровых взрослых гвинейцев в возрасте от 24 до 71 года и 136 сывороток крови, полученных от детей и взрослых-пациентов госпиталя г. Кинди (Гвинейская Республика). Клинические образцы были получены в 2015-2016 гг. Использованы тест-системы производства Euroimmun Medizinische Labordiagnostika AG (Германия): «Anti-Measles Virus ELISA (IgМ)», «Anti-Measles Virus ELISA (IgG)»; «Аvidity: Anti-Measles Virus ELISA (IgG)», а также ИФА тест-система «Вектор-Бест IgM-корь» (Россия). Результаты и обсуждение. Только у одного из 22 обследованных не были выявлены IgG-антитела к вирусу кори. Количественное определение титров IgG-антител, а также их авидности у остальных обследованных (21 человек) свидетельствуют о перенесенном в недавнем или отдаленном прошлом заболевании корью. При исследовании 116 сывороток крови пациентов госпиталя г. Кинди на IgM-корь-антитела, ретроспективно выявлен случай кори у ребенка 2,5 лет. При исследовании 130 сывороток крови на наличие IgG-антител к вирусу кори, выявлены 12,3% серонегативных к кори лиц. Все обследованные в возрасте 23 года и старше были серопозитивны к вирусу кори, причем 60% из них имели высокие титры антител. Резюме. В обзоре изложены современные знания о морфологии и структурно-функциональной организации генома парвовируса В19, циркулирующих генотипах вируса. Дана клинико-эпидемиологическая характеристика парвовирусной инфекции (ПВИ). Представлены сведения о распространении ПВИ в разных регионах России и полученные авторами данные о превалентности инфекции в СЗФО РФ, в том числе в основной группе риска-среди беременных женщин. Описаны методы лабораторной диагностики постанатальной и врожденной ПВИ.
Abstract.The Western Pacific Region (WPR) is comprised of 37 countries such as China, Japan, Mongolia, Republic of Korea, The Socialist Republic of Vietnam, Papua-New Guinea, Australia, including Pacific Island Countries and Territories (21 countries of PICTs, approx. 3 million people) etc., with a population of 1.85 billion people. Among them, China is the largest and most populous (1.3 billion people) country of the Region. Large measles outbreaks were documented to occur in the Region. In 2003, the Regional Committee announced officially about the WPR action plan on measles elimination 2005, which, however, failed. Since 2012, WPR countries joined the WHO 2012–2020 Global Measles and Rubella Strategic Plan performing a routine measles vaccination (national immunization schedule) or within Expanded Programme on Immunization (EPI). Basically, a two-dose immunization strategy is followed in the WPR countries. Since 2002, measles supplementary immunization activities (SIAs) in children were conducted in the following countries: Japan, Laos, Vietnam, Philippines, Mongolia, Cambodia, Papua New Guinea, and China. Starting from 2005, measles management was considerably improved, demonstrating by 2012 decreased measles incidence rate down to 5.9 cases per million population. In last years, a decreased measles immunization coverage in decreed population groups was noted in the WPR countries that resulted in 2013–2015 measles epidemic involving almost all regional countries. In particular, in China measles incidence rate was 19.6 cases per million population, whereas in the Vietnam Papua New Guinea and Philippines it progressively increased reaching 182.8, 345.9 and 548.0 cases per million population, respectively. Early children not vaccinated according to schedule, adolescents and young adults dominated among measles patients. It was found that measles outbreaks were due to missed vaccination and increased level of vulnerability to measles. Children under one, adolescents and young adults who did not receive a two-dose measles vaccination were in risk group. Analyzing WPR measles epidemiology demonstrated that refusal of parents to vaccinate children, poor knowledge of advantages related to vaccination, insufficient immunization coverage in immigrants, travelers, subjects changing place of residence, workers of healthcare and educational facilities require special attention. In 2017–2018 season, the following measles genotypes were found in the WPR: D8 — Australia, New Zealand, Republic of Korea, Singapore, Japan; Н1 — China; В3 — Philippines, Australia and Japan; D9 — Singapore, Australia, Macau (China), Malaysia and Japan, Н2 strains endemic in Vietnam. According to the WHO, measles endemic transmission has been successfully interrupted; Australia, Macau, Mongolia and Republic of Korea are being verified to eliminate measles; Hong Kong (China) and Singapore (based on available information) are ready to verify measles elimination. Thus, in the Western Pacific Region measles elimination is achievable after solving current issues such as increasing and maintaining high-level routine vaccination and conducting measles supplementary immunization campaigns in epidemically important contact clusters.
