18F sodium fluoride PET/CT proves to be a useful tool in the diagnosis of complex spine pathology of the postoperative patients. In varied cases, a high correlation of metabolic activity to the source of the patient's pain was observed.
Takayasu's arteritis, known as “pulseless disease,” is a chronic idiopathic inflammatory disease, which has a greater predilection for large vasculature in the body. Initially described in the1800s, this rare condition is more commonly seen in Asian women in 40 years age group. Herein, the authors report the case of a 36-year-old woman whose exertional claudication was the initial manifestation of active Takayasu's arteritis along with involvement of multiple peripheral arteries not involving ostial area of the vessels, instead affecting the proximal area and also presenting with thrombosis of right CIA (common iliac artery), which is an unusual presentation in case of Takayasu's arteritis without aneurysmal involvement.
Primary testicular lymphoma (PTL) a relatively rare disease of non-Hodgkin's lymphomas occurring with a lesser incidence of 1-2% has a propensity to occur at later ages above 50 years. PTL spreads to extra nodal sites due to deficiency of extra cellular adhesion molecules. We present detection of multiple sites of extra nodal involvement of PTL by F-18 positron emission tomography/computed tomography study aiding early detection of the dissemination thus aiding in staging and management.
Background We aimed to recognize the predictors of long-term major adverse cardiac events (MACE) in the elderly candidates for elective percutaneous coronary intervention (PCI) in relation to gender at our center.
Methods In this retrospective cohort study, we reviewed the data of the elderly patients (age ≥70 years) who underwent elective PCI who met our study criteria in our institution during 2008 and 2018. Demographical data, clinical history, angiographic details, PCI procedure, and follow-up data of the patients enrolled in the study were studied by using the angiographic and PCI procedure details. Patients were characterized in the study group as those with or without MACE, which were then compared and analyzed using the statistical analysis in a univariable and binary linear regression analysis.
Results A total of 355 elderly patients (older than 70 years) undergoing elective PCI were selected who fulfilled the inclusion criteria; 277 patients were men and had more comorbidities, including hypertension, diabetes mellitus (DM), chronic obstructive pulmonary disease, history of heart failure, previous coronary artery bypass graft, and presentation with acute coronary syndrome. MACE occurred in 24 events patients of whom 20 were suffering from DM. Binary logistic regression showed that the only determinant for the 1-year follow-up outcome is diabetes (p = 0.000). Even in univariate analysis, DM (0.01) is the determinant. DM is a strong predictor for death in univariate analysis (p = 0.00).
Conclusion PCI is a safe and effective method of coronary revascularization in elderly patients, and some risk factors can predict long-term MACE in this group of patients.
Transradial access is the commonly followed route for performing percutaneous coronary diagnostic and angioplasty interventions. Transradial access has drastically decreased the vascular access site complications when compared with the transfemoral access. Arteriovenous fistula developing at the transradial access site is one of the rare complications. The risk factors and the mechanism of development are not entirely known. We present a rare case of a 63-year-old female developing radiocephalic fistula a few days after transradial coronary angiography and discuss management.The transradial route is preferred for cardiovascular interventions. Complications arising out of this are rare even though this route is widely used. Arteriovenous fistula is one of the rare complications, and the mechanisms causing it are not always clearly evident. This report will elaborate on the possible causes and steps to prevent this complication and help in managing once it occurs.
