INTRODUÇÃO: A pandemia de covid-19 tornou necessário estudos sobre o impacto da pandemia e das novas rotinas de trabalho impostas ao trabalhador no estado de saúde de profissionais de saúde, em especial os fisioterapeutas hospitalares. OBJETIVO: Avaliar a qualidade de sono e sonolência diurna de profissionais de fisioterapia hospitalar durante o período de pandemia do covid-19. MATERIAIS E MÉTODOS: Pesquisa observacional, transversal, prospectiva em um hospital público do nordeste brasileiro. A pesquisa teve como público-alvo fisioterapeutas hospitalares atuantes ou não em setores covid durante a pandemia de covid-19. Foram aplicados os instrumentos Índice de Qualidade do Sono de Pittsburgh (IQSP), Escala de Sonolência de Epworth (ESE) e questionários com características demográficas, trabalho, saúde e percepção de estresse. RESULTADOS: Participaram do estudo 45 fisioterapeutas e foi observado que 62,2% eram do sexo feminino, 66,7% relataram trabalhar 60h por semana e 55,6% trabalharam em setor covid e não covid. Observou-se elevada frequência de má qualidade do sono (68,9%) independente de carga horária ou setor de trabalho. Houve maior prevalência de sonolência diurna excessiva (43,3%) entre os fisioterapeutas que trabalhavam 60h por semana. CONCLUSÃO: Os fisioterapeutas hospitalares de uma instituição pública têm má qualidade do sono e aqueles que trabalham com maior carga horária apresentam maior prevalência de sonolência diurna excessiva.
Introdução: Um recurso imprescindível da UTI é a ventilação mecânica invasiva (VMI), utilizada em pacientes com insuficiência respiratória grave. O processo de transição da VMI para a respiração espontânea sofre influências de diferentes sistemas biológicos que podem levar a indução da inflamação sistêmica. Objetivo: explorar se marcadores inflamatórios clinicamente acessíveis são capazes de predizer o sucesso/insucesso de extubação em pacientes adultos. Métodos: pesquisa quantitativa, retrospectiva, longitudinal e analítica realizado em duas UTI adulto em um hospital universitário. A amostra foi dividida em grupo sucesso (GS) e grupo insucesso (GI). Coletou-se os resultados de exames laboratoriais, PCR, neutrófilos, linfócitos, leucócitos, promielócitos, mielócitos, metamielócitos e bastonetes, no período de 48 e 24 horas antes, no dia, 24 e 48 horas após a extubação. Resultados: Dos 147 pacientes, 39 (27%) falharam no processo de extubação. O GI apresentou maior tempo de internação em UTI com p<0,0001 e desfecho óbito durante internamento com p=0,03 (44%). A razão neutrófilos por linfócitos no GI apresentou valores elevados no dia, 24 e 48 horas após a extubação comparados ao GS com p=0,0035, p=0,0005 e p<0,0001, respectivamente. O ponto de corte da RNL no dia da extubação para prever o insucesso foi >6,9, com sensibilidade de 74,4% (IC 95% 57,9 a 86,9), especificidade de 58,5% (IC 95% 48,5 -68,0). Conclusão: A RNL emergiu como um marcador inflamatório eficaz, simples e de fácil obtenção, sugere-se que seja considerada por profissionais da saúde no dia da extubação como potencial preditor de insucesso e avaliada nas 48 horas subsequentes ao processo.Palavras-chave: Inflamação. Observou-se que 25% (2) dos pacientes evoluíram para IOT e posteriormente para óbito, estando à idade avançada diretamente relacionada com este desfecho. Considerações Finais: O uso de VNI pode ser considerado como alternativa no tratamento de pacientes com COVID-19. Contudo, os diversos fatores intrínsecos e extrínsecos ao paciente ainda contribuem para a alta taxa de IOT e de mortalidade. Com evidências científicas robustas nesta população específica escassa, ressaltase a necessidade de novos estudos acerca de estratégias terapêuticas no contexto de pacientes criticamente