Цель. Профилактика инфекционного процесса в области реципиентной раны при свободной кожной пластике расщепленным трансплантатом.Материалы и методы. Разработан способ иммобилизации бактериофагов в области аутодермотрансплантата путем перевода раствора, содержащего бактериофаги, в гелевую форму. Выполнены микробиологические и клинические исследования эффективности предложенного способа.Результаты. Подтверждена жизнеспособность бактериофагов в гелевой повязке в сроки до 4 сут и снижение вероятности развития местных инфекционных осложнений при кожной пластике.Заключение. Гелевая композиция, содержащая бактериофаги, позволяет оперативно реагировать на изменения актуальной госпитальной микрофлоры и эффективно противодействовать опасности нозокомиального инфицирования. Aim. To improve the results of surgical treatment in patients with large lipomas by using the device for closing the edges of wounds, allowing local preventive hemostasis in the surgical wound area, to create optimal conditions for atraumatic suturing and thereby reduce the amount of intraoperative bleeding and subsequently reduce the amount of postoperative wound complications. Methods. The results of treatment of 105 patients with different localization of large lipomas were analyzed. Lipomas ranged from 8.0 to 20 cm in diameter. All patients were divided into 2 groups: the control group consisted of 53 (50.5%) patients for which used the traditional technique of tumor excision with closing surgical wounds, and the study group with 52 (49.5%) patients, for which to reduce tissue damage during tumor removal and the edges of wound suturing, one of the device variants developed by us was used, approximation-distraction device. Fisher's exact test was used for the comparison of the outcomes rate, a p-value 0.05 was considered statistically significant. Results. Surgical wound complications occurred in 7 (6.7%) of 105 patients. In the study group, surgical wound complications occurred in 1.9% (1 observation), in the control group 11.3% (6 observations). Surgical wound complications were severity grade I and grade IIIa. Complications of the first grade did not require a debridement and will regress on its own; in grade IIIa complications, an opening, excision and drainage surgical wounds were performed without general anesthesia. Conclusion. Moderate local selective near-tumor tissue compression that occurs when the device branches come closer to each other accelerates spontaneous thrombus formation in damaged small vessels. This creates favorable conditions for the surgeon to work in a dry wound.

Оценить клиническую эффективность разработанных адаптационных инструментов для механической обработки раневой поверхности и ушивания релапаротомных ран. Методы. Проведено лечение 55 больных в возрасте от 43 до 79 лет с релапаротомиями, оперированных по поводу различных острых хирургических заболеваний органов брюшной полости. В основной группе (n=22, средний возраст 63,5±4,75 года) при неосложнённом и осложнённом течении раневого процесса после релапаротомии для обработки раневой поверхности и ушивания раны использовали предложенные авторами адаптационные инструменты. В группе сравнения (n=33, средний возраст 62±6 года; p=0,98) применяли традиционные механические способы санации раневого дефекта и ушивания раны -простые узловые, 8-образные и П-образные швы. Мониторинг характера заживления релапаротомной раны осуществляли с помощью ультразвукового метода исследования. Анализ послеоперационных местных осложнений в обсуждаемых группах больных проводили согласно классификации Клавьен-Диндо. Результаты. У пациентов основной группы кратность обработки раневой поверхности уменьшилась в 1,8 раза по сравнению с группой сравнения. Отсутствовали случаи повреждения глубжележащих тканей краёв раны. Раневые послеоперационные осложнения соответствовали I, II, IIIа степени тяжести (severity grade) по классификации Клавьен-Диндо: в основной группе -27,3%, в группе сравнения -54,5%. Нагноения зарегистрированы соответственно у 9,1 и 33,3% больных (p=0,049). Вывод. Разработанные адаптационные инструменты служат безопасными и эффективными механическими средствами профилактики раневых послеоперационных осложнений после релапаротомии.
