INTRODUCCIÓN:El mucocele apendicular es una entidad clínica rara que con poca frecuencia se considera en el diagnóstico diferencial del dolor localizado en el cuadrante inferior derecho del abdomen.CASO CLÍNICO: Se trata de un paciente masculino de 38 años de edad que acude al servicio de emergencia por dolor abdominal de 2 días de evolución referido a FID. Al examen físico se evidenció un abdomen blando, depresible, doloroso a la palpación superficial y profunda en FID con la presencia de una masa de 6 x 4 cm aproximadamente. La biometría reveló leucocitosis y predominio de segmentados. EVOLUCIÓN:Se realizó una laparotomía exploratoria en la que se encontró un apéndice cecal con aumento de su diámetro, engrosado y traslúcido de 8 x 4 cm por lo que se procedió a realizar una apendicetomía por la técnica habitual. El resultado de la biopsia tuvo como resultado un quiste mucinoso de apéndice cecal. CONCLUSIÓN:El mucocele apendicular es una patología poco frecuente que debe ser considerada dentro del diagnóstico diferencial del dolor abdominal por las múltiples complicaciones derivadas de un abordaje inadecuado y su potencial de malignidad.
23 RESUMENOBJETIVO: Describir las características clínicas de los pacientes con diagnóstico de hernia ventral e incisional intervenidos para resolución con cirugía laparoscópica y los resultados obtenidos. MÉTODO:Se incluyeron los pacientes sometidos a reparación laparoscópica de hernias ventrales e incisionales realizadas durante el periodo entre enero de 2013 hasta Diciembre de 2016. Las variables de estudio fueron: sexo, edad, tipo de hernia, diámetro de la hernia, sín-toma principal y complicaciones transoperatorias y postoperatorias. RESULTADOS:Se incluyeron 21 pacientes, predominó el sexo masculino (52.38 %) y los grupos de edad más frecuentemente afectados estuvieron entre 50 y 69 años de edad (80.95 %). El 52.38 % de pacientes tuvieron un diagnóstico de hernia incisional y el 57.15 % de los defectos herniarios tuvieron un tamaño mayor a 10 cm (: 11.80 ±4.06 cm). La tasa de conversión fue del 4.76 % y el 9.52 % (n=2) de los pacientes refirió continuar con dolor luego de 3 y 6 meses después de la cirugía.CONCLUSIÓN: Las características de los pacientes intervenidos fueron similares a las reportadas en la bibliografía disponible, la experiencia en la resolución laparoscópica de hernias ventrales e incisionalesen el Centro de Salud tipo B IESS de La Troncal fue satisfactoria. *DESCRIPTORES DeCS AIM: To describe clinical characteristics of patients diagnosed with ventral and incisional hernias treated with laparoscopic surgery and its results. RESUMEN INTRODUCCIÓN:El embarazo en adolescentes es tema de Salud Pública. El objetivo fue determinar la prevalencia en el Hospital de Especialidades José Carrasco Arteaga durante el año 2015.METODOLOGÍA: Se trata de un estudio descriptivo transversal de prevalencia y las características asociadas con la vía del parto. Se analizaron 125 historias clínicas de adolescentes registradas en el departamento Toco Quirúrgico del departamento de Ginecología y Obstetricia del Hospital de Especialidades José Carrasco Arteaga de Cuenca -Ecuador, 2015. RESULTADOS:La prevalencia del embarazo en adolescentes es 5.8 %, el 40 % parto eutócico, el 50.4 % cesárea y el 9.6 % abortos. La adolescencia tardía representó el 65 %, cursan el nivel secundario 83 %, el 63 % continúan sus estudios. El estado civil: solteras 76 %, residencia urbana 72 %. El 88 % curso su primer embarazo, el 80 % no utilizó métodos anticonceptivos, el 76 % inició su vida sexual a los 14 -16 años, el 80 % tiene una pareja sexual, el 2.4 % presentaron enfermedades de transmisión sexual y el 100% se reportó serología negativa. CONCLUSIÓN: La prevalencia del embarazo en adolescentes en el Hospital de Especialidades JoséCarrasco Arteaga en el año 2015 fue del 5.8 %, es menor en comparación con las estadísticas locales, nacionales e internacionales. La edad gestacional se consideró pre término, termino, postér-mino y aborto según la OMS. Se detalla las complicaciones durante la gestación, vía del parto, causa de cesárea, peso del recién nacido y valor del APGAR.Se consideró el registro del área toco quirúrgico (partos, cesáreas y abortos) en las historias clínicas. Se utilizó el formulario elaborado por los autores y la información obtenida se registró en la base de datos en Excel. Los resultados son expresados en tablas de distribución de frecuencias.
