BackgroundResearch has shown that fecal biomarkers are useful to assess the activity of inflammatory bowel disease (IBD). The aim of the study is: to evaluate the efficacy of the fecal lactoferrin and calprotectin as indicators of inflammatory activity.FindingsA total of 78 patients presenting inflammatory bowel disease were evaluated. Blood tests, the Crohn's Disease Activity Index (CDAI), Mayo Disease Activity Index (MDAI), and Crohn's Disease Endoscopic Index of Severity (CDEIS) were used for the clinical and endoscopic evaluation. Two tests were performed on the fecal samples, to check the levels of calprotectin and lactoferrin. The performance of these fecal markers for detection of inflammation with reference to endoscopic and histological inflammatory activity was assessed and calculated sensitivity, specificity, accuracy.A total of 52 patient's samples whose histological evaluations showed inflammation, 49 were lactoferrin-positive, and 40 were calprotectin-positive (p = 0.000). Lactoferrin and calprotectin findings correlated with C-reactive protein in both the CD and UC groups (p = 0.006; p = 0.000), with CDAI values (p = 0.043; 0.010), CDEIS values in DC cases (p = 0,000; 0.000), and with MDAI values in UC cases (p = 0.000).ConclusionFecal lactoferrin and calprotectin are highly sensitive and specific markers for detecting intestinal inflammation. Levels of fecal calprotectin have a proportional correlation to the degree of inflammation of the intestinal mucosa.
RESUMO: Dentre as causas de defecação obstruída, a disfunção do pubo-retal ou anismus é a mais frequente e o seu diagnóstico é feito com maior frequência através de eletromiografia , considerado como o melhor exame para o diagnóstico desta afecção. O objetivo deste trabalho é verificar a influência do anismus ou contração paradoxal do pubo-retal nos exames de fisiologia anal. Para este trabalho foram estudados 40 pacientes com diagnóstico de defecação obstruída e separados em dois grupos:com e sem anismus. Todos os pacientes foram submetidos aos exames de manometria, sensibilidade retal, eletromiografia, latência do nervo pudendo e proctografia com a finalidade de observar a influência da presença de anismus nos resultados de exames de fisiologia anal. Não observamos diferenças entre os dois grupos em relação aos exames realizados, exceto na eletromiografia que é considerada padrão ouro e no ângulo ano retal na proctografia em que nos pacientes com anismus foi significantemente menor. Conclusão: a presença de anismus interferiu nos resultados dos exames de fisiologia anal apenas na eletromiografia, considerado padrão ouro para esse diagnóstico. Pode ser classificada em dois tipos: transito colônico lento ou inércia colônica e defecação obstruída (4; 5;6;7).A defecação obstruída é a constipação que ocorre por distúrbios da dinâmica da evacuação, como inabilidade para evacuar o volume do reto, sensação de reto repleto de fezes, dor retal, descida do diafragma pélvico e esforço evacuatório (5).Dentre as causas de defecação obstruída, a disfunção do pubo-retal ou anismus é frequente (3) e o seu diagnóstico é feito através de eletromiografia , considerado como o melhor exame para o diagnóstico desta afecção (2).O objetivo deste trabalho é verificar a utilização dos exames de fisiologia anal no diagnóstico da contração paradoxal do pubo-retal em pacientes com constipação .
RESUMO: INTRODUÇÃO -A retocele é causa comum de constipação por defecação obstruída. Freqüentemente está relacionada com outras causas de defecação obstruída, e os exames de fisiologia anal são importantes para o diagnóstico preciso da causa de constipação intestinal. OBJETIVO -observar a influência da retocele nos exames de fisiologia anal, e a necessidade da realização desses após o diagnóstico da retocele. MÉTODO -Para este trabalho foram estudados 40 pacientes com diagnóstico de defecação obstruída. Todos os pacientes foram submetidos aos exames de manometria, sensibilidade retal, eletromiografia, latência do nervo pudendo e proctografia, e foram separados em 2 grupos: portadores e não portadores de retocele . Foram encontrados 18 pacientes com retocele, e o diagnóstico mais freqüentemente associado foi o anismus . Em relação aos exames foi encontrada diferença significante apenas na proctografia no ângulo ano retal, demonstrando que a presença de retocele não interferiu nos resultados de exames de fisiologia anal quando comparados com outros constipados, mas esteve associada a outras causas de constipação, como anismus que necessita de outro tipo de tratamento. CONCLUSÃO -A retocele não interferiu nos valores dos exames de fisiologia anal, mas esteve associada a outros diagnósticos, sendo importante a realização dos mesmos.Descritores: Retocele; Constipação Intestinal; Técnicas de Diagnóstico e Procedimentos. Trabalho Realizado Na Área de Coloproctologia do Departamento de Cirurgia da Faculdade de Ciências Médicas da Santa Casa de INTRODUÇÃOA constipação intestinal pode ser definida como a ocorrência de menos que três evacuações por semana ou evacuação com dificuldade, incompleta ou prolongada (13; 12; 11) podendo se classificar em 2 tipos principais : transito colônico lento ou inércia colônica e defecação obstruída (2; 3 ; 7; 6).Dentre as causas da defecação obstruída a retocele é uma bolsa da parede anterior do reto e da parede vaginal posterior para a luz da vagina, e que freqüentemente é encontrada quando as pacientes são investigadas por constipação (8), considerada clinicamente quando maior que 20 mm. (5) É achado comum e pode ser considerada uma variável normal, e sua contribuição na desordem evacuatória é complexa e não clara (10;4).São sintomas da retocele o esvaziamento incompleto, sensação de pressão retal e sintomas vaginais. Pacientes constipados usualmente tem sintomas similares (8,9).
INTRODUÇÃO: a constipação é um sintoma de doença multifatorial. O diagnóstico correto é importante para orientar a terapêutica. Nas formas de defecação obstruída há vários fatores relacionados como gênero, idade, hábitos, paridade, doenças associadas e distúrbios específicos da evacuação. Entre os métodos para diagnóstico a manometria é usada pela facilidade técnica e disponibilidade. OBJETIVO: verificar o valor da manometria isoladamente em constipados por defecação obstruída. MÉTODO: examinamos quarenta pacientes do Ambulatório de Coloproctologia da Santa Casa de São Paulo com diagnóstico de defecação obstruída. As medidas de pressão retal e anal foram comparadas com um grupo controle de 60 indivíduos considerados normais do ponto de vista proctológico. Separados os pacientes consoante a causa da constipação, verificou-se o valor do método manométrico em cada causa específica. RESULTADOS: houve somente diferenças entre as medidas de pressão retal e anal em repouso e pressão máxima de contração entre os normais e os vários tipos de constipados, mas não diferenças específicas entre as várias modalidades de constipação. CONCLUSÃO: os vários métodos de fisiologia anal são importantes e necessários em conjunto para o diagnóstico correto. A manometria contribui para a investigação dos distúrbios funcionais, devendo sempre ser incluída. Contudo, seu valor como método isolado é questionável.
