Σκοπός της μελέτης ήταν η διερεύνηση του ρόλου της 3ωρης μεταγευματικής pH μετρίας: • στη μελέτη του συνδρόμου της παθολογικής γαστροοισοφαγικής παλινδρόμησης (ΓΟΠ) και το διαχωρισμό αυτής από το φυσιολογικό φαινόμενο, όπως επίσης και • στο μετεγχειρητικό έλεγχο ασθενών που είχαν υποστεί στο παρελθόν αντιαναγωγική επέμβαση. Έτσι, βάσει συγκεκριμένου πρωτοκόλλου, μελετήθηκαν 82 ασθενείς (48 άνδρες και 34 γυναίκες) οι οποίοι χωρίστηκαν σε δύο ομάδες ανάλογα με το αν είχαν υποστεί ή όχι επέμβαση στην καρδιοοισοφαγική περιοχή. Η πρώτη ομάδα (χωρίς επέμβαση) περιελάμβανε 65 ασθενείς: - 38 άνδρες ηλικίας 48 χρ. κατά μέσο όρο, και - 27 γυναίκες ηλικίας 51 χρ. (μ.ο.). Η δεύτερη ομάδα (χειρουργημένοι) περιελάμβανε 17 ασθενείς: - 10 άνδρες ηλικίας 56 χρ. (μ.ο.) και - 7 γυναίκες ηλικίας 59 χρ. (μ.ο.). Εκτός από την 3ωρη pH μετρία, η μελέτη των περιστατικών στηρίχτηκε: • Στη λήψη λεπτομερέστατου ιστορικού, βάσει ειδικού πρωτοκόλλου καταγραφής των συμπτωμάτων και εκτίμησης της βαρύτητας αυτών. • Σε πλήρη ακτινολογικό έλεγχο οισοφάγου, στομάχου, 12δακτύλου με σκοπό τη διαπίστωση της ύπαρξης ή μη KOT, επιπλοκών της οισοφαγίτιδας (έλκος ή στένωση), όπως και ελκοπάθειας στομάχου ή 12δακτύλου με συνοδό καθυστέρηση της κένωσης του στομάχου, ή ακτινολογική παλινδρόμηση βαρύου σε θέση Trendelenburg. • Στην ενδοσκόπηση του ανώτερου πεπτικού για τη διαπίστωση οισοφαγίτιδας ή ελκοπάθειας και τη λήψη βιοψιών, όταν αυτό χρειάστηκε. Η μεθοδολογία αυτή αποδείχτηκε η πιο απλή, ανεκτή από τον άρρωστο και αποτελεσματική για τη μελέτη ενός τόσο περίπλοκου φαινομένου, στην καθημερινή πρακτική. Με βάση τα αποτελέσματα της θεραπευτικής τακτικής στο υλικό μας, φαίνεται πως η σωστή συντηρητική αγωγή είναι δυνατόν να απαλλάξει τους περισσότερους ασθενείς από τα ενοχλήματα τους και λίγοι από αυτούς θα χρειαστούν χειρουργική αντιμετώπιση. Όσον αφορά την 3ωρη pΗμετρία, αυτή αποδείχτηκε πολύ χρήσιμη και απλή εξέταση, τόσο στη διάγνωση της παθολογικής γαστροοισοφαγικής παλινδρόμησης, όσο και στην εκτίμηση της αποτελεσματικότητας της θεραπευτικής αγωγής - συντηρητικής ή χειρουργικής.
