The aim is to investigate if the overweight (OW) and obesity (OB) impact on the quality of life (QOL) of children with bronchial asthma (BA). Materials and methods: The study included 73 children aged 7 – 17, with moderate BA. Depending on the body mass index, patients were divided into the three clinical groups: normal body weight (NW) – 30 children, OW – 28 children and OB – 15 children. QOL was conducted by the Pediatric Asthma Quality of Life Questionnaire (PAQLQ). The statistical processing was carried out using the IBM SPSS Statistics Base (version 22) and EZR version 1.32. Results were considered statistically significant when p<0.05. Results: Children with BA and OB had statistically lower QOL than children with BA and NW in all PAQLQ domains. The Kruskal-Wallis H-test revealed a statistically significant difference between the different weight groups of children with BA both for the general QOL: H(2) = 37.51, p<0.001and for each rating scale separately. Pairwise comparisons using Steel-Dwass test indicated that scores of NW were observed to be significantly different from those of OW and OB for rating Activity and Symptoms scales (p<0.001). Conclusion: Comorbid OW and OB reduce the specific QOL of children with BA. In assessing the effectiveness of specific approaches to treating BA in children with OW and OB, an assessment of the QOL of children should be added to the traditional common clinical and laboratory assessment.

There is a steady increase of bronchial asthma (BA) in the world, including for child population. Today, according to WHO 300 million people worldwide suffer from BA, and by 2025 the WHO has predicted 600 million patients. In addition to inadequate treatment, the causes of inadequate asthma control include: environmental issues, the using of large amounts of chemicals in the home, often uncontrollable drug intake, severe asthma, which is often associated with severe inflammation; chronic exposure to sensitizing unaccounted factors and comorbidities. Particularly noteworthy are comorbidities, which are having common or similar etiological and pathogenetic factors, such as combination of BA and obesity.

Актуальность. На сегодняшний день вопрос влияния избыточной массы тела и ожирения на клиническое течение бронхиальной астмы у детей остается малоизученным и требует дальнейших исследований. Цель: изучить клинические особенности течения бронхиальной астмы у детей с избыточной массой тела и ожирением. Материалы и методы. Проведено исследование 94 больных бронхиальной астмой детей в возрасте от 6 до 18 лет. В зависимости от индекса массы тела пациенты были разделены на следующие клинические группы: 30 детей с нормальной массой тела, 45 детей с избыточной массой тела и 19 детей с ожирением. Методы исследования: общеклинические, антропометрия, лабораторные, анкетно-опросный (опросник контроля бронхиальной астмы ACТ). Статистическую обработку полученных данных проводили с помощью статистического пакета IBM SPSS Statistics Base (версия 22) и программного обеспечения EZR, версия 1.32 (графический интерфейс среды R (версия 2.13.0)). Результаты. Установлено, что для детей групп с избыточной массой тела и ожирением характерен ранний дебют астмы (3 [3; 4]) года) с длительным персистирующим характером. Для всех трех групп наиболее существенными по частоте были жалобы на кашель, причем в группе нормального веса — на сухой (83,3 ± 7,4 %), а в группах с избыточной массой тела и ожирением — на влажный (соответственно 66,7 ± 7,0 % и 73,7 ± 10,1 %), и одышку при физической нагрузке: группа нормальной массы тела — 46,7 ± 9,1 %, избыточной массы тела — 82,2 ± 5,7 % и ожирения — 100 %. Для групп избыточной массы тела и ожирения также существенными по частоте были жалобы на ночные приступы удушья — 68,9 ± 6,9 % и 73,3 ± 10,1 % соответственно. χ2-тест показал статистически значимые различия между тремя группами по этому показателю: χ2(2) = 15,947; p < 0,001. У больных из группы ожирения чаще встречалось (70,0 ± 10,5 %) тяжелое течение бронхиальной астмы, выявлены статистически значимые различия между тремя группами по тесту: χ2(2) = 5,2463, p < 0,001. Неконтролируемая астма статистически значимо преобладала в группе детей с ожирением. Выявлена статистически значимая разница между различными весовыми группами в потребности в бронхолитиках: H(2) = 40,756; p < 0,001, и в количестве ночных приступов астмы: H(2) = 17,803, p < 0,001. Выводы. Проведенное нами исследование свидетельствует о значительном влиянии избыточной массы тела и ожирения на клиническое течение бронхиальной астмы у детей, может быть основанием для коррекции терапии астмы у таких пациентов.

У лекції розглянуто питання оцінки аускультативної картини серцевих шумів у дітей. Проаналізовані класифікація кардіальних шумів у дітей, механізми виникнення органічних і функціональних шумів. Зокрема, описано їх диференціювання за такими показниками, як фаза їх виникнення, епіцентр шуму, інтенсивність, тембр, іррадіація, залежність від фаз дихання, від положення дитини, від фізичного навантаження та ін. Акцентовано увагу на тактиці лікаря при виявленні різних серцевих шумів у дітей.

