Amaç: Bu çalışma, Başkent Üniversitesi Ankara Hastanesi Acil Servisine başvuran 0-18 yaş grubundaki adli olguların demografik özelliklerini, başvuru nedenlerini ve hazırlanan adli raporların niteliklerini belirlemek, elde edilen verileri benzer çalışmalarla karşılaştırmak amacıyla yapılmıştır.Gereç ve Yöntem: Tanımlayıcı nitelikteki bu araştırmanın örneklemini, Başkent Üniversitesi Ankara Hastanesi Acil Servisinde 01/01/2013-31/12/2013 tarihleri arasında hazırlanan 2397 adli bildirim raporu oluşturmaktadır. Çalışma, bu raporların çocuklara ait 664’ünün değerlendirmesini içermektedir.Bulgular: Çocuk Acil Servisimize başvuran 0-18 yaşındaki çocuklara ait olgular, tüm adli raporların %27,70’sini oluşturmaktadır. Çocukların 316’sı (%45,0) kız,348’i (%49,5) erkektir. Olguların enfazla olduğu yaş 402 (%50,54) ile 0-6 yaş grubu çocuklardır. Olay tipinin cinsiyet ayrımı yapılmaksızın dağılımına baktığımızda en sık yanık olayları (n:218, %32,83) nedeniyle acil servislere başvurulduğu, bu değeri sırasıyla; zehirlenmeler (n: 119, %17,92), düşme (n: 77, %11,60), trafik kazaları (n: 70, %10,54) ve yabancı cisim aspirasyonu (n: 63, %9,49) olgularının izlediği görülmektedir. Olay tipleri cinsiyet ayrımı yapılarak değerlendirildiğinde yanıkların kızlarda %39,55, erkeklerde ise %27,87 değeriyle yine ilk sırada yer aldığı görülmektedir.Sonuç: Çocukluk çağındaki önlenebilir sağlık sorunlarının önemli bir kısmını kazalar oluşturmaktadır. Bu kazalar sonucu oluşan ve adli nitelik taşıyan yaralanmalar çoğunlukla ev kazaları sonucunda oluşmaktadır. Çocuk kazalarının önlenebilmesi için yaşam alanlarında gerekli güvenlik tedbirlerinin alınması kaza riskleriniz azalı dmasında etkin bir yöntem olarak görülmektedir.Anahtar kelimeler: Adli olgu, Çocuk, Kaza, Tedbir.
ObjectiveAlthough dentists are valuable assets in identification teams during disaster events, forensic dentistry is not used effectively in the identification studies conducted in Turkey, and the importance of dental data is ignored. The aim of this study was to determine the level of knowledge of dentists regarding their duties and responsibilities during major disasters.MethodsThis descriptive study was conducted between December 2015 and June 2016. Registered dentists (n=20.280) of the Turkish Dental Association were invited to complete the organization’s online survey. A total of 539 dentists participated in the volunteer workshop. Data were analyzed using SPSS, version 22.0 (IBM Corp, Armonk, NY). The chi-square analysis was used to evaluate the knowledge level of dentists by group regarding disaster victim identification (DVI) – the process and procedure of recovering and identifying victims of major disasters (eg, earthquake, terrorist attack).ResultsThe dentists included in the study consisted of 320 (59.4%) females and 219 (40.6%) males with a mean age of 37.4±12.6 years. The number of specialists and general dentists were 249 (45.6%) and 297 (54.4%), respectively; 249 (69.71%) dentists who had knowledge about forensic dentistry stated that they received this information during their formal training. The percentage of dentists who were aware of the existence of an organization of a disaster response operation in Turkey was 74.2%, but only 20.5% (n=110) had knowledge about DVI. We found that 92.9% (n=104) of these dentists believed that dentists should be included in the team for the identification of disaster victims. On the other hand, only half (52.3%) of the dentists with knowledge of DVI wanted to work on the identification teams. The majority (99.1%) considered DNA analysis to be the safest method for identification.ConclusionOur findings show that, although dentists know about the identification process, they do not have enough relevant knowledge. Amaç: Bu çalışmada, 2013 yılında trafik kazası sonucu yaralanarak Başkent Üniversitesi Ankara Hastanesi Acil Servisine başvuran hastaların adli bildirim raporları retrospektif olarak incelenerek; yaş, cinsiyet, olayın oluş yeri, olay tipi, olay zamanı, yaralanmanın tipi ve lokalizasyonu gibi özelliklerinin yanı sıra hazırlanan bildirim raporlarının eksikliklerinin de değerlendirilmesi amaçlanmıştır.Gereç ve Yöntem: Tanımlayıcı nitelikteki bu araştırmanın örneklemini, Başkent Üniversitesi Ankara Hastanesi Acil Servisinde 2013 yılında hazırlanan 2397 adli bildirim raporu oluşturmaktadır. Çalışma, bu raporların trafik kazalarına ait 553’ünün (%23,07) değerlendirmesini içermektedir. Bulgular: Çalışma kapsamına alınan 553 adli olgunun; %38,52’si kadın, %47,92’si erkek, %6,51’i kız çocuk ve %7,05’i erkek çocuktur. Yaş bilgisine ulaşılan 537 olgunun (%13,97)’si <18, (%51,96)’sı 18-40, (%24,58)’i 41-63, (%9,49)’u ise ≥ 64 yaş arasındaki bireylerden oluşmaktadır. Kazaların %67,81’i araç içi, %29.48’i araç dışı trafik kazasıdır. Lezyon bulgusu olan 407 olgudan 329’unda (%80.84) tek sistem yaralanması mevcuttur. Baş-boyun (n:123, %30.23) ve alt ekstremite (n: 101, %24,82) bölgelerine ait yaralanmalar ilk iki sırada yer almaktadır. Bu yaralanma lokalizasyonları nedeniyle de sıklıkla ortopedi ve beyin cerrahisi kliniklerinden konsültasyon istendiği verisi elde edilmiştir. Trafik kazası nedeniyle başvuran hastaların yaralanmalarının sadece %1,88’unda (n:10) yaşamsal tehlikenin olduğu belirlendi.Sonuç: Adli raporlardaki eksiklikler hak kayıplarına neden olabilir. Trafik kazalarında vatandaşların hakkının korunması, haksızlığın engellenmesi için adli raporların dikkatle düzenlenmesi gereklidir. Acil servis hekimleri adli olgulara yaklaşım konusunda eğitilmeli, adli tıp anabilim dallarından konsültasyon istemeli ve koordineli çalışmalıdırlar.
