Клинические рекомендации уже давно вошли в число рабочих инструментов современного врача, помогая ему быстро ориентироваться в наиболее эффективных доказанных методах лечения и профилактики различных заболеваний, а также адаптировать эти методы к конкретным задачам своих больных и добиваться максимальной персонализации лечения. Клинические рекомендации составляются профессиональными некоммерческими ассоциациями и одобряются научным советом МЗ РФ, при этом нередко одна рекомендация готовится двумя или даже тремя ассоциациями. Особенность предлагаемых вашему вниманию рекомендаций в том, что в профилактику и лечение ожирения вовлекаются не только эндокринологи, но и терапевты, кардиологи, гинекологи, гастроэнтерологи и врачи многих других специальностей. Мультидисциплинарная рабочая группа представляет этот проект в многопрофильном журнале с целью объединения усилий нескольких профессиональных ассоциаций, что связано с необходимостью уделить внимание не только самому ожирению, но и коморбидным состояниям. Мы надеемся на конструктивную критику и разностороннее обсуждение проблемы на страницах нашего журнала.
Резюме. В статье представлен обзор литературы, отражающий современные представления о клиническом значении возрастного андрогенного дефицита и необходимости его коррекции. Обобщены результаты исследований, продемонстрировавшие эффективность и безопасность заместительной гормональной терапии. Авторы указывают на необходимость врачей всех специальностей понимать и видеть преимущества заместительной терапии. Но приоритет в назначении и мониторинге терапии должен оставаться за урологом и эндокринологом.
On September 30, 2022, a meeting of the interdisciplinary expert council “Prevention and treatment of obesity. How to Achieve a Healthy Metabolic Balance. To reduce the social and economic burden of obesity and its consequences in the Russian Federation, it is necessary to introduce socially significant initiatives to prevent obesity and increase its detection rate, as well as to update modern approaches to the treatment of this chronic disease, taking into account its multifactorial pathogenesis, comorbidity, risk of complications and patient disability. Based on the results of the scientific reports and discussions held during the expert council, the experts made decisions on a further plan within the framework of socially significant initiatives for the prevention of obesity
S h o r t c o m m u n i c a t i o n SОбследовано 115 мужчин (средний возраст 53±6,4 года) с сахарным диабетом (СД) 2 типа. Эндотелиальная дисфункция (ЭД) оценивалась при помощи биохимических маркеров (VCAM-1, Р-селектина, Е-селектина) и ультразвукового исследования артериальной вазореактивности плечевой артерии. Проводилось анкетиро-вание при помощи опросника DSC. Выявлена тесная взаимосвязь уровня VCAM-1 с показателями липидного обмена. Уровень Р-селектина в большей степени был связан с неврологической симптоматикой СД, а также с показателями эндотелий-зависимой вазодилятации сосудов. Показатель Е-селектина коррелировал с вы-раженностью клинических проявлений СД. The endothelial dysfunction was assessed using biochemical markers (VCAM-1, Р-selectin and Е-selectin) and doppler ultrasound of the brachial artery. To evaluate the severity of symptoms of diabetes mellitus was used the DSC questionnaire. The study revealed a close relationship between VCAM-1 level and biochemical parameters of lipid metabolism. The level of P-selectin was more influenced on the neurological symptoms of diabetes, as well as parameters of endothelium dependent vasodilatation of blood vessels. E-selectin was correlated with the severity of the diabetic symptoms.
Endothelial dysfunction is an early marker for the development and progression of cardiovascular diseases. Scientific studies in recent years have shown the necessity to study the endothelial function in different groups of patients in clinical practice. This article is focused on the possibilities and perspectives for clinical use of laboratory and instrumental methods for the study of endothelial function. One of the factors causing the development of vascular disease in men is testosterone deficiency. The review highlights the most important mechanisms of action of sex hormones on the vascular endothelium and its function in men. The data about the effects of hormone replacement therapy with testosterone on endothelial function in patients with late onset hypogonadism were critically analyzed.
