Обследована 61 семья: пробанды и родственники I, II, III степени родства. Отбор семей осуществлялся по пробанду (дети в возрасте 3-16 лет) с внешними признаками дисплазии соединительной ткани. Всего обследовано 369 человек: 61 пробанд, 123 человека-родственники I степени родства, 164 человека-II степени, 21 человек-III степени. Клинико-генеалогический анализ структуры соматической патологии в семьях таких детей выявил тесную корреляционную связь хронической патологии у родственников и пробанда. Повышенный уровень аутоантител к коллагенам у детей и их родителей как маркер НСДСТ, вероятно, лежит в основе наследственной предрасположенности к формированию соединительнотканных нарушений у последующего поколения. Полученные результаты важны для разработки индивидуальных программ профилактики возможных осложнений при НСДСТ у детей. Selection of families was carried out by a proband (children at the age of 3-16) with external signs of connective tissue dysplasia. In total 369 persons are surveyed: 61 probands, 123 persons-relatives of I degree of relationship, 164 persons-II degree, 21 persons-III degree. Clinical and genealogical analysis of the structure of somatic pathology in the families of these children has revealed a close correlation connection of chronic pathology in the proband and relatives. The elevated level of autoantibodies to collagen in children and their parents, as a UCTDS marker, probably underlies the hereditary predisposition to the formation of connective tissue disorders in the next generation. The received results are important for the development of individual programs of prevention of possible complications in children with UCTDS.
Неонатальный сепсис (НС) -наиболее часто встречающееся заболевание среди новорожденных детей, особенно недоношенных. Ранняя диагностика сепсиса у недоношенных детей весьма затруднительна, поскольку его клинические проявления неспецифичны, а чувствительность и специфичность рутинных лабораторных анализов низки. Целью работы было установить значимость уровней сывороточного пресепсина, сывороточного амилоида А и ИЛ-18 для ранней диагностики неонатального сепсиса у недоношенных новорожденных. Обследован 161 новорожденный гестационного возраста 28-34 недели. Первую группу составили 122 недоношенных, родившихся от матерей с верифицированной генитальной и экстрагенитальной инфекционной патологией и потребовавших оказания реанимационного пособия в родильном зале или в первый час жизни, которым по данным клинико-лабораторного обследования был выставлен диагноз НС. В группу сравнения (вторая группа) были включены новорожденные того же гестационного возраста, у которых в последующем не наблюдалось развития НС. Уровень пресепсина (ПСП) у недоношенных с НС в первые часы после рождения был в 5,7 раза выше, чем в группе без НС, что позволило назначить антибактериальную терапию в первые сутки заболевания. Концентрации сывороточного белка А (SАА), ИЛ-18, СРБ и ПКТ имели высокие показатели на вторые сутки после рождения у недоношенных с НС, что свидетельствовало о более позднем их реагировании в сравнении с содержанием ПСП на развитие бактериальной инфекции. Таким образом, повышение концентрации ПСП может служить ранним диагностическим маркером бактериальной инфекции у недоношенных, так как его уровень повышается раньше, чем уровень традиционных маркеров воспаления (СРБ, ПКТ), а его высокие уровни свидетельствуют о тяжелом развитии и течении НС у недоношенных. Уровни SАА, ИЛ-18 увеличивались на вторые сутки развития НС, что не позволяет предложить их в качестве раннего маркера НС.
