In the article there are presented some epidemiological and epizootological data on brucellosis infection in the territory of the Omsk region in 2010-2014. There is specified the territorial spread of disadvantaged points for brucellosis of animals in the Omsk region. There is presented the incidence rate of newly diagnosed brucellosis cases in the region in the comparison with similar indices for Russia and the Siberian Federal District (SFD). The excess in the morbidity rate in the region in the comparison with the index on Russia in 2010 was shown to be by more than 2 times (0.7 and 0.3 per 100 thousand of population, respectively) with practically comparable indices for the region (0.15-0.35), in Russia (0.24-0.34) and SFD (0.22-0.39) over the subsequent 4 years. There are presented data on clinical pathogenetic variants in patients with newly diagnosed brucellosis for the indicated period. There was noted a significant excess of chronic brucellosis (in 20 of 35 patients) over other clinical and pathogenetic variants of infection. Based on the analysis of clinical and laboratory data in patients who were on the treatment in the D.M. Dalmatova Infectious Clinical Hospital No 1 in 2010-2014 years (N = 205), clinical characteristics of chronic and residual brucellosis with the specification of leading syndromes and symptoms at the present stage are presented.

Молекулярная генетика открывает новые горизонты в современной медицине, особенно сейчас, когда многим заболеваниям уделяется огромное значение в виду их распространенности среди различных слоев населения. Чрезвычайно высокой степенью полиморфизма обладают гены интерлейкинов, из которых достаточно широко изучен полиморфизм фактора некроза опухоли. Пациенты с ВИЧ-инфекцией на территории России в настоящее время обуславливают самую высокую степень летальности, что является наиболее актуальной и социально значимой проблемой здравоохранения. Изучение данной проблемы привлекает многих исследователей. Особенно интересны работы в плане генетической невосприимчивости к вирусу и влияния продукции цитокинов на прогноз заболевания. Одним из факторов, влияющих на репликацию ВИЧ в организме, являются цитокины. Некоторые из них, в том числе фактор некроза опухоли и интерлейкин-6, могут способствовать репликации ВИЧ, повышая экспрессию регуляторных генов вируса. По мере прогрессирования заболевания параллельно нарастанию уровня противовоспалительных цитокинов, обусловливающих относительно малоэффективное в данном случае нарастание уровня антител, происходит угнетение T-хелперного ответа, стимулирующего сильный клеточный компонент. Функционирование цитокиновой сети при ВИЧ-инфекции зависит от многих причин, в число которых входят индивидуальные различия в продукции цитокинов, обусловленные рядом генетических особенностей, наличие оппортунистической инфекции. Изучение цитокинов у пациентов с ВИЧ-инфекцией в клинической практике является необходимым для оценки прогноза течения болезни к неблагоприятному быстрому переходу в СПИД, что важно учитывать в выборе тактики поддерживающей терапии ВИЧ-инфицированных пациентов. Учитывая недостаточную эффективность современных методов лечения, восстановление и модулирование баланса цитокинов усилит антивирусную активность иммунной системы, влияя на факторы, блокирующие репликацию вируса иммунодефицита человека.
The review is dedicated to features of clinical manifestations of infections caused by Epstein-Barr virus (EBV) in HIV-infected patients, problems of diagnostics and execution of antiviral therapy in the case of combination of these infections. Individuals at AIDS stage develop tumors, associated with EBV: non-Hodgkin’s lymphomas, including Berkitt’s lymphoma, primary B-cell lymphoma ofCNS, nasopharyngeal carcinoma. Formation of lymphoid interstitial pneumonitis and leukoplakia is known to be associated with EBV. A large list of preparations that are inhibitors of EBV replication are currently known, however, there is no clear pathogenetically justified therapy scheme for patients with this infection against the background of HIV-infection.
