PurposeTo test whether quantitative functional tests and optical coherence tomography (OCT)-defined structure can serve as effective tools to diagnose and monitor early diabetic neuroretinal disease.MethodsFifty-seven subjects with diabetes (23 without diabetic retinopathy [no DR], 19 with mild nonproliferative diabetic retinopathy [mild NPDR], 15 with moderate to severe [moderate NPDR]), and 18 controls underwent full ophthalmic examination, fundus photography, spectral-domain optical coherence tomography (SD-OCT), e-ETDRS (Early Treatment Diabetic Retinopathy Study) acuity, and the quick contrast sensitivity function (qCSF) method. Perimetry testing included short-wavelength automated perimetry (SWAP), standard automated perimetry (SAP), frequency doubling perimetry (FDP), and rarebit perimetry (RBP).ResultsETDRS acuity and RBP were not sensitive for functional differences among subjects with diabetes. AULCSF, a metric of qCSF, was reduced in diabetics with moderate compared to mild NPDR (P = 0.03), and in subjects with no DR compared to controls (P = 0.04). SWAP and SAP mean deviation (MD) and foveal threshold (FT) were reduced in moderate compared to mild NPDR (SWAP, MD P = 0.002, FT P = 0.0006; SAP, MD P = 0.02, FT P = 0.007). FDP 10-2 showed reduced MD in moderate compared to mild NPDR (P = 0.02), and FDP 24-2 revealed reduced pattern standard deviation (PSD) in mild NPDR compared to no DR (P = 0.02). Structural analysis revealed thinning of the ganglion cell layer and inner plexiform layer (GCL+IPL) of moderate NPDR subjects compared to controls. The thinner GCL+IPL correlated with impaired retinal function.ConclusionsThis multimodal testing analysis reveals insights into disruption of the neuroretina in diabetes and may accelerate the testing of novel therapies.

PurposeTo elucidate the relationship between disorganization of retinal inner layers (DRILs) and retinal function in diabetic patients without diabetic retinopathy (DR) and with nonproliferative DR, but without diabetic macular edema (DME).MethodsFifty-seven participants with diabetes mellitus (DM) and 18 healthy controls underwent comprehensive ophthalmic examination, fundus photography, and spectral-domain optical coherence tomography. Scans of the fovea were evaluated for the presence of DRIL. Retinal function was evaluated using Early Treatment Diabetic Retinopathy Study (ETDRS) visual acuity, the quick contrast sensitivity function (qCSF) on the AST Sentio Platform, short-wavelength automated perimetry (SWAP), standard automated perimetry (SAP), and frequency doubling perimetry (FDP). ANOVA and Kruskal-Wallis were used to compare retinal function in subjects with and without DRIL. Tukey-Kramer test and Wilcoxon were used for post hoc analysis.ResultsDRIL was identified in 9 of 57 diabetic subjects. DRIL subjects had higher body mass index and longer diabetes duration compared to diabetic subjects without DRIL (P = 0.03 and P = 0.009, respectively). Subjects with DRIL had reduced ETDRS visual acuity (P = 0.003), contrast sensitivity function (P = 0.0003), and SAP performance (PSD, P < 0.0001) compared to controls and diabetic subjects without DRIL. Structural analysis revealed inner retinal thinning, and some outer retinal thinning, associated with DRIL.ConclusionsDiabetic subjects with DRIL have reduced retinal function compared to those without DRIL, and defective retinal lamination may be an early cellular consequence of diabetes responsible for this in some patients. Following further longitudinal studies, DRIL may be a readily available and reliable structural biomarker for reduced retinal function in early diabetic neuroretinal disease.
Although morphological changes such as mild glial activation and material remnants were observed histologically 1 month and 3 months after injection in all injected groups, minor cell damage was noted only in the PLGA group at 3 months after injection. No evidence of functional abnormality relative to untreated eyes could be detected by ERG 3 months after injection in all groups. Changes observed in in vivo imaging such as OCT and FAF disappeared after 3 months in almost all cases.
Patients' Families (FBIS-BR), and the results were analysed using the Mann-Whitney test and analysis of variance. Results: the global objective overload, and also in each subscale, was significantly greater in the group of main caregivers; the subjective overload showed no difference between the groups. Comparing the subscales, the routine assistance provided to the patient had greater influence on objective overload in both groups, and the concern with the elderly patient was the dimension that had most influence on the subjective overload of main caregivers and also of secondary caregivers. Conclusion: the differences in overload between the different groups reinforces the need for planning of health care actions for each type of caregiver, seeking to reduce these differences.
