Background. Lymphangiomas are non-specific malformations that develop from the lymphatic vessels. Usually, the disease is asymptomatic, but there may be complications requiring emergency care. In the literature there is no consensus about the method of choice in the treatment of these formations. Description of A Case Series. The article is devoted to the evaluation of the efficacy of surgical treatment of lymphangiomas in children. There are also several clinical cases showing mediastinal lymphangioma with a clinical picture of respiratory failure, lymphangioma of the abdominal cavity, imitating the symptoms of an acute abdomen and lymphangioma of a rare anatomical localization. The results of treatment of 48 children for the period from 2008 to 2017 were analyzed at «Morozov Children Municipal Clinical Hospital» (Moscow). The basis of treatment was the removal of education within healthy tissues. When the radical intervention was impossible in 5 (10.4%) children we used resection of the tumor with sparing coagulation of the surface of the formation. Infectious complications arose in 3 (6.3%), lymphorrhea — in 13 (27.1%) patients. Recurrences of lymphangiomas developed in 2 (4.2%) patients. In the case of lymphorrhea, expectant management and parenteral nutrition were used. The repeated operations were performed with recurrences. The catamnesis ranges from 6 months to 10 years. Conclusion. Radical removal of lymphangiomas of internal organs can be recommended to children of any age as an effective way to treat this disease.
Widespread peritoneal carcinomatosis in gastric cancer, in fact, is the end-stage of the disease. The survival median of patients is no more than 3-6 months. Development of various methods of intraperitoneal chemotherapy can improve the prognosis of this category of patients. Резюме Распространенный перитонеальный канцероматоз при раке желудка, по сути, является терминальной стадией заболевания. Медиана выживаемости больных не превышает 3-6 месяцев. Разработка различных методов внутрибрюшной химиотерапии позволяет улучшить прогноз этого тяжелого контингента больных. Цель. Оценить эффективность и безопасность внутрибрюшной аэрозольной химиотерапии под давлением (ВАХД) у больных раком желудка с перитонеальным канцероматозом. Материалы и методы. Лечебный протокол включал в себя лапароскопию или лапаротомию с целью стадирования опухолевого процесса, 3-4 курса системной химиотерапии по схеме XELOX с последующим проведением не менее 3 сеансов внутрибрюшной аэрозольной химиотерапии под давлением (ВАХД) с интервалом 6 недель на фоне продолжения химиотерапии. В случае прогрессирования больной исключался из исследования. В настоящее время в исследование включено 27 пациентов с диссеминированным раком желудка, которым выполнено 46 процедур ВАХД. Мужчин было 8, женщин-19. Средний возраст больных составил 50,6 года. Результаты. В рамках оценки безопасности проведения ВАХД было зафиксировано 3 случая развития нежелательных явлений. Двое больных (7,4%) отмечали тошноту в течение первых 2 суток после выполнения сеанса ВАХД. У одного больного произошла ятрогенная перфорация диафрагмы во время взятия биопсии брюшины с развитием карбокситоракса. Медиана выживаемости составила 11 месяцев. Годичная выживаемость (по Kaplan-Meier)-50,7%. 14 пациентов живы и продолжают участие в исследовании в течение первого года наблюдения. Заключение. Внутрибрюшная аэрозольная химиотерапия под давлением (ВАХД) является простым, малоинвазивным и безопасным методом паллиативного лечения больных с распространенным канцероматозом при раке желудка. Разработанный лечебный протокол позволил добиться годичной выживаемости более чем у половины больных.
Цель исследования -изучить непосредственные и отдалённые результаты применения внутрибрюшной аэрозольной химиотерапии под давлением (ВАХД) у больных с перитонеальным карциноматозом при раке желудка. Материал и методы. На первом этапе, согласно протоколу исследования, пациентам выполняли стадирующую лапароскопию, в ходе которой определяли распространённость перитонеального карциноматоза по шкале pCI с обязательной множественной биопсией брюшины и последующим гистологическим исследованием измененных участков брюшины. Лечение начинали с 4 курсов системной химиотерапии по схеме XELOX или FOLFOX. В дальнейшем больным в условиях торакоабдоминального отделения осуществляли сеансы ВАХД препаратами цисплатин и доксорубицин с интервалом 6-8 нед. Между сеансами пациенты продолжали получать системную химиотерапию по прежней схеме. Каждый сеанс ВАХД включал лапароскопию, повторную биопсию и оценку pCI. результаты. У 102 (80,3 %) больных был первичный рак желудка с ПК, у 25 (19,7 %) -перитонеальный рецидив. pCI<10 баллов зафиксирован у 60 (47,2 %), pCI 10-18 -у 33 (26 %), pCI>18 -у 34 (26,7 %). Диффузный тип рака желудка выявлен у 114 больных (89,7 %), кишечный тип -у 6 (4,7 %), смешанный тип -у 7 (5,5 %). С 2013 г. 127 больным проведено 310 процедур ВАХД. Тяжелых осложнений и летальности не отмечено. Лечебный патоморфоз был оценен у 72 больных. У 40 (55,6 %) больных отмечен выраженный патоморфоз (pRGS 2) и у 10 (13,9%) -полный патоморфоз (pRGS 1). Выживаемость оценена у 63 % (n=80), получивших более 1 сеанса ВАХД. Медиана выживаемости составила 16,0 мес, 1-годичная выживаемость -77,9 %. Заключение. Новый подход к лечению больных раком желудка с перитонеальным карциноматозом, сочетающий системную химиотерапию и внутрибрюшную аэрозольную химиотерапию под давлением, зарекомендовал себя как относительно простой и безопасный метод, позволяющий добиться увеличения общей выживаемости у пациентов с диссеминированным раком желудка. ключевые слова: рак желудка, перитонеальный карциноматоз, внутрибрюшная аэрозольная химиотерапия под давлением.
