des Nerves mrerccs tcbrachialis bei der u irlären lymphOflod ektomi e im Hinblick auf postoperative neurologisch~Defizite wurde an 23 Patientinnen der Univ~rsitätsfrau enklinik Homburgl5aar untersucht, die sich im Rahmen der ope rativen Therapie ihres Mammakarzinoms einer axillären lymphonod ektomie unterzi ehen mußten. Im Zuge der axillären lymphonod ektomie wurde bei 11 Patientinnen der Nervus intercostobrachialis mit seinen Nebenästen erha lten. bei 12 Frauen wurde der Nerv intraoperat iv durchtrennt. Die präope rativ untersuchten Patientinnen wurde n erneut postoperativ zunächst nach 1-2 Wochen und zum zweiten Mal nach 5 MOflaten neurologisch nachun te rsucht. Diese Untersuchung umta gte die 5ensibilität , die 5chmerzsymptomatik und die Motorik, das subjektive Beschwer de bild sowie Änderungen der Sudcmctodk, Ein weiterer Schwerpunkt der Unte rsuchung war die Objektivierung der postoperativen neu rologischen Defizite als folge der Durchtrenn ung des Nerve s rntercostobrachlalls mitt els der Ableitung von sometcsenub el evozierten Potentialen (SSEP). Bei der neurologischen Untersuchun g wurden deutl ich weniger epikrltisr he Sensibilitäts defizite (p < O.OS) und Parästh esien nach Nerverhaltung beob achtet . Dies führt e zu einer rascheren Schmerzfreiheit und zeitlichen Verbesserung der Bewegun gsfreiheit des betroffenen Armes. Mittel s SSEP gelang allerdings keine eindeut ige Objekt ivierung der postoperativen neur ologischen Defizite . Using a balanced anaesthesia technique, we found desflurane as suitable as isoflurane for geriatric patients. Additionally, due to the fast emergence from anaesthesia, an improved cooperativity was found. In the DES group overall better postoperative psychomotor performance resulted in a shortening of discharge times from the recovery room. Hence, desflurane anaesthesia may be advantageous in geriatric patients.
