The aim of this study was to determine the frequency and significance of the tumor DNA content heterogeneity in 33 previously untreated human neuroblastomas. We used image cytometry to selectively analyze neuroblasts by excluding karyorrhectic or stromal cells from cytometric measurements. DNA content heterogeneity with more than one clonal subpopulation on DNA histogram was found in 8 of 33 cases. Of these 8 cases, 4 showed MYCN amplification. Double labeling fluorescent in situ hybridization with probes for the centromeric region of chromosome 2 and MYCN gene was used to confirm the DNA content heterogeneity. DNA content heterogeneity was associated with poorer prognosis in this study (P<0.05). There was a significant correlation between euploidy (di- and tetraploidy) and worse prognosis, but only when heterogeneous neuroblastomas with euploid cell population were assigned to euploid tumors (P=0.006). Our results may explain the conflicting data in the literature regarding ploidy and suggest that DNA content heterogeneity and the presence of a euploid population may predict worse prognosis in neuroblastoma patients.
Bevezetés: A humán papillomavírus (HPV) perzisztáló fertőzése a méhnyakrák és a cervicalis intraepithelialis neoplasia (CIN), valamint több más lokalizációjú daganat legfőbb okozója. A rendelkezésre álló adatok arra utalnak, hogy dön-tően a magas rizikójú HPV-típus lehet felelős ezeknek a daganatoknak és rákmegelőző állapotainak kialakulásáért. Azonban nincs információnk arról, hogy milyen a HPV-típusok gyakorisági eloszlása cervicalis intraepithelialis neoplasiákban, és milyen összefüggés van a HPV-gyakorisági eloszlás és a párhuzamosan végzett citológiai diagnózis között. Célkitűzés: Ebben a munkában új adatokat kívántunk szolgáltatni a HPV-vel összefüggő daganatos betegségek 2009-2015 közötti előfordulási gyakoriságáról és halálozási arányáról Magyarországon. Célunk volt vizsgálni a méhnyak citológiai diagnózisa és mellette egyidejűleg elvégzett HPV-tipizálás összefüggéseit, végül azt, hogy a HPV-ellenes vakcinák várhatóan milyen hatékonysággal képesek megelőzni a méhnyakrákot. Módszer: A HPV-vel összefüggő daganatok epidemiológiai adatai a Nemzeti Rákregiszterből származnak. A HPVtipizálás Linear Array HPV Genotyping Test segítségével történt. Az egyidejűleg elvégzett méhnyak-citológiai vizsgálatot és HPV-tipizálást 2006 és 2016 között gyűjtött összesen 2048 mintán végeztük. Eredmények: Magyarországi epidemiológiai adataink szerint a szájüregi, oropharynx-gége és anogniális régiókat tekintve a leggyakoribb daganatos előfordulás férfiaknál a gégerák, nőknél a méhnyakrák. A fej-nyaki carcinomák elő-fordulási gyakorisága a 2009-2015 közötti időintervallumban férfiaknál nem változott, nőknél a nyelvgyöki laphám-rákok incidenciája fokozatosan nőtt, a méhnyakrák előfordulási gyakorisága a 2011-2015 közötti időszakban szintén emelkedett. Meghatároztuk az egyszeres és egyidejű többszörös HPV-fertőzés klinikai jelentőségét, és vizsgáltuk a HPV-típusok gyakorisági eloszlását, valamint összefüggését a citológiai diagnózissal CIN-ben. Azt találtuk, hogy az IACR Working Group által a valószínűleg/esetleg carcinogen pHR-HPV csoportba sorolt vírusoknál a citológiai negativitás jóval magasabb (56% versus 47%), a HSIL-pozitivitás viszont jóval alacsonyabb volt (9,7% versus 17,9%), mint a HR-HPV csoportban. A többszörös vírusfertőzés a HPV-típusoktól függetlenül magas rákkockázatot jelent. A HPV16 nemzetközi közleményekből ismert előfordulási gyakorisága méhnyakrákban kétszerese annak, amit mi CIN-ben találtunk (60% versus 30%). A HPV18 carcinomában a második leggyakoribb, CIN-ben a kilencedik. A HR-HPV35 sokkal ritkább, a pHR-HPV53, a pHR-HPV66 és a pHR-HPV73 viszont gyakoribb CIN-ben, mint carcinomában. A HPV-ellenes védőoltások preventív hatását értékelve jelentős különbségeket találtunk akkor, ha az egyik betegcsoportban mintánként csak egy, a másikban egyszerre több HPV is elfordult. Következtetések: Nőknél a nyelvgyöki laphámrákok (CO1) incidenciája évről évre egyenletesen emelkedik, másrészt a méhnyakrák (C53) előfordulási gyakorisága 2011-2015-ben az előző évekhez viszonyítva szintén fokozatosan emelkedik.
