Introduction: Coercion can be defined as the use of force to limit a person's choices. In Poland, coercive measures may tend to be overused. However, there is limited information regarding the attitudes of nurses toward coercion in psychiatric settings and the factors influencing any decisions to use coercion.Aims: To validate the Staff Attitudes to Coercion Scale (SACS) for a group of psychiatric nurses and psychiatrists, to compare the said with the original Norwegian SACS version, and to compare nurses' attitudes with those displayed by psychiatrists. A second aim was to understand the relationship between self-efficacy and attitudes to coercion.Method: We surveyed 351 psychiatric nurses and psychiatrists rating SACS and GSES (General Self Efficacy Scale). We validated the SACS factor structure using confirmatory principal component factor analysis, calculated the internal consistency of subscales, and analyzed the test-retest reliability and face validity of the subscales themselves. Further, we analyzed the differences in attitudes toward coercion between nurses and psychiatrists, as well as whether there was an association between GSES and the SACS subscales. We compared the means on the SACS items between three countries—Germany, Norway, and Poland.Results: The confirmatory factor analysis of the Polish version of SACS found the same factor structure with three factors as was displayed in the original Norwegian SACS, except that one item was loaded on another factor. Internal consistency was acceptable for the factors on coercion as security and the coercion as offending, and unacceptable for the factor on coercion as treatment. Test-retest reliability was excellent for all the three subscales. Face validity was high for the factor coercion as security, partly present for coercion as offending, and not present for coercion as treatment. The subscale Coercion as Treatment was rated significantly higher by nurses than by psychiatrists, but there was no difference for the two other subscales. There was no significant association between the General Self-Efficacy Scale and any of the SACS subscales. The biggest differences in attitudes toward forms of coercion was noted between Poland and Germany.Discussion: The three-factor structure of SACS was the best solution for the Polish nurses and psychiatrists. The attitudes toward coercion differed between the two groups, but a low correlation was computed for the SACS subscales and self-efficacy. There is a cultural diversity visible amongst the three countries examined. Reduction in the use of coercion is a priority worldwide. More knowledge about the process involved in using coercive measures may contribute to this. The use of coercive interventions may harm patients and threaten patients' rights. Thus, education is needed for pre-service and in-service nurses alike.
Wstęp. Agresja pacjentów to problem w pracy pielęgniarek. Badania wskazują na jej duży wpływ na jakość opieki. Istnieje konieczność badania tego zjawiska w pracy personelu medycznego, brak jednak adekwatnych narzędzi. Cel. Celem pracy było określenie psychometrycznych właściwości polskiej wersji 12-pytaniowego kwestionariusza postrzegania agresji POAS. Materiał i metody. Wykonano tłumaczenie i tłumaczenie wsteczne oraz analizę czynnikową. W badaniu wzięło udział 150 studentek pielęgniarstwa po zakończeniu praktyk psychiatrycznych. Wyniki. Przetłumaczona wersja wykazuje wysoką wartość psychometryczną i podobieństwa z wcześniejszymi badaniami. Analiza czynnikowa pozwoliła na wyodrębnienie trzech czynników (wymiar dysfunkcjonalny, funkcjonalny oraz ochronny). Wszystkie czynniki wyjaśniały 52,57% wariancji, a współczynnik alfa Cronbacha wynosił odpowiednio ,805; ,755 oraz ,828. Wnioski. Polska wersja POAS jest wartościowym i rzetelnym narzędziem służącym do badania postrzegania agresji pacjentów przez pielęgniarki.
