The objective of this study was to evaluate the vasculomotor function of endothelium in patients with type 2 diabetes mellitus and assess the role of its functional state in the development of ischemic episodes. A total of 93 patients (52 men and 41 women) of the mean age of 58.3+-4.8 years were involved in the study. Group 1 comprised 47 patients with coronary heart disease (CHD) and type 2 diabetes, group 2 included 46 patients with CHD in the absence of disturbances of carbohydrate metabolism. Patients of the two groups were matched for age, gender, and major risk factors. Their comprehensive examination included 24 hour ECG monitoring, veloergometry, echocardiography, and reactive hyperemia test (ultrasound evaluation of endothelium-dependent dilation of the brachial artery). The patients of group 2 showed longer total duration of episodes of myocardial ischemia, the elevated number of painless (PMI) episodes, and greater maximum depression of ST-segment compared with CHD patients having no disturbances of carbohydrate metabolism. Correlation analysis demonstrated significant negative relationship between endothelial dysfunction, the number and duration of PMI episodes, and delay of pain syndrome with respect to ischemic depression of ST-segment in patients of group 1.

Сочетание гипергликемии, гиперинсулинемии и инсулиноре-зистентности является пусковым механизмом акти-вации окислительного стресса и эндотелиальной дисфункции, ускоряет развитие и прогрессирова-ние заболеваний, связанных с атеросклерозом и за-нимающих ведущее место среди причин смертности населения в индустриально развитых странах [2]. К ранним маркерам поражения миокарда при АГ относится диастолическая дисфункция левого же-лудочка (ДД ЛЖ). Именно диастолическая функция миокарда определяет функциональный резерв серд-ца и толерантность к нагрузкам [3], и ее снижение повышает риск сердечно-сосудистых осложнений; прогрессирование хронической сердечной недоста-точности (ХСН) возможно при наличии нормаль-ной фракции выброса ЛЖ. Сочетание АГ и СД дик-тует необходимость выяснения процессов, влияю-щих на ремоделирование миокарда и обмен веществ.Мы оценивали клинические и гемодинамиче-ские показатели при лечении АГ антагонистом ре-цепторов ангиотензина II (АРА) олмесартаном у больных СД2 с диастолической сердечной недоста-точностью.
Материал и методыВ открытом контролируемом исследовании на-блюдали 56 больных (26 мужчин и 30 женщин) с признаками ХСН I-II функционального класса (ФК) по классификации NYHA и нарушением диа-столической функции ЛЖ по типу аномальной ре-лаксации при фракции выброса ЛЖ более 50%. Средний возраст больных -58,2±5,3 года. Обяза-тельным условием включения было наличие СД2 длительностью не более 15 лет с момента установле-ния диагноза на фоне приема пероральных сахаро-снижающих препаратов. Aim. To assess the role of vasomotor dysfunction and impaired flowdependent vasodilatation as a risk factor of silent myocardial ischemia (SMI) in patients with Type 2 diabetes mellitus (DM-2).Material and methods. The study included 128 patients (66 men and 62 women; mean age 59,3±4,7 years), who underwent Holter ECG monitoring, stress test (Bruce protocol), echocardiography, vascular ultrasound, and brachial artery endothelium-dependent vasodilatation (EDVD) assessment.Results. Patients with coronary heart disease and DM-2 demonstrated reduced EDVD levels (3,7±1,1%) and highly prevalent SMI (93,3%). There was a correlation between EDVD and the number (r=–0,68; p<0,05) or duration of SMI episodes (r=–0,53; p<0,01).Conclusion. Patients with DM-2 require the assessment of vasomotor endothelial function in order to predict cardiovascular complications. Coronary artery endothelial dysfunction, which manifests in inadequate vasodilatation in response to increased myocardial demand for oxygen, plays an important role in the ischemia development and progression.
We investigated the relationship between the severity of silent myocardial ischemia and the functional state of arteries endothelium. We studied the significance of violations of vasomotor function with the loss of the ability to flow-dependent vasodilation as a risk factor, which increases the probability of the episodes of silent myocardial ischemia among patients with Type II DM. We observed 128 patients (66 male and 62 female), average age was 59,3±4,7. Each patient signed an agreement to take part in our research as a volunteer. The agreement was adopted by the local ethics committee.Patients with coronary heart disease and Type II DM had dominating daily duration of episodes of myocardial ischemia and a maximum depth of ST segment depression.
