Ревматоїдний артрит (РА) -це хронічне системне запальне аутоімунне захворювання, яке в першу чергу вражає дрібні суглоби тіла. Незважаючи на те, що він може впливати на багато систем в організмі, його основні симптоми пов'язані з синовітом та руйнуванням хряща в суглобах [1].Патогенез цього захворювання пов'язаний з 2 основними процесами, які йдуть рука об руку: утворення антитіл, а потім -запалення. Вважають, що антитіла з'являються в крові за 3-5 років до того, як РА стає симптоматичним, атакуючи багато цитрулінованих пептидів та білків. Поточний патофізіологічний процес підтримується підвищеними рівнями антитіл та маркерами запалення, наприклад, таким як рівень С-реактивного білка у сироватці крові [1, 2]. Ефектом цього запального процесу при РА є ураження суглобів. Однак еволюція медицини та медичного лікування, наприклад, застосування препаратів проти фактора некрозу пухлин (anti-TNF) та модифікованих захворюваннями протиревматичних препаратів зменшили потребу в ендопротезуванні суглобів [3,4,5]. Ендопротезування суглобів є стандартною рекомендацією щодо лікування болю, спричиненого пошкодженням суглобів та деформацією, що не піддаються консервативному лікуванню [6]. Aim – to determine the effect of drug antirheumatic therapy on the functional outcomes of hip and knee arthroplasty in patients with rheumatoid arthritis. Materials and methods. The results of 160 operations of total hip arthroplasty and 148 operations of total arthroplasty of knee joints in patients with rheumatoid joint lesions were analyzed using mathematical methods of statistical processing. Patients were divided into 4 groups: Group I – without anti-inflammatory therapy, Group II – received hormone therapy, Group III – basic therapy and Group IV – received both the basic and hormonal therapy at the time of surgery. Results. The obtained results indicated a moderate correlation between therapy and the joint range of motion of the lower extremities using basic therapy and combined nonsteroidal, hormonal and basic therapy, and the range of motion differed significantly in combination therapy from hormonal alone. The highest scores of the J. Joseph, E. E. Kaufman rating scale were observed in combination therapy with NSAIDs, hormonal and basic therapy; the lower scores – in hormonal and NSAIDs combination; and the lowest ones – in the combination of basic therapy with NSAIDs. The greatest increase in the knee joint range of motion was observed in the patients who received hormonal drugs or basic therapy compared to the patients who did not take any one or received both of these medicines in combination. Almost the same effect of antirheumatic therapy on the results of both hip and knee arthroplasty was determined with an upward trend in percentage of good and excellent results in males regardless of the lesion location and the most optimal ratio between satisfactory-unsatisfactory and good-excellent results in the patients under 40 years of age. Conclusions. The studies conducted allow to consider the combined drug therapy as a factor of weak and moderate influence on the functional outcomes of hip and knee arthroplasty in the late stages of rheumatoid arthritis and suggest that antirheumatic drug therapy should not be discontinued in the perioperative period.
Objective. To estimate the impact of remodelling and microbial factors on the results of total endoprosthesis (ТЕP) of the knee joint in patients, suffering rheumatoid arthritis (RА). Маterials and methods. The results of microbiological investigations of operative specimen were compared with results of cloning of the stem stromal cells of the bone marrow. Міcrobiologically and microscopically there were studied 247 specimen of operative material, obtained during performance of primary TЕP of a knee joint in 64 patients, suffering RA. Results. As the investigations have shown, in 1 сm³ of spongious bone of distal femur there is in 1.4 times more a general quantity of cells, containing the nuclei, than in 1 сm3 of spongious bone of proximal tibiae (the difference is not statistically significant). In distal part of femur the quantity of a colony-creating units of fibroblasts (CCUF) in 1 сm³ is in a 9.8 times lower, than the CCUF quantity in 1 сm³ in proximal part of tibiae (the difference is statistically significant), аnd the efficacy of the CCUF cloning is accordingly more than in 2.1 times lower (difference is statistically nonsignificant). Conclusion. High level of contamination of predominantly gram-positive microorganisms of the bone operative material, obtained while primary TEP of a knee joint in 82.8% patients, suffering RA, have a strong (almost a functional one) linear adverse correlation (coefficient r = -0.98) with osteogenic аctivity of the stem stromal cells in the bone marrow, which form a knee joint, what necessitates overestimation of approaches, concerning preparation and performance of surgical intervention for guaranteeing the efficacy of orthopedic treatment and standards of the protocols.
Стан репаративної регенерації кісткової тканини при ураженні кульшового суглоба у хворих на ревматоїдний артрит та анкілозивний спондиліт Резюме. У статті подано аналіз показників репаративної регенерації 67 зразків спонгіози кісток, що утворюють кульшовий суглоб, 26 хворих на ревматоїдний артрит і 30 зразків спонгіози кісток, що утворюють кульшовий суглоб, 18 хворих на анкілозивний спондиліт, яким виконано ендопротезування кульшового суглоба. Проведені нами дослідження виявили суттєві відмінності щодо ефективності клонування колонієутворюючих одиниць фібробластів серед 10 5 ядерних клітин кісткового мозку спонгіози вертлюжної западини, головки й міжвертлюжної ділянки стегнової кістки. Отримані дані об'єктивно відображають стан репаративної регенерації кісткової тканини в різних ділянках кульшового суглоба в цієї складної категорії хворих. State of reparative bone regeneration in lesions of the hip joint in patients with rheumatoid arthritis and ankylosing spondylitis Abstract. This article provides an analysis of indicators of reparative regeneration of 67 samples of spongiosis of bones forming the hip joint in 26 patients with rheumatoid arthritis, and 30 samples of spongiosis of bones forming the hip joint in 18 patients with ankylosing spondylitis, who underwent hip replacement. Our studies have revealed significant differences in the efficiency of cloning fibroblast colony-forming units among 105 nucleated cells in bone marrow spongiosis of the acetabulum, head, and intertrochanteric area of the femur. The data obtained objectively reflect the state of reparative regeneration of bone tissue in various parts of the hip in this complex group of patients.
