Реферат Плано-вальгусные деформации стоп имеют ригидные и мобильные формы в зависимости от подвижности суставов заднего отдела стопы. Ригидные деформации, как правило, приводят к развитию болевого синдрома. Наиболее частой причиной развития ригидных плано-вальгусных деформаций стоп является тарзальная коалиция. Определение степени подвижности суставов предплюсны производится на основании специальных клинических тестов. Цель исследования-определение диагностической значимости клинических тестов мобильности стопы при дифференциальной диагностике плано-вальгусных деформаций стоп. Результаты. Получены следующие значения: чувствительность теста «вставания на цыпочки» и теста Jack составила 87% и 85%, специфичность-65% и 64% соответственно. Чувствительность мануального определения пассивной эверсии и инверсии стопы составила 80%, специфичность-88%. Последний тест обладает более высокой специфичностью по сравнению с тестами «вставания на цыпочки» и Jack. Выводы. При оценке мобильности стопы в 1 балл по любому из описанных тестов вероятность тарзальной коалиции составляет 80% и более, что служит поводом для продолжения диагностического поиска и перехода на следующий этап диагностики, включающий рентгенологическое исследование.
Background. Despite the success in the treatment of children with mucopolysaccharidoses (MPS) as a result of the widespread of enzyme replacement therapy and hematopoietic stem cells transplantation, orthopedic manifestations continue to be a significant problem, while the pathology of the upper limbs in children with MPS is not sufficiently represented in the literature. The aim of this studywas to analyze orthopedic and neurological manifestations in the upper extremities of children with mucopolysaccharidosis based on a sequential case series. Materials and Methods. We carried out a comprehensive analysis of clinical and radiological involvement of the upper extremities in 49 patients with MPS. Results. The most common complaints reated to the upper extremities were difficulties in the daily activities (dressing, self-care, playing), impairment of the fine motor skills, and muscle weakness. The most frequent clinical manifestations related to the upper extremities were limited active shoulder abduction, impaired hand grip, flexion contractures of the elbow joint, ulnar deviation of the hand. All patients with MPS types I, II, and VI had limited active and passive extension and flexion of the metacarpophalangeal and interphalangeal joints. In patients with MPS IV, hypermobility of the hand joints prevailed. We noticed minimal presence of typical clinical manifestations related to compression of the median nerve secific for carpal tunnel syndrome. The majority of patients showed a decrease in tendon and periosteal reflexes. The most pronounced decrease in muscle strength was observed in to extensors (elbow, fingers) and shoulder abductors, which may contribute to the predominant formation of a flexion pattern of contractures. On radiographs of the hand, the “melting sugar” symptom and shortening of the metacarpal bones were observed in most patients. Conclusion.Clinical and radiological manifestations related to the upper extremities take place in all types of the mPS, and lead to functional disorders that complicate daily life and self-care. Upper limb pathology in children with MPS requires earlier detection and more active treatment after comprehensive risk assessment.

Для определения величины деформации, ее вершины, а также степени необходимой коррекции используют различные методы расчета. Цель исследования-оценить воспроизводимость основных рентгенометрических параметров, характеризующих деформации нижних конечностей во фронтальной плоскости у детей с системными дисплазиями скелета, на основании расчета межэкспертной надежности (inter-rater reliability) Материал и методы. В статье представлены расчеты основных ангулометрических параметров коленного сустава (девиация механической оси, соотношение между осями и суставными линиями) у 18 пациентов с системными дисплазиями скелета (30 нижних конечностей) с угловыми деформациями нижних конечностей во фронтальной плоскости на уровне коленного сустава (основная группа). В контрольную группу включены результаты аналогичных расчетов ангулометрических параметров у 19 детей (30 нижних конечностей) с вальгусными и варусными деформациями коленного сустава без системных дисплазий скелета (идиопатические угловые деформации, посттравматические деформации, пороки развития нижних конечностей). произведена оценка межэкспертной надежности (interrater reliability) в программе SpSSv. 23. Результаты. проведенный анализ показал, что расчет референтных показателей у детей с системными дисплазиями скелета внутриклассовый коэффициент корреляции для отклонения механической оси составил 0,861% [0,763-0,926]. это соответствует нормальной воспроизводимости данного параметра: для мпгу-0,586% [0,295-0,781], для мДБу-0,796% [0,653-0,892]. при расчете этих же показателей у детей контрольной группы внутриклассовый коэффициент корреляции с 95% доверительным интервалом (Icc [95% cI]) при расчете референтных углов мпгу, мДБу составил 0,981% [0,971-0,991] и 0,993% [0,989-0,997] соответственно, для ДМо-0,996% [0,993-0,998]. Заключение. при расчете выраженности угловых деформаций нижних конечностей у детей с системными дисплазиями скелета наиболее воспроизводимым является показатель отклонения механической оси нижних конечностей по сравнению с исследуемыми ангулометрическими параметрами.
