En este artículo se presentan los resultados de una investigación que se llevó a cabo en la Universidad Autónoma de Campeche-Facultad de Medicina en el que se desarrolló un estudio observacional, descriptivo, transversal y prospectivo para la identificación de cualquier enfermedad que sea factor de riesgo para presentar Enfermedad Renal Crónica en los pacientes que acudieron a sesiones de hemodiálisis y que presentaron algún síntoma de COVID-19, las variables analizadas en los pacientes fueron la edad de inicio de hemodiálisis, género y factores asociados que precipitaron la aparición de Insuficiencia Renal Crónica y por consiguiente el uso de hemodiálisis como terapia sustitutiva. Los resultados mostraron que los pacientes tuvieron diversos factores de riesgo que han presentado desde hace mucho tiempo atrás, que nos habla de la importancia del adecuado control y tratamiento de enfermedades para prevenir complicaciones de la Insuficiencia Renal Crónica y tratamiento sustitutivo diálisis.
Adolescence is a crucial stage in human development; It is a particular form of transition between childhood and adulthood where profound physical changes occur, including neuroendocrine growth and development, psychological changes, and social changes. Objective: Identify the Protective Factors and level of self-esteem in adolescents with the absence or presence of depression. Methodology: Cross-sectional, observational, descriptive study. The teachers, administrators and parents were informed about the objective of the study, they signed the informed consent authorizing the participation of their child in the study. Results: The depressive symptomatology through the Birleson scale, indicated that 21.9% present this symptomatology. Self-esteem evaluated with the Rosenberg scale, 17.6% with low self-esteem was observed. Conclusions: The observed prevalence is above that reported at the national level.
El término de Síndrome de Burnout apareció por primera vez en 1974, cuando el psicoanalista Herbert J. Freudenberger observó que, al año de trabajar, la mayoría de los voluntarios sufría una progresiva pérdida de energía, hasta llegar al agotamiento, síntomas de ansiedad y depresión, desmotivación laboral y agresividad con los pacientes. Por otra parte, La Diabetes Mellitus 2, es un trastorno complejo caracterizado por deterioro de secreción de insulina y el aumento de la resistencia a la insulina con grados variables de predisposición hereditaria y con participación de diversos factores ambientales. Objetivo: Identificar la relación del síndrome de Burnout al control o descontrol metabólico en pacientes Diabéticos tipo 2. Metodología: Se trata de un estudio descriptivo, observacional, prospectivo y transversal. Participaron 70 trabajadores con edades entre los 27 y 60 años de ambos sexos de Instituciones de Salud; con Diabetes Mellitus 2, se aplicó la escala de Maslach Burnout Inventory para evaluar el grado de desgaste profesional y se obtuvo medición de la hemoglobina glucosilada. Resultados: Se demostró una prevalencia baja de 8.5% para el estrés laboral crónico o Burnout y en quienes se presentó este síndrome se mostró una correlación significativa con el grado de control o descontrol metabólico en un 83:3%
Antecedentes. La colecistectomía es una de las cirugías mayormente practicadas alrededor del mundo, cerca del 96% de ellas se realiza de manera laparoscópica, así la lesión de vía biliar es una complicación ineludible a dicho procedimiento y genera una repercusión importante de manera clínica, económica y hasta emocional para el paciente, que afectará en algún grado su calidad de vida, además de poder tener incluso alcance legal para el médico tratante. Metodología. Se realiza la búsqueda intencionada en el área de archivo clínico y departamento de estadísticas, de aquéllos pacientes que fueron atendidos durante el periodo comprendido de enero de 2020 a diciembre de 2021 utilizando palabras clave que incluyen lesión biliar, lesión no traumática biliar, Strasberg, Bismut, así como las claves diagnósticas según el sistema CIE-10 que incluye K83.1 (Obstrucción conducto biliar), K83.2 (Perforación conducto biliar), K83.3 (Fístula conducto biliar), K83.8 (Otras enfermedades biliares), K83.9 (enfermedad vías biliares) y K87.0 (Trastornos vesícula biliar y vías biliares). Mediante los resultados obtenidos se realiza el análisis retrospectivo descriptivo con el propósito de determinar la existencia de factores comunes que predispongan a las lesiones biliares iatrogénicas así como condiciones intrahospitalarias que influyan en una mejor y pronta recuperación del paciente o por el contrario contribuyan a la mala evolución. Resultados y Conclusiones. La frecuencia de lesiones iatrogénicas de la vía biliar atendidas de enero de 2020 a diciembre de 2021 fueron 26 casos, con predominio durante el abordaje quirúrgico abierto. Los principales factores asociados con respecto a la técnica quirúrgica fueron la presencia de adherencias y difícil disección de la vía biliar, mientras que los principales factores asociados dependientes de la condición clínica del paciente fueron el género femenino, la condición de urgencia y la presencia de diabetes mellitus. Es poco utilizado el recurso de la colangiografía transoperatoria y la resonancia magnética, las cuales permiten conocer con precisión la naturaleza de la lesión biliar y el resto de las estructuras implicadas, por lo que se debe de incrementar su uso, mientras que la colangiopancreatografía retrógrada endoscópica (CPRE) aporta una valiosa opción diagnóstico-terapéutica. El momento diagnóstico predominante fue durante el periodo postoperatorio tardío, mientras que el tiempo promedio en que se efectúa la corrección quirúrgica de la lesión biliar iatrogénica fue de 14 días. El porcentaje de pacientes re-intervenidos en este hospital fue de 75% en quienes se realizó principalmente hepatoyeyunoanastomosis como opción quirúrgica. La evolución clínica de los pacientes clasificados como lesiones biliares Strasberg E fue principalmente tórpida y con presencia de complicaciones; sin embargo, no se presentó ninguna defunción.
Antecedentes: El dolor posoperatorio en la colecistectomía abierta es de tipo severo lo presentan 50% de los pacientes intervenidos. Objetivos: Demostrar que el uso del Sulfato de Magnesio (MgSO4) por vía intravenosa es una mejor alternativa como analgésico que el tratamiento convencional (ketorolaco) en pacientes postoperados de colecistectomía Métodos: Se realizó un estudio longitudinal, prospectivo, experimental y analítico comparándose el Sulfato de Magnesio contra el ketorolaco midiendo dosis de rescate, grado de dolor con Escala de EVA y complicaciones con el uso de Sulfato de Magnesio en el posoperatorio. Resultados: Se incluyeron dentro del estudio 40 pacientes divididos en 2 grupos, en el grupo del Sulfato de Magnesio solo 9 pacientes que corresponde al 45% utilizaron medicamento de rescate en 146.7 ± 67.8 minutos (rango: 60-240 min) comparado con el grupo Control, 14 (70%) pacientes requirieron fármacos de rescate a un tiempo promedio de 154.3 ± 36.9 minutos (rango: 90-180 min). Conclusiones: Se comprobó la utilidad del Sulfato de Magnesio para control del dolor postoperatorio severo, se observó que al momento de la extubación y los 30 minutos subsecuentes la EVA fue menor en el grupo de estudio, lo cual nos indica mejor analgesia con el sulfato de Magnesio. En este estudio no se observaron efectos colaterales o complicaciones con ninguno de los dos tratamientos.
