Background Pseudomonas aeruginosa Sequence Type 235 is a clone that possesses an extraordinary ability to acquire mobile genetic elements and has been associated with the spread of resistance genes, including genes that encode for carbapenemases. Here, we aim to characterize the genetic platforms involved in resistance dissemination in bla KPC-2 - positive P. aeruginosa ST235 in Colombia. Results In a prospective surveillance study of infections in adult patients attended in five ICUs in five distant cities in Colombia, 58 isolates of P. aeruginosa were recovered, of which, 27 (46.6%) were resistant to carbapenems. The molecular analysis showed that 6 (22.2%) and 4 (14.8%) isolates harboured the bla VIM and bla KPC-2 genes, respectively. The four bla KPC-2 -positive isolates showed a similar PFGE pulsotype and belonged to ST235. Complete genome sequencing of a representative ST235 isolate shows a unique chromosomal contig of 7097.241 bp with eight different resistance genes identified and five transposons: a Tn 6162-like with ant(2″)-Ia , two Tn 402-like with ant(3″)-Ia and bla OXA-2 and two Tn 4401b with bla KPC-2 . All transposons were inserted into the genomic islands. Interestingly, the two Tn 4401b copies harbouring bla KPC-2 were adjacently inserted into a new genomic island (PAGI-17) with traces of a replicative transposition process. This double insertion was probably driven by several structural changes within the chromosomal region containing PAGI-17 in the ST235 background. Conclusion This is the first report of a double Tn 4401b chromosomal insertion in P. aeruginosa , just within a new genomic island (PAGI-17). This finding indicates once again the great genomic plasticity of this microorganism . Background: Helicobacter pylori is a bacterium associated with gastroduodenal disease and gastric cancer. Empirical therapy in the treatment of H. pylori infection increases the risk of apparition of antimicrobial drug resistance. In a previous report, in H. pylori clinical isolates, resistance rates to commonly used antimicrobial drugs were as follows: metronidazole 82%, clarithromycin 3.8%, and amoxicillin 1.9%. The aim was to establish the variation of resistance rates and the detection of H. pylori genetic mutations isolated from dyspeptic patients. Methods: Antimicrobial susceptibility profiles were performed by the E-test method for metronidazole, clarithromycin, amoxicillin, and tetracycline in 61 clini-cal isolates. Sequencing was performed to detect mutations associated with resistance to clarithromycin. Results: According to our results, resistance rates found in the 61 isolates were 78.60% for metronidazole and 8.20% for clarithromycin. None of the studied isolates had resistance to tetracycline and amoxicillin. Secondary resistance rates displayed an increase when compared to primary rates for metronidazole (87.50 vs. 77.35%) and for clarithromycin (25.66 vs. 5.66%). Of 5 isolates resistant to clarithromycin, 3 had the A2143G mutation. By comparing the results in this work with previous reports, antimicrobial drug resistance rates did not show major modifications for metronidazole, amoxicillin, and tetracycline during the last 10 years. For clarithromycin, the resistance rate showed a moderate increase; nevertheless, it remains low (< 15%) and this change was not statistically significant. Conclusion: Together, all findings in this work indicate that these antimicrobial drugs can still be used as first line of defense on infected patients living in this region of the country.
La piedra blanca es una micosis superficial crónica y asintomática poco frecuente del pelo causada por Trichosporon spp. (T. asahii, T. cutaneum o T. beigelii, T. inkin y T. mucoides), caracterizada por presentar nódulos blandos, blanquecinos, adheridos a los tallos pilosos, preferentemente del cuero cabelludo, axilas, cejas, pestañas y con menor frecuencia, en la barba, el bigote, el pubis y el perineo. Suele aparecer con mayor frecuencia en zonas de climas templados y tropicales. En los tres casos reportados se tomó muestra de los cabellos con los nódulos sospechosos, se le realizaron pruebas diagnósticas que permitieron confirmar la presencia de estructuras levaduriformes compatibles con Trichosporon spp. Frecuentemente, el motivo de consulta de los pacientes con piedra blanca es porque sospechan que padecen de pediculosis, que es la infestación por piojos, confundiendo los nódulos con liendres; por consiguiente es importante que el médico tenga claro las enfermedades micóticas con las cuales se debe hacer el diagnóstico diferencial para poder orientar y establecer el adecuado tratamiento para el paciente.
