Background. According to a report by the International Agency for Research on Cancer (IARC), lung cancer (LC) is among the leading causes of morbidity and mortality worldwide, with an estimated incidence of 14.1 million new cases of the disease and 8.2 million cancer deaths in 2012. Lung cancer is the most common cancer worldwide, accounting for 13 % of all new cancer cases and 19.4 % of deaths.The purpose of the study was to evaluate LC prevalence and to measure the quality of population-based cancer registries by the indices of the proportion of total incident cases.Material and methods. The study material was given from the monograph «Cancer on Five Continents», which included data from the database of the Cancer Registry of PA. Herzen Moscow Research Oncology Institute (St. Petersburg), surveys of morbidity and mortality in the North-West Federal District, estimations of proportion of the true incidence that was registered in population-based registries.Results. The analysis of lung cancer morbidity and mortality in Russia showed a significant improvement in analytical indices over the past 10 years, however, underestimation of primary cases, reduced the overall lung cancer incidence rate.Conclusions. The dynamics of age-specific lung cancer incidence was shown. The loss of primary lung cancer cases was estimated to be 15-20 % annually.
The most effective treatment of the hormone-dependent breast cancer is based on the antiestrogens SERM and aromatase inhibitor treatment, however its efficiency is limited by the acquired resistance to the drugs.Previously we have revealed the effect of the transferring of the hormonal resistance from the resistant to the sensitive cells under in vitro cell co-cultivation, and demonstrated exosomes involvement in this process. Here we have shown that the exosomes of the resistant cells caused the marked inhibition of the estrogen signaling in the recipient cells, and identified microRNAs – ERα suppressors that overexpressed in the resistant exosomes.Taken together, the results obtained demonstrate the important role of the estrogen signaling suppression in the exosome-induced transferring of the hormonal resistance, and revealed the involvement of the exosomal microRNA in the ERα down-regulation.
Cases of dental needle fracture during mandibular block were studied in the last 10 years. Despite dental anesthesia development, the cases of needle fracture occur regularly in the world practice. Some of these complications has iatrogenic nature while others may be attributed to independent reasons like patients' sharp movement and needle manufacturing defect. In most of the cases the needle migrates to surrounding anatomical spaces making operative removing a challenging task. Subsequent surgical removing of a foreign body has severe consequences to patient in the form of a large operating injury, long-term disability and functional limitations. The article presents an algorithm for prevention and emergency care for a patient with dental needle fracture during mandibular block.
Malignant tongue neoplasms (C01, 02) belong to the group of rare visual tumor localizations. Only data on the primary tongue cancer registration can be obtained by the state statistics, mortality rate data are summarized and presented in the list of 14 headings of ICD-10-C00-14 - malignant neoplasms of lip, oral cavity and pharynx, it is impossible to obtain any analytical characteristics at the All-Russian or regional level (the distribution of patients by stages of the disease, mortality of patients in the first year of observation, accumulated patient population, etc.). This information can be obtained only from the databases created by the Population-based Cancer Registries (PCR) of a country. Aim. The aim is to study the prevalence of tongue cancer and the detailed structure of morbidity, to calculate a number of analytical indicators characterizing tongue cancer at the population level of the Federal District for the first time in Russia. Materials and methods. Based on the materials of the International Agency for Research on Cancer (IARC), statistical reference books of the P. Hertsen Moscow Oncological Research Institute, our own extensive materials and the newly created PCR database at the level of the Northwestern Federal District of Russia, to study all the possible aspects set in the research objectives, using a set of methodological techniques recommended by the IARC, of which we are a member No. 221. Results. The study has shown that tongue cancer morbidity rate of the population (standardized indicator the world standard) in the various countries from 1970 to 2010 has increased slightly or maintained its level. In Russia, such an assessment can be carried out only from 1989 to 2018. During this period, the incidence of tongue cancer among males has increased from 2.2 to 2.43 0/0000, among females from 0.4 to 0.71 0/0000. In the Northwestern Federal District of the Russian Federation, the incidence of males has reached 2.69, females 0.82 0/0000. The mortality rate of patients in the first year of observation (PCR database) has decreased from 2000 to 2018 from 59.5 to 43.1%, but remains significantly above the EU average.
According to the literature review, the use of modern endoscopes with high resolution and a narrow-band imagine function with optical magnification, as well as autofluorescence, chromoscopy, endosonography, makes it possible to establish a diagnosis of early colorectal cancer with a high degree of accuracy. Nowadays, endoscopic submucosal dissection and endoscopic mucosal resection are the methods of choice in the treatment of early (Tis, T1sm1N0M0) colon cancer.