ozpoznawanie uzależnień od alkoholu i tytoniu wśród osób starszych Diagnosing addictions to alcohol and nicotine among elderly people STRESZCZENIE Wstęp. Postępujący proces starzenia się społeczeństwa skłania do zadania pytania o częstość występowania uzależnień, nie tylko ze względu na negatywny wpływ na stan zdrowia, ale również ze względu na konsekwencje społeczne. Cel. Calem pracy było rozpoznanie uzależnień od alkoholu i tytoniu u osób starszych. Materiał i metody. Badanie przeprowadzono wśród 100 osób po 60. roku życia. W badaniu posłużono się metryczką pytań demograficzno-społecznych własnej konstrukcji oraz kwestionariuszami: Tolerancji Nikotyny Fagerströma (FTND), Rozpoznawania Zaburzeń Związanych z Piciem Alkoholu (AUDIT), Krótkim Alkoholowym Michigeńskim Testem Przesiewowym -Wersja Geriatryczna (S-MAST-G). Wyniki. Wśród badanych 21% paliło tytoń, mężczyźni częściej niż kobiety palili (25% vs. 17%) i wykazywali wyższy stopień uzależnienia. Problemy z alkoholem wykazano u co czwartej badanej osoby (23%). Mężczyźni częściej niż kobiety wykazywali obecność problemów alkoholowych (p = 0,0001). Tylko mężczyźni wykazywali picie alkoholu ryzykowne i szkodliwe, ponad połowa badanych kobiet wykazała picie o niskim ryzyku (p = 0,04). Osoby niepalące tytoń w większości nie miały problemów alkoholowych. Wśród osób palących tytoń wykazano problem z alkoholem (p = 0,0007) oraz objawy picia szkodliwego (p = 0,006). Wnioski. Płeć stanowi predyktor w identyfikowaniu uzależnień u osób starszych, tylko mężczyźni wykazali picie alkoholu ryzykowne i szkodliwe. Współwystępowanie uzależnień od tytoniu i alkoholu stanowi problem zdrowotny osób starszych, których należy systematycznie motywować do zmiany zachowań oraz wspierać odpowiednią terapią.

Agresja pacjentów i ich rodzin stanowi poważny problem w pracy personelu medycznego. Agresja powoduje wiele psychologicznych reakcji: od poczucia winy przez lęk do złości i wściekłości. Efekt długofalowy stanowi pogorszenie jakości opieki zdrowotnej powiązane z wypaleniem zawodowym personelu. Cel pracy. Celem pracy była adaptacja skali IMPACS, służącej do oceny konsekwencji agresji wobec pielęgniarek. Materiał i metody. Badaniem objęto 102 studentki III roku pielęgniarstwa. Narzędzie badawcze stanowiła adaptacja skali IMPACS. Wyniki. W celu uzyskania polskiej wersji narzędzia zastosowano metodę eksploracyjnej analizy czynnikowej. Przedstawiono dwa warianty analizy-trzy-i dwuczynnikowe. Wnioski. W badaniu wykazano różnice w rozbieżnościach pomiędzy oryginalną wersją skali i uzyskaną w badaniach. Wskazuje to na różnice w postrzeganiu agresji i jej konsekwencji w zależności od wpływu kultury i miejsca pracy.
Introduction: Diabetes patients experience burdens associated with adherence to treatment, self-control, and taking preventive measures against diabetic complications. These can contribute to the development of depressive disorders. Aim of the study: To assess the occurrence and severity of depressive disorders and factors connected with them among diabetes type 2 patients aged 35-64 years. Material and methods: The research was done with the use of a diagnostic survey method among 70 patients in the Metabolic Disease Department of the University Hospital in Krakow. The Beck Depression Inventory and the authors' own interview questionnaire were used. Statistical analysis was carried out using Statistica 13.1, and p < 0.05 was the significance level. Results: The average number of points in the Beck Depression Inventory was 12.81 (moderate level of depressive disorders). Greater severity of depressive disorders was present in people who were professionally inactive, who defined their monthly income as insufficient, who felt uncertainty after diabetes diagnosis, whose treatment included both diet and insulin intake, who had problems with foot care, and who did not follow the diabetes treatment principles. The respondents who declared lack of support from their spouses felt the impact of diabetes on their mental well-being and confirmed the need for help in connection with the disease and had increased levels of depressive disorders. Conclusions: Due to the occurrence of depressive symptoms among diabetes patients it is necessary to conduct screening research for depressive disorders. It should be an indispensable element of the therapeutic team's activities.
