White-cell count and differential blood count, total protein, lactate dehydrogenase activity (E.C. 1.1.1.27) and lactate concentration were determined in 496 CSF samples, obtained by lumbar puncture. Lactate was measured with a new enzymatic test. Reference values were determined for lactate and lactate dehydrogenase (90% limits: 1.2-2.1 mmol/l and 6-26 U/l, respectively). Lactate content proved to be best in the differential diagnosis between acute bacterial and abacterial meningitis: concentrations of 3.5 mmol/ml and above were found exclusively in bacterial meningitis. As for other diseases, markedly increased lactate concentrations were measured only in patients with brain tumour or cerebrovascular disease (up to 9.3 mmol/l). Determination of lactate concentration alone would have led to a misdiagnosis of acute bacterial meningitis in 3% of cases. If the CSF contains 3.5 mmol/l lactate or more and the leucocyte cell count is above 800/microliter, brain tumour or cerebrovascular disease can be practically excluded and the diagnosis of bacterial meningitis made with a high degree of reliability.
Zusammenfassung: Mit einem neuen enzymatischen Test kann der Lactatgehalt im enteiweißten Blut und im Plasma rasch und sicher bestimmt werden. Bei der "Plasmamethode" (Verdünnungsgrenze 18 mmol/1) war auch mit hohen Lactatkonzentrationen die Reaktion bei Raumtemperatur in 10 Minuten abgelaufen; die "Methode mit Enteiweißung" (Verdünnungsgrenze 10 mmol/1) erforderte 15 Minuten. Präzision und Wiederfindung waren gut. Störungen durch Lipämie, Hämolyse, Bilirubin, Metabolite, Enzyme und Pharmaka wurden nicht beobachtet. Kurze, venöse Stauung hatte keinen Einfluß auf den Lactatgehalt. Eine rasche Glykolysehemmung war durch die Kombination von Glykolysehemmer mit sofortiger Kühlung des abgenommenen Blutes im Eisbad möglich. Die niedrigsten Plasma-Lactat-Werte gaben unter diesen Bedingungen Natriumfluorid/EDTA und danach Kaliumfluorid/EDTA. Zwischen 30 Minuten und 2 Stunden nach Blutabnahme war bei allen Glykolysehemmern die Lactatkonzeiitration konstant. Nach dem Abzentrifugieren der Erythrocyten konnte das Plasma -ohne daß sich die Lactatkonzentration änderte -bis zu 6 Tage bei Raumtemperatur aufbewahrt werden.Mit der "Plasmamethode" wurden zwischen 8 und 23% höhere Werte als mit der "Methode mit Enteiweißung" gefunden. Diese Unterschiede lassen sich auf einen variablen und relativ niedrigen Lactatgehalt in den Erythrocyten zurück-fuhren. When the "plasma method" was used, the reaction ran to completion in 10 minutes at room temperature even at high lactate concentrations (up to 18 mmol/1), whereas it required 15 minutes at room temperature when deproteinized blood was used (linear range up to 10 mmol/1). Glycolysis can be speedily inhibited by the addition of an appropriate inhibitor to the specimen, which must be cooled in an ice-bath immediately after being obtained. Under these conditions, the lowest plasma lactate values were found with sodium fluoride/EDTA and the second-lowest with potassium fluoride/EDTA. The lactate concentration remained constant, irrespective of the glycolysis inhibitor used, between 30 minutes and 2 hours after blood sampling. Plasma can be stored up to 6 days at room temperature with no effect on the lactate concentration.The "plasma method" gave values 8-23% higher than the method using deprpteinized blood. A new and rapid vital-stain method for the diagnosis of CSF cells demonstrates not only red blood cells but also white blood cell types, predominantly by differentiating nuclear patterns. 20 microliter untreated CSF are put on the prestained slides and a cover slip put over it. Differentiation can be made after 15 minutes. The lower limit is at 3-4 WBC/ml of CSF. At room temperature the preparation keeps for about an hour. The method is reproducible and has the advantage also over other routine methods that the CSF volume is very small and there is neither loss of cells nor cell denaturation.
Zusammenfassung: Für eine modifizierte Methode nach Hillmann (1) zur Bestimmung der katalytischen Konzentration an saurer Phosphatase im Serum (Substrat: a-Naphthylphosphat) werden Ergebnisse mitgeteilt.In einem Kollektiv von 158 Männern im Alter von 16 bis 85 Jahren ergab sich als Obergrenze des Referenzbereiches (95. Perzentil) für die gesamte saure Phosphatase 4,7 U/l (37 °C) bzw. 4,2 U/l (30 °C) und für die tartrathemmbare saure Phosphatase 1,6 U/l (37 °C) bzw. 1,5 U/l (30 °C). Bei 60 Frauen im Alter von 18 bis 80 Jahren lag die Obergrenze des Referenzbereiches (95. Perzentil) für die gesamte saure Phosphatase bei 3,7 U/l (37 °C) bzw. 3,0 U/l (30 °C).Eine Altersabhängigkeit der katalytischen Konzentrationen war weder bei Männern noch Frauen nachweisbar.Bei 89 Patienten mit Prostata-Carcinom der Stadien C und D wurde die katalytische Konzentration der tartrathemmbaren sauren Phosphatase mit den Substraten -Naphthylphosphat und p-Nitrophenylphosphat bestimmt.Bei 74 dieser Patienten wurde die Konzentration an prostataspezifischer saurer Phosphatase mit einem Enzyrri-Immunoassay sowie einem Radio-Immunoassay ermittelt.Die Sensitivität der Methode mit p-Nitrophenylphosphat ist unbefriedigend (66%), die anderen Methoden ergeben eine bessere und vergleichbare Seiisitivität (ca. 90%). Die Ergebnisse mit p-Nitrophenylphosphat unterscheiden sich signifikant von denen mit -Naphthylphosphat als Substrat; die beiden immunchemischen Verfahren zeigen jedoch gegenüber der neuen Methode keinen signifikanten Unterschied.Es wird deshalb vorgeschlagen, die ältere Methode mit/7-Nitrophenylphosphat zugunsten der neuen Methode aufzugeben.
