Цель: анализ применения эндоскопических методов диагностики и лечения билиарных осложнений после ортотопической трансплантации печени, их эффективности и безопасности. Материал и методы. В основу данного исследования легли результаты эндоскопического лечения 29 больных с билиарными осложнениями после ортотопической трансплантации печени, возникшими в период с декабря 2001 по январь 2017 г. в НИИ СП им. Н.В. Склифосовского. Выводы. Современные методы лечебной эндоскопии позволяют в большинстве случаев успешно пролечивать билиарные стриктуры и несостоятельность билиобилиарных анастомозов и тем самым достигнуть хороших отдаленных результатов малоинвазивным способом. Research Institute of emergency medicine named after N.V. Sklifosofskiy Представлен анализ результатов использования фибробронхоскопии у больных с закрытой сочетанной травмой груди. Исследование показало, что у 51,6% пациентов закрытая травма груди сопровождается множественными переломами ребер с одной или обеих сторон с наличием гидро-, пневмоторакса или их сочетанием. Это значительно затрудняет рентгенологическую диагностику внутрилегочных повреждений. Фибробронхоскопия (ФБС) является прямым методом диагностики повреждения бронхов и легких, что подтверждается совпадением данных ФБС с интраоперационными на-ходками и результатами аутопсии и влияет на выбор хирургической тактики. Однако информативность однократного эндоскопического исследования при травматических разрывах бронхов на фоне продолжающегося легочного кровоте-чения недостаточно высока. Одновременно ФБС является и лечебным мероприятием, которое позволяет в большинстве случаев добиться уменьшения интенсивности легочного кровотечения вплоть до полной его остановки. Ключевые слова: закрытая травма груди, диагностика, фибробронхоскопия.Analysis of the results of fibrobronchoscopy in patients with concomitant closed chest trauma is presented. The study showed that in 51.6% of cases closed chest trauma is accompanied by multiple fractured ribs on one or both sides with the presence of hydro-, pneumothorax, or a combination of them. This greatly complicates radiological diagnosis of pulmonary lesions. Fibrobronchoscopy is a direct method of diagnosis of bronchi and lungs injury that is proved by conjunction of fibrobronchoscopic data with intraoperative findings and autopsy results and affects on the choice of surgical approach. However, sensitivity of single endoscopy for traumatic bronchi rupture is low because of ongoing pulmonary hemorrhage. Along with that fibrobronchoscopy is a therapeutic procedure that allows in majority of cases obtaining reduction of the intensity of pulmonary hemorrhage up to its arrest.
Aim of the study to evaluate the impact of the maternal endothelial dysfunction during the first trimester of pregnancy on the health state of her infant. Material and methods. The study included 163 babies delivered by 163 pregnant women. In all pregnant women, during the first trimester of the pregnancy, the endothelial dysfunction was assessed by means of cytometric examination of desquamated endothelial cells in the peripheral blood. There were studied the somatic health state of pregnant women and their 163 babies during the first year of life. Results. In groups of children from mothers with detected endotheliopathy by a decrease in the average diameter of desquamated endothelial cells in the first trimester of pregnancy there were observed significantly more frequently encountered disorders of the central nervous system, a higher rate of psychomotor retardation. In babies from such mothers there were also often diagnosed infectious-inflammatory diseases in the first year of life and significantly increased the incidence of iron deficiency anemia. Discussion. Endothelial dysfunction at early pregnancy causes the impaired formation of the uterine-fetal relationship, which is reflected in the formation of organ systems of the fetus, and leads to the development of neonatal and pediatric pathology. Conclusion. Indices of desquamated endothelial cells in the peripheral blood of the mother during the first trimester of pregnancy correlate with the somatic health of the newborn that can be applied in the practice of medicine as an informative test for early diagnosis of neonatal and infant morbidity.
