Неонатальный сепсис (НС) -наиболее часто встречающееся заболевание среди новорожденных детей, особенно недоношенных. Ранняя диагностика сепсиса у недоношенных детей весьма затруднительна, поскольку его клинические проявления неспецифичны, а чувствительность и специфичность рутинных лабораторных анализов низки. Целью работы было установить значимость уровней сывороточного пресепсина, сывороточного амилоида А и ИЛ-18 для ранней диагностики неонатального сепсиса у недоношенных новорожденных. Обследован 161 новорожденный гестационного возраста 28-34 недели. Первую группу составили 122 недоношенных, родившихся от матерей с верифицированной генитальной и экстрагенитальной инфекционной патологией и потребовавших оказания реанимационного пособия в родильном зале или в первый час жизни, которым по данным клинико-лабораторного обследования был выставлен диагноз НС. В группу сравнения (вторая группа) были включены новорожденные того же гестационного возраста, у которых в последующем не наблюдалось развития НС. Уровень пресепсина (ПСП) у недоношенных с НС в первые часы после рождения был в 5,7 раза выше, чем в группе без НС, что позволило назначить антибактериальную терапию в первые сутки заболевания. Концентрации сывороточного белка А (SАА), ИЛ-18, СРБ и ПКТ имели высокие показатели на вторые сутки после рождения у недоношенных с НС, что свидетельствовало о более позднем их реагировании в сравнении с содержанием ПСП на развитие бактериальной инфекции. Таким образом, повышение концентрации ПСП может служить ранним диагностическим маркером бактериальной инфекции у недоношенных, так как его уровень повышается раньше, чем уровень традиционных маркеров воспаления (СРБ, ПКТ), а его высокие уровни свидетельствуют о тяжелом развитии и течении НС у недоношенных. Уровни SАА, ИЛ-18 увеличивались на вторые сутки развития НС, что не позволяет предложить их в качестве раннего маркера НС.
