BACKGROUND Visceral leishmaniasis (VL) caused by Leishmania infantum is characterised by the loss of the ability of the host to generate an effective immune response. Chemokines have a direct involvement in the pathogenesis of leishmaniasis, causing a rapid change in the expression of these molecules during infection by Leishmania.OBJECTIVES Herein, it was investigated the role of CXCL10 in controlling infection by L. infantum.METHODS RAW 264.7 macrophages were infected with L. infantum in vitro and treated or not with CXCL10 (25, 50 and 100 ng/mL). Parasite load, as well as nitric oxide (NO), IL-4 and IL-10 production were assessed at 24 and 48 h after infection. In vivo, BALB/c mice were infected and treated or not with CXCL10 (5 μg/kg) at one, three and seven days of infection. Parasite load, IFN-g, IL-4, TGF-β and IL-10 were evaluated one, seven and 23 days post treatment.FINDINGS In vitro, CXCL10 reduced parasitic load, not dependent on NO, and inhibited IL-10 and IL-4 secretion. In vivo, CXCL10 was able to reduce the parasite load in both liver and spleen, four weeks after infection, representing a higher decrease in the number of parasites in these organs, also induced IFN-γ at day 23 after treatment, correlating with the decrease in parasite load, and reduced IL-10 and TGF-β.MAIN CONCLUSIONS This study suggests a partial protective role of CXCL10 against L. infantum, mediated by IFN-g, not dependent on NO, and with suppression of IL-10 and TGF-β. These data may provide information for the development of new approaches for future therapeutic interventions for VL.
Resumo: Este estudo avaliou a prevalência e os fatores associados a não procura por transplante renal entre pacientes em diálise crônica na Região Metropolitana de Fortaleza, Ceará, Brasil. Foram excluídos os pacientes sem condições clínicas e aqueles em avaliação pré-transplante renal. Um questionário semiestruturado foi aplicado, incluindo opções para a pergunta “Qual o principal motivo pelo qual o senhor/senhora não está inscrito(a) para o transplante renal?”. A prevalência de pacientes considerados aptos, mas não inscritos e nem em avaliação pré-transplante renal foi de 50,7%. As principais causas foram: receio de insucesso/perda do enxerto (32,5%), dificuldade de transporte e acesso aos exames (20,9%) e problemas pessoais ou familiares temporários (13,7%). Em análise múltipla, as variáveis associadas a risco de receio do insucesso/perda do enxerto foram: sexo feminino (OR = 1,763; IC95%: 1,224-2,540) e doença renal dialítica (DRC 5-D) por hipertensão (OR = 1,732; IC95%: 1,178-2,547), tendo a renda mensal (salários mínimos) uma associação de proteção (OR = 0,882; IC95%: 0,785-0,991). O tempo em diálise (meses) foi um fator de risco para a dificuldade de transporte e acesso aos exames (OR = 1,004; IC95%: 1,001-1,007) e o sexo feminino apresentou uma associação de proteção (OR = 0,576; IC95%: 0,368-0,901). Esses resultados mostram elevada prevalência de pacientes em diálise fora de lista para transplante renal. As principais causas são reflexo da desinformação e falta de acesso. Sexo feminino, baixa renda e DRC 5-D por hipertensão foram os fatores de risco para a não procura por transplante renal por receio de perda do enxerto, reflexo da carência de informações sobre a modalidade. Sexo masculino e maior tempo em diálise foram os fatores de risco para a dificuldade de acesso à terapia.
As leishmanioses são antropozoonoses causadas por protozoários do gênero Leishmania, consideradas grande problema de saúde pública e representam um complexo de doenças com importante espectro clínico e grande diversidade epidemiológica. Este trabalho visa divulgar o conhecimento, por meio de uma revisão bibliográfica, sobre manifestações clínicas das leishmanioses, assim como epidemiologia e tratamento, incluindo o uso de plantas medicinais. Para isso foram utilizadas as bases de dados LILACS, MEDLINE, SCIELO e PUBMED com leishmaniose servindo como descritor nuclear e leishmaniose tegumentar, visceral e tratamento como descritores complementares. As leishmanioses foram diferenciadas em dois tipos, visceral e tegumentar. Os pacientes podem apresentar febre prolongada, hepatoesplenomegalia, leucopenia, hipergamaglobulinemia, perda de peso e se não tratados, evoluem para um quadro de caquexia, entre outros sinais e sintomas. Essas patologias estão presentes, endemicamente, em 98 territórios, e causam cerca de 30.000 mortes por ano. Portanto, é de fundamental importância que essa doença seja tratada. No Brasil, até 2014, o protocolo de medicamentos de primeira escolha no tratamento das leishmanioses era o uso de antimoniais pentavalentes. Quando os antimoniais não são efetivos, outras medicações usadas incluem pentamidina e anfotericina B. Nesse contexto, as plantas medicinais também surgem como uma alternativa terapêutica para o tratamento dos diversos tipos de manifestação da leishmaniose. Destarte, fica nítida a necessidade de aprofundar-se os estudos sobre a leishmaniose, uma doença endêmica do Brasil e que possui manifestações graves.
Introduction: Immunoglobulins, soluble antigens, cells, cytokines and other immune system products can be transferred from infected mother to her offspring, leading to suppression or stimulation of immune response. Objective: To evaluate the influence of gender and maternal infection with Leishmania braziliensis in the course of the disease in the offspring of hamsters. Meodths: Offspring born from infected mother (IMO) or non-infected mother (NIMO) by Leishmania braziliensis, both sexes, was infected with the same strain of the mother after 30 days of life and followed for 18 weeks. We evaluated the thickness of the lesion, parasite load and histology of the lesions. Results: The number of parasite in both lesions and lymph node of IMO offspring showed a significant reduction in the 5th week post-infection compared to the NIMO offspring; however, this did not correspond to clinical symptoms. Histopathological analysis revealed that in the IMO offspring, the inflammatory process was more prominent. In relation to gender, it was observed that the male offspring showed lesion thickness and higher parasite burden than females. Conclusion: Maternal infection by L. braziliensis in hamsters does not appear to influence the course of the disease in the homologous offspring infection, as well as the male offspring presented augmented susceptibility to L. braziliensis infection regardless of whether they were born from IMO or NIMO. Also, the reduction of the granuloma index in the IMO offspring, together with the higher inflammatory response, suggests a less effective cellular response in the chronic phase of the disease in these animals.