Introduction. We have previously shown the feasibility of hormonal resistance horizontal distribution from cell to cell, with the joint cultivation of sensitive and resistant cells and/or through exosomes secreted by resistant cells. What is the mechanism of such resistance distribution, and how do cells with secondary resistance reproduce the characteristics of donor resistant cells? To answer these questions, we analyzed the overall level of DNA methylation in MCF-7 estrogen-dependent breast cancer cells and estrogen-independent sublinia.The purpose of the study was to analyze DNA methylation profiles for the development of hormonal resistance by breast cancer cells and for resistant phenotype further accession.Methods. DNA methylation was evaluated by the RRBS (Reduced Representation Bisulfite Sequencing) method in MCF-7 breast cancer cells and their resistant sublines.Results. 19 CpG dinucleotides, differentially and generally unidirectionally methylated in cells with primary and secondary resistance to tamoxifen, were detected. Differential changes in methylation were found for DNA regions that regulated the expression of six protein-coding genes: PRKCZ, TRAPPC9, AS IC2, C2CD4A, ZNF787, CRTAC 1. Bioinformatics analysis showed that two of these six genes, PRKCZ (protein kinase C Zeta) and TRAPPC9 (Trafficking Protein Particle Complex Subunit 9) were directly involved in the regulation of NF-κB activity.Conclusion. The data obtained indicate the existence of common DNA patterns, the methylation of which varies in the same direction in cells with primary and secondary resistance. The involvement of two of the identified genes in the regulation of NF-κB may indicate the inclusion of the latter in the formation of a resistant phenotype of tumor cells, even under conditions of horizontal transfer of resistance.
Рак прямой кишки (РПК) - это злокачественная опухоль дистального отдела толстой кишки эпителиального происхождения. Ежегодно в России регистрируется более 30 тыс. (30 969 в 2018 г.) новых случаев РПК и 16 тыс. (16 151) случаев смерти. Индекс достоверности учета (отношение числа умерших к заболевшим) составляет 0,52 %. РПК относится к локализациям со средним уровнем летальности. В Северо-Западном Федеральном округе зарегистрировано более 3 тыс. первичных случаев РПК (3 285 в 2018 г.). Государственная статистика РПК представляет данные о заболеваемости и смертности населения по трем рубрикам МКБ-10 вместе (С19-21). Детальная разработка данных не только отдельно по каждой рубрике МКБ-10, но и по четвертому знаку возможна только по материалам баз данных раковых регистров. Созданный нами Популяционный раковый регистр Северо-Западного Федерального округа насчитывал на 01.01.2019 г. более 1 млн (1 067 661) случаев злокачественных новообразований (ЗНО), в том числе РПК - 50 745 случаев, или 4,8 %. Удельный вес РПК близок к среднероссийскому - 4,9 %. В работе представлена детальная структура онкологической патологии по РПК. Ведущая роль из трех указанных рубрик принадлежит ЗНО прямой кишки (С20) - 75,4 %; 19,8 % приходится на ЗНО ректосигмоидного отдела (С19) и только 4,7 % на С21 - ЗНО ануса и анального канала. По всем этим рубрикам представлена динамика структуры, которая оказалась достаточно устойчивой. Rectal cancer is a malignant tumor of the distal colon of epithelial origin. Every year, more than 30,000 (30 969 - 2018) new cases of Rectal cancer and 16,000 deaths (16 151) are registered in Russia. The index of reliability of accounting (the ratio of the number of dead to sick) is 0,52 %. Rectal cancer refers to localities with an average mortality rate. In the North-Western Federal district, more than 3 thousand primary cases of Rectal cancer were registered (3285 - 2018). The state statistics of the Rectal cancer provides data on morbidity and mortality in three categories of ICD-10 together (C19-21). Detailed development of data not only separately for each category of ICD-10, but also for the fourth sign is possible only from the data bases of cancer registers. As of 01.01.2019, the Population cancer register of the North-Western Federal district created by us totaled more than 1 million cases of malignant tumors (1 067 661), including Rectal cancer - 50 745 cases, or 4,8 %. The share of the Rectal cancer is close to the national average - 4,9 %. The article presents a detailed structure of cancer pathology in Rectal cancer. The leading role of the 3 specified categories belongs to the rectal malignant tumors (C20) - 75,4 %, 19,8 % falls on the malignant tumors of the rectosigmoid department (C19) and only 4,7 % on the C21 - malignant tumors of the anus and anal canal. For all these categories, the dynamics of the structure is presented, which proved to be quite stable.