аtopic dermatitis and food allergy are common diseases that usually begin in early childhood and can occur together in the same individuals. The aim of this study was to assess the significance of interventions such as skin care products for the skin barrier improvement, breastfeeding and early complementary foods introduction for the primary prevention of atopic dermatitis and food allergy in infants by building logistic regression models. We performed a survey of 97 parents of children. The survey was conducted with the help of Google forms and distributed on the Internet. Method of building and analysing logistic regression models was used to analyse the association of the risk of atopic dermatitis in children with the factor characteristics. Characteristics were as follows: "Skin care 1: application moisturizers to the infant's skin", "Skin care 2: bathing infants with water containing moisturizing substances or moisturizing oils", "Skin care 3: usage of less soap, bathing the child less often", "paternal history of atopy", "duration of breastfeeding less than 1 year", "early introduction of supplementary food (up to the 6th month of life)". We obtained the following results: 42.2% of respondents reported about the application of moisturizing cream, parents of 16.5% of children were using moisturizing oil for bathing the child, 12.4% of parents used less soap and bathed the child less often, 28.9% reported that they were not using any interventions. It was established that when applying care method 1, namely applying moisturizing agents to the infant’s skin, the risk of developing atopic dermatitis increases, OR=12.8 (95% CI 3.89 - 42.3) (p<0.0001). When constructing a three-factor logistic regression model for predicting the risk of food allergy, no dependence was found between the development of food allergy and the presence of allergic diseases in parents, the period of introduction of complementary foods, and the duration of breastfeeding. Thus, this study did not reveal the protective role of skin care products, breastfeeding, and early introduction of complementary foods for the development of atopic dermatitis and food allergies in children. Multivariate analysis showed that atopic dermatitis is associated with emollients application to the infant’s skin. The use of moisturizing creams for the treatment of already existing atopic dermatitis or the path of percutaneous sensitization to allergens could have influenced the data.

The aim: Malassezia has been linked to atopic dermatitis, and TLRs are suggested to mediate influence of Malassezia spp on human cells. The aim of the study was to examine if TLR2 rs4696480 polymorphism predisposes to atopic dermatitis, IgE sensitization to Malassezia or to severe phenotype of atopic dermatitis. Materials and methods: The study included 103 patients with eczema and 84 healthy children. Specific IgE against Malassezia mix (m227) was analyzed in 47 patients using immunochemiluminescent method on the ImmunoCAP 100 (Thermo Fisher Scientific Inc., Phadia, Sweden). Genotyping for TLR2 rs4696480 was performed by using Real-time PCR. Results: Increased IgE to Malassezia spp. was observed in 34,3 % of children with eczema. Higher Malassezia spp.-specific IgE titre positively correlated with duration of atopic dermatitis and a higher total IgE. There were no difference in allele distribution among patients and control group (OR=1.096 (0.549- 2.191) for AT, OR=0.946 (0.430– 2.078) for TT, р > 0,05). TLR2 polymorphism rs4696480 was not associated with Malassezia spp.-sIgE. AA-genotype was significantly more frequent among patients with severe and moderate-to-severe AD (OR=6.395 (1.240-32.991). Conclusions: AA variant of TLR2 rs4696480 polymorphism predisposes to severe phenotype of AD.

Анотація. Вимоги, зумовлені реформуванням системи медичної освіти у зв’язку зі вступом нашої країни до єдиного європейського і світового простору, і водночас умови війни та пандемії визначають підвищену потребу пошуку ефективних методів підготовки майбутніх фахівців. Дана робота присвячена аналізу результатів анонімного опитування студентів 6 курсу медичного факультету, спеціальність 222 «Медицина», які завершили дисципліну «Педіатрія» на кафедрі педіатрії № 2 Національного медичного університету імені О. О. Богомольця, щодо оцінки засвоєння дисципліни, форм і методів навчання. На думку студентів, змішана форма навчання є найбільш ефективною системою організації навчального процесу при вивченні педіатрії на сучасному етапі. Аналіз результатів анкетування виявив певні труднощі у студентів при вивченні окремих розділів педіатрії (кардіології, нефрології), опануванні надання екстреної допомоги дітям при деяких невідкладних станах, навичок маніпуляцій та інтерпретації результатів інструментальних методів дослідження. Серед ймовірних причин труднощів під час вивчення педіатрії переважна більшість студентів зазначила брак часу для вивчення теми, недостатній рівень засвоєння знань при вивченні попередніх дисциплін та недостатню кількість аудиторних занять. Найбільш ефективним методом навчання переважна більшість студентів вважає вирішення та обговорення клінічних кейсів. Водночас самостійне вирішення клінічних завдань є складним для значної кількості студентів. Збільшення застосування можливостей цифрових технологій у навчальному процесі, посилення інтегративних міждисциплінарних зв’язків та пререквізитів, надання можливості студентам додаткових тренінгів з опанування маніпуляцій є потужними інструментами підвищення ефективності засвоєння педіатрії.
