Актуальность: В хирургическом лечении грыж пупочной локализации используются продольные и поперечные доступы.Данные о преимуществах и недостатках каждого разреза, в том числе относительно формирования сером противоречи-вы.Материалы и методы: Проведено биомеханическое исследование продольных и поперечных ран околопупочной областиживота на 15 нефиксированных трупах. Выполнялся продольный разрез в 2 см от пупка слева и поперечный – выше пупкана те же 2 см. Длина и глубина ран 3 см при толщине подкожно-жировой клетчатки в параумбиликальной области 3,5-4см. Далее в рану помещался полиэтиленовый баллон ёмкостью 4мл подсоединенный к чашечному наклонному жидкост-ному манометру. Кожа над ним ушивалась непрерывным интрадермальным швом. В баллон постепенно вводился воздух,при этом регистрировалось давление в нём. Объем баллона и соответствующий диапазон показаний давления выбраны сучетом описанного в литературе давления в лимфатических сосудах (1-10 ммHg).Результаты: Было установлено, что увеличение давления в системе при введении каждые 0,2 мл воздуха в диапазоне от 0до 2 мл составило в среднем 149,1±28,4 Па для продольно ориентированной раны и 128,4±22,5 Па для поперечно ориенти-рованной раны. Описанные показатели статистически достоверно выше в продольной ране при уровне значимости р≤0,01.Для оценки использован парный критерий t. То есть при скоплении в подкожном слое раны некоторого дополнительногообъема возрастание давления происходит в более выраженной степени в продольной ране. Данный факт может бытьсвязан с ориентацией соединительнотканных волокон самой клетчатки, а также дермы.Заключение: Полученные в ходе исследования данные возможно использовать при прогнозировании вероятности образо-вания послеоперационных сером околопупочной области при продольном и поперечном доступах.Ключевые слова: продольный разрез, поперечный пазрез, параумбиликальный, напряжение.
