Лимфангиолейомиоматоз является редким, сложным в диагностике и лечении, полисистемным заболеванием, которым болеют преимущественно женщины репродуктивного возраста. По данным Европейского респираторного общества за 2010 год, распространённость лимфангиолейомиоматоза составляет 1 случай на 400 тысяч женщин. Принято выделять определённый, вероятный и возможный лимфангиолейомиоматоз. В статье представлен обзор литературы, подкреплённый клиническим случаем пациентки с определённым лимфангиолейомиоматозом лёгких. Пациентка М., 1975 года рождения, обратилась за медицинской помощью с жалобами на одышку при незначительной физической нагрузки и слабость. Была выполнена рентгенограмма органов грудной полости, на который обнаружился правосторонний гидроторакс. Больная была направлена и госпитализирована в торакальное отделение Донецкого клинического территориального медицинского объединения, где ей был выполнен торакоцентез и дренирование правой плевральной полости. Объективно состояние пациентки было расценено, как средней степени тяжести. Над лёгкими перкуторно: лёгочный звук слева, справа – притупление ниже угла лопатки. Аускультативно в лёгких везикулярное дыхание слева, справа в средних и нижних отделах не выслушивалось, в верхних – ослаблено. ЧДД- 22 в мин. В пунктате плевральной жидкости от 31.08.2017 года – картина реактивного плеврита. 1.09.2017 была выполнена мультисрезовая компьютерная томография органов грудной клетки, на который были выявлены множественные кисты от 0,7 см до 1,2 см. С 1.09.2017 г. до 13.09.2017 г. отмечалась отрицательная динамика, прогрессивно увеличивалось количество лимфы, объём которой, по данным ультразвукового исследования от 13.09.2017 г., достиг 2-х литров. 19.09.2017 пациентке была выполнена операция «клипирование грудного лимфатического протока, плевродез, дренирование плевральной полости». После произведённой операции отмечалась положительная рентгенологическая динамика. Исходя из характерных изменений лёгких на мультисрезовой компьютерной томографии (наличие множественных, больше 10, тонкостенных кист), а также клинических проявлений в виде рецидивирующего хилоторакса, данной пациентке был выставлен диагноз: Лимфангиолейомиоматоз лёгких. Согласно критериям Европейского респираторного общества, данная клиническая картина соответствует определённому ЛАМ.Данная патология является крайне редкой, часто не диагностируемой. Информированность врачей о данном заболевании низкая. Заболевание имеет невыясненную этиологию, недостаточно изученный патогенез, что ведёт к отсутствию эффективных методов медикаментозного лечения. Также, заболевание имеет крайне неблагоприятный прогноз.
Идиопатический легочной фиброз относится к группе интерстициальных пневмоний. В последнее время встречаемость данной патологии возросла, прогноз неблагоприятный. Выделяют возможные варианты течения: стабильное (медленно прогрессирующее), быстропрогрессирующее, периоды рецидивов при любом варианте. Приведен клинический случай, который примечателен сразу несколькими особенностями. Он свидетельствует о длительном стабильном течении идиопатического легочного фиброза (более 9 лет), а также о рецидиве заболевания, несмотря на терапию, проводимую системными глюкокортикоидами.У пациентов с ИЛФ отмечается выраженная одышка и снижение повседневной активности, возможности терапии ограничены, болезнь имеет очень плохой прогноз. У пациентов с ИЛФ наблюдаются различные сценарии течения заболевания: у одних ИЛФ прогрессирует медленно, у других развиваются обострения ИЛФ, у третьих болезнь прогрессирует очень быстро.