Introducción:Las infecciones asociadas a la atención en salud (IAAS) son aquellas infecciones que el paciente adquiere mientras recibe tratamiento y en quien la infección no se había manifestado ni estaba en periodo de incubación en el momento de ingreso. Las IAAS son causadas por bacterias, hongos y virus. Por su parte, la resistencia a los antimicrobianos se produce cuando los microorganismos sufren cambios que no permiten la acción de los medicamentos empleados para tratar las infecciones. Debido a esto, la emergencia y rápida diseminación de infecciones por microorganismos resistentes a los antimicrobianos se considera un problema de salud pública que se asocia con la estancia hospitalaria, morbimortalidad y altos costos para el sistema de salud. En adición a esta problemática, las complicaciones infecciosas constituyen una de las causas más importantes de morbimortalidad en los pacientes oncológicos. Actualmente, las mortalidades más altas se encuentran asociadas a enterobacterias y P. aeruginosa. Es por esto, que el conocimiento adecuado de las infecciones en los pacientes oncológicos y su correcto manejo y prevención son determinantes para reducir sus complicaciones. Método: búsqueda de literatura en bases de datos indexadas. Resultados: En esta revisión se discute la importancia del estudio de las IAAS, las principales bacterias Gram negativas causantes de IAAS en pacientes oncológicos, el uso de antimicrobianos y los principales mecanismos de resistencia antimicrobiana.Conclusión: El estudio y entendimiento de la resistencia antimicrobiana en los procesos infecciosos de pacientes oncológicos, como las IAAS, son indispensables para el control, manejo y prevención de estas infecciones, con el fin de mejorar los tratamientos de los pacientes. Objetivo general: determinar la efectividad del examen histológico frente al cultivo, para el diagnóstico de Helicobacter pylori, en pacientes del Eje Cafetero, 2018. Materiales y métodos: estudio de pruebas diagnósticas, derivado de una cohorte. Se incluyeron 95 registros de pacientes que requirieron interconsulta por gastroenterología en las capitales de Risaralda y Caldas, se les realizó histopatología para el diagnóstico de infección por Helicobacter pylori (Hp). Para el diagnóstico de Hp se hizo un análisis de los resultados del examen histológico (1 biopsia/paciente) y cultivo (3 biopsias/paciente). Se calculó la sensibilidad, especificidad, valor predictivo positivo y valor predictivo negativo. Resultados: 77,8 % de los pacientes presentaron comorbilidades; el 61,7 % (58/94) de los pacientes presentaron cultivo positivo para H. pylori, mientras que la presencia de H. pylori en el examen histológico fue del 50 %. Se encontró relación estadísticamente significativa entre el resultado del examen histológico y el cultivo como Gold estándar (p=0,001) para el diagnóstico de infección por Hp. Para el examen histológico se dio una sensibilidad del 63,6 % y una especificidad de 71,4 %. Conclusiones: el cultivo resultó ser la mejor prueba para el diagnóstico definitivo de casos positivos de Helicobacter pylori, con niveles superiores de especificidad y sensibilidad; no obstante, el examen histológico resulta una muy buena opción para la detección de Hp, considerando las condiciones de acceso al sistema de salud de la población colombiana.
La infección por Helicobacter pylori afecta alrededor del 50% de la población mundial y es el principal agente etiológico de la gastritis crónica, la ulcera péptica, la duodenal y el cáncer gástrico. Cerca del 30% de los casos son resistentes al tratamiento con antibióticos y se requiere un estricto seguimiento durante el tratamiento. El objetivo de este estudio fue determinar la frecuencia, las características clínicas y sociodemográficas relacionadas con el abandono del tratamiento en un grupo de pacientes sometidos a esofagastroduodenoscopia y diagnosticados con infección por H. pylori en ciudades del eje cafetero 2018. El método utilizado es un estudio observacional, descriptivo y longitudinal. Entre los resultados se observó que, de la totalidad de pacientes (N: 94), el 74,5% debió tratarse (70/94) y, de estos, el 88,6% estuvo en tratamiento (62/70). Al finalizar el estudio, se observó que aproximadamente el 84% de los pacientes que recibieron tratamiento lograron terminarlo (52/62). Se concluye que la frecuencia de abandono fue del 16% y, al comparar las características tanto sociodemográficas como clínicas entre los que terminaron y no terminaron su tratamiento, no hubo ninguna relación significativa.
Helicobacter pylori is a Gram-negative, microaerophilic bacterium with the ability to survive in extreme acid environments, with pH below 4.0. It is estimated that more than half of the world’s population is infected with this bacterium. Objective: To determine the prevalence, clinical and demographic characteristics of Helicobacter pylori infection in patients undergoing esophagogastroduodenoscopy in two specialized centers of the Coffee Growing Axis in Pereira and Manizales from February to October 2018. Method: Cross-sectional descriptive observational study in which the proportion of positivity to Helicobacter pylori in a period of time was determined. Results: The study included 94 patients; the prevalence of positivity to Helicobacter pylori was 86.1% (81/94), 56 (59.5%) of which were diagnosed by culture and 52 (55.3%) by histology. In the histological study, of the 24 (25.5%) subjects who presented precursor lesions of malignancy, 9 (9.5%) were positive for Helicobacter pylori, while in the 70 subjects (74.5%) without precursor lesions of malignancy, 43 (45.7%) were positive. The significant variables for Helicobacter pylori infection were: Histological response for precursor lesions of malignancy: OR of 0.3808 (95% CI: 0.1408-0.9907) p=0.04 and epigastric pain: OR 0.09489 (95% CI: 0.01928-0.3637) p=0.001. Conclusion: The frequency of Helicobacter pylori infection in the Departments of Risaralda and Caldas is high, and does not discriminate against gender, age, race or origin of the study subjects; Epigastric pain continues to be the most relevant symptom in the diagnosis of Helicobacter pylori infection and the combination of diagnostic tests improves the diagnosis of Helicobacter pylori infection.