Aim: to identify the leisure activities performed in Long-Stay Institutions for the Elderly (LSIEs), registered in the city of Maringá-PR, Brazil, and to analyze the attitude of the elderly people toward leisure promoted by the institutions. METHOD: this was a descriptive and transversal study with a quantitative approach, carried out with 97 elderly people, through the establishment of the socio-demographic profile and the application of the Leisure Attitude Scale. The data was subjected to descriptive statistical analysis, association tests (chi-square or Fisher's) and Spearman's correlation. RESULTS: males, aged 80 or over, widowed, with one to eight years of study, who had a monthly income were predominant. Age group and income were significantly associated with the performance of leisure activities. The results reflected the positive attitude of the elderly people in relation to leisure activities, except in the behavioral component. CONCLUSION: the findings of this study indicate the need for further investigation into the difficulties linked to the attitude toward leisure in the behavioral component, considering aspects such as individual concepts of leisure and the health status of the elderly people.
<p>Objetivo: identificar as atividades de lazer realizadas em uma Instituição de Longa Permanência para Idosos e apreender a percepção dos idosos acerca do lazer. Método: estudo descritivo e qualitativo realizado entre janeiro e abril de 2013 junto a 15 idosos institucionalizados. Os dados foram coletados por meio de entrevistas semiestruturadas, analisados pelos princípios da interpretação temática e discutidos à luz do referencial teórico de Foucault. Resultados: o entendimento do lazer para os idosos estava vinculado ao bem-estar psicossocial. As atividades de lazer praticadas eram em sua maioria artísticas, intelectuais, manuais, físicas e sociais, as quais eram promovidas principalmente pela comunidade externa à instituição. Idosos com déficits sensoriais, motores ou cognitivos tiveram dificuldade e/ ou foram impossibilitados de participar ativamente das atividades. Conclusão: as atividades de lazer realizadas na instituição proporcionavam benefícios aos idosos, contudo eram permeadas por relações de poder.<br />Descritores: Pessoa idosa. RESUMOO objetivo do presente estudo foi compreender o processo de vivenciar o cuidado aos idosos com doença de Alzheimer. Tratou-se de uma pesquisa exploratória e descritiva, com abordagem qualitativa. Os sujeitos da pesquisa foram oito cuidadores familiares de pessoas idosas com Alzheimer, residentes nas áreas de abrangência pertencentes a duas Unidades Básicas de Saúde localizadas no município de Maringá-PR. Os dados foram coletados entre julho e agosto de 2015 por meio de entrevista semiestruturada. Da análise temática dos dados emergiram três categorias: "O diagnóstico de doença de Alzheimer no idoso e as mudanças no cotidiano do cuidador familiar"; "Percepção do cuidador familiar sobre as alterações cognitivas e comportamentais na doença de Alzheimer"; "Preocupações vivenciadas pelos cuidadores familiares no dia a dia com o idoso". Foi constatado que a vivência do processo de adoecimento gera sofrimento, tristeza e insegurança, trazendo grandes mudanças no contexto familiar. O enfermeiro, além da assistência ao idoso com DA, pode oportunizar diálogos e definição conjunta de estratégias de cuidados para a convivência com a doença.Palavras-chave: Doença de Alzheimer. INTRODUÇÃOO envelhecimento populacional é uma realidade conhecida em todo o mundo, que vem ocorrendo de forma abrupta nos países em desenvolvimento, como o Brasil. A queda da fecundidade e da mortalidade e o aumento da expectativa de vida são responsáveis pela mudança na pirâmide etária brasileira. Juntamente com a transição demográfica, observa-se uma transição epidemiológica, em que as doenças e agravos não transmissíveis (DANT) predominam sobre as doenças infecciosas e parasitárias, embora essas últimas ainda participem da carga de doenças no país (1) . Entre as DANT, destacam-se as demências, das quais a Doença de Alzheimer (DA) é a mais prevalente, correspondendo a 60% dos casos (1) . A DA é entendida como uma doença neurológica progressiva e irreversível, decorrente de degeneração do tecido nervoso, caracterizada por alterações da função cognitiva, do comportamento e do afeto, podendo impactar de maneira expressiva nas atividades da vida diária (2) . Na população idosa, é a principal causa de dependência funcional, institucionalização e morte (3) . Nos últimos anos tem aumentado de forma progressiva o número de famílias que cuidam de pessoas idosas com algum tipo de dependência, principalmente quando esta é provocada pela DA (4) . A maior parte dos cuidados a esse idoso é realizada no domicílio e, apesar de comprometer todos os integrantes da família, há um familiar que, normalmente, assume a responsabilidade pelo cuidado, o qual é denominado cuidador principal (5) .