enfermos com COVID-19.Palavras-chave: Falência respiratória; Coronavírus; Ventilação não invasiva. ABSTRATObjective: To evaluate the use of non-invasive positive blood pressure therapy in the outcome of patients with acute or chronic respiratory failure exacerbated by COVID-19. Methodology: Research carried out in the ICU-COVID of a federal public hospital located in the city of Teresina, capital of Piauí, on an observational and retrospective basis through data collection in electronic medical records from November 2021 to January 2022 with patients undergoing to the use of NIV. Results and Discussion: 8 patients were included in the study, 75% (6) of whom were male, with a mean age of 50.8 ± 13.2 years. The mean time of NIV use was 5 ± 2.26 days. It was observed that 25% (2) of the patients evolved to OTI and subsequently died, with advanced age being directly related to this outcome. Final Considerations: The use of NIV can be considered as an alternative in the treatment of patients with COVID-19. However, the various intrinsic and extrinsic factors to the patient still contribute to the high rate of OTI and mortality. With robust scientific evidence in this scarce specific population, the need for further studies on therapeutic strategies in the context of critically ill patients with COVID-19 is highlighted.
Introdução: Durante a pandemia de COVID-19, tornou-se necessário o uso de estratégias no tratamento dos pacientes que evoluem com insuficiência respiratória aguda. Objetivo: Avaliar o uso de estratégias não invasivas no desfecho de pacientes com insuficiência respiratória aguda ou crônica agudizada por COVID-19. Métodos: Pesquisa de caráter observacional e retrospectivo por meio da coleta de dados em prontuário eletrônico com pacientes submetidos ao uso de cânula nasal de alto fluxo (CNAF) e/ou ventilação mecânica não invasiva (VNI). Resultados: 81 pacientes, sendo 70,4% (57) do sexo masculino, com 56,5 ± 14,6 anos. 49,4% (40) dos indivíduos fizeram uso de CNAF e VNI, 9,9% (8) e 40,7% (33) apenas VNI ou CNAF, respectivamente. O tempo médio de uso da CNAF foi de 4,4 ± 3,7 dias e de VNI foi de 2,7 ± 3,4 dias. Observou-se que 43 (53,1%) dos pacientes pesquisados evoluíram para intubação orotraqueal (IOT) e 40 (49,4%) para óbito. Destes, 22 encontravam-se em IOT. Houve diferença estatística quando comparados idade entre os grupos IOT e não IOT, 60,5 ± 13,9 anos vs 52,1 ± 14,2 anos (p = 0,012), respectivamente. Conclusão: O uso de VNI e/ou CNAF pode ser considerado como alternativa no tratamento de pacientes com COVID-19. Contudo, os diversos fatores intrínsecos e extrínsecos ao paciente ainda contribuem para a alta taxa de IOT e de mortalidade.

Na prática, isso funciona dentro de um único sistema, em que cada esfera do governo possui um órgão para administrar os serviços de saúde de cada local. É o caso das Secretarias de Saúde municipais, por exemplo, que além de gerir, ainda traçam estratégias de vacinação e executam as ações necessárias para que os postos de saúde e hospitais consigam atender à demanda da população local.Vale ressaltar que o Sistema Único de Saúde (SUS) é um dos maiores e mais complexos sistemas de saúde pública do mundo, abrangendo desde o simples atendimento para avaliação da pressão arterial, por meio da Atenção Primária, até o transplante de órgãos, garantindo acesso integral, universal e gratuito para toda a população do país. Com a sua criação, o SUS proporcionou o acesso universal ao sistema público de saúde, sem discriminação. A atenção integral à saúde, e não somente aos cuidados assistenciais, passou a ser um direito de todos os brasileiros, desde a gestação e por toda a vida, com foco na saúde com qualidade de vida, visando a prevenção e a promoção da saúde.
