El término de Síndrome de Burnout apareció por primera vez en 1974, cuando el psicoanalista Herbert J. Freudenberger observó que, al año de trabajar, la mayoría de los voluntarios sufría una progresiva pérdida de energía, hasta llegar al agotamiento, síntomas de ansiedad y depresión, desmotivación laboral y agresividad con los pacientes. Por otra parte, La Diabetes Mellitus 2, es un trastorno complejo caracterizado por deterioro de secreción de insulina y el aumento de la resistencia a la insulina con grados variables de predisposición hereditaria y con participación de diversos factores ambientales. Objetivo: Identificar la relación del síndrome de Burnout al control o descontrol metabólico en pacientes Diabéticos tipo 2. Metodología: Se trata de un estudio descriptivo, observacional, prospectivo y transversal. Participaron 70 trabajadores con edades entre los 27 y 60 años de ambos sexos de Instituciones de Salud; con Diabetes Mellitus 2, se aplicó la escala de Maslach Burnout Inventory para evaluar el grado de desgaste profesional y se obtuvo medición de la hemoglobina glucosilada. Resultados: Se demostró una prevalencia baja de 8.5% para el estrés laboral crónico o Burnout y en quienes se presentó este síndrome se mostró una correlación significativa con el grado de control o descontrol metabólico en un 83:3%
Systemic lupus erythematosus (SLE) is an entity that promotes the formation of autoantibodies that trigger immune complexes that damage various organs of the body. Worldwide, SLE has a prevalence of 13-7000 per 100,000 people and leads to a high mortality from cardiovascular diseases, as well as the risk of developing lupus nephritis (LN) in 60% of cases. We present the case of a 41-year-old patient with a history of recently diagnosed arterial hypertension and bronchial hyperreactivity of 20 years of evolution, admitted for 1 month of evolution with asthenia, myalgia, arthralgia and fever; she identifies malar erythematous dermatosis and systolic murmur in a pulmonary focus. When presenting with proteinuria, microhematuria and renal functional impairment, positive ANAs were performed, which is why it was classified as lupus nephropathy, and she needed to start hemodialysis. In the presence of the murmur, an echocardiogram was performed, which showed a pulmonary valve with a 1cm image of vegetation, causing moderate regurgitation. With the diagnosis of Libman-Sacks endocarditis, anticoagulant treatment, steroids, and mycophenolate were started, evolving to normal renal function.