Aim. Genotyping and molecular genetic characteristics of parvovirus B19, circulating in the NorthWest Federal District. Material and Methods. The material of the study is based on serum samples from 821 patients with maculopapular rash negative for antibodies IgM-measles and IgM-rubella were received at the St. Petersburg Regional Center for Measles and Rubella Surveillance in 2009-2017. In the present study we used genotyping by direct sequencing of the NS1/VP1 region of Parvovirus B19 genome. Results. DNA of the virus was detected in 59 (42.4%) of seropositive patients. To assess the heterogeneity of isolates, 14 samples were selected from patients from geographically remote regions of the NWFO, of different ages, regardless of gender, with a high viral load (106-107 copies/ml). Based on the phylogenetic analysis of the isolates showed that only the genotype 1A was detected in all isolates. The nucleotide sequences can be divided into two subgroups: 13 isolates (92.8%) belong to the subgroup 1A2, one isolate to the subgroup 1A1. Conclusion. The introduction of B19 screening from patients with fever / rash can provide meaningful information on the prevalence of parvovirus infection in the Russian Federation. Identifying new mutations of the virus and further analysis of their possible relationships with the course of the mutation disease can help in the development of medicines, as well as in the development of an effective vaccine against the parvovirus B19.
Резюме. Открытие новых парвовирусов привело к пересмотру таксономической структуры и номенклату-ры семейства Parvoviridae. Выделяют два подсемейства: Densovirinae и Parvovirinae. Подсемейство Densovirinae включает вирусы членистоногих. Подсемейство Parvovirinae объединяет вирусы позвоночных животных и включает в себя восемь родов. В данном обзоре будут рассмотрены парвовирусы, представляющие интерес для практического здравоохранения. Представлены данные о молекулярно-биологических и вирусологиче-ских характеристиках вирусов, эпидемиологических и клинических особенностях вызываемых этими ви-русами инфекций. Парвовирусы, изучение которых актуально для терапии различных заболеваний челове-ка, относятся к пяти родам: Bocaparvovirus, Dependoparvovirus, Erythroparvovirus, Protoparvovirus и Tetraparvovirus. Наиболее известным патогенным парвовирусом человека является вирус Primate erythroparvovirus 1, ранее называвшийся Human parvovirus B19, открытый в 1975 г. и относящийся к роду Erythroparvovirus. В данном об-зоре основное внимание уделено менее известным вирусам данного семейства. В структуре вирусных инфек-ций нижних дыхательных путей ведущее место занимают бокавирусы человека -Primate bocaparvovirus 1 и Primate bocaparvovirus 2, относящиеся к роду Bocaparvovirus. В 2005 г. у человека обнаружен вирус Primate tetraparvovirus 1, который относится к роду Tetraparvovirus. Клинически острые формы тетрапарвовирусной инфекции проявляются заболеваниями верхних и нижних дыхательных путей и пищеварительного тракта. Вирус обладает тератогенным действием. Представители рода Protoparvovirus вызывают заболевания пищева-рительного тракта. Описаны два вида, патогенные для человека: буфавирус человека (Primate protoparvovirus 1) был обнаружен в 2012 г., тузавирус (Primate protoparvovirus 2, Tusavirus 1) был выделен в 2014 г. Вид Rodent protoparvovirus 1, который объединяет вирусы преимущественно грызунов, имеет особое значение для тера-пии человека. Вирусы этого вида онкотропны, способны инфицировать широкий спектр раковых клеток че-ловека, обладают выраженными онколитическими свойствами и перспективны для лечения онкологических заболеваний. Вирус человека рода Dependoparvovirus, названный аденоассоциированным вирусом (Human adeno-associated viruses), был открыт при изучении препаратов аденовируса в 1965 г. Связь аденоассоцииро-ванного вируса человека с патологией человека неустановлена, однако вирус обладает способностью встраи-ваться в определенный локус 19 хромосомы человека и является перспективным кандидатом на роль вектора для лечения генетических заболеваний.
