Tumor do seio endodérmico (tumor do saco vitelino) é um subtipo histológico dos tumores de células germinativas não-germinomatosos. No sistema nervoso central (SNC), geralmente, localizam-se na linha média (região pineal). As apresentações clínica e radiológica desses tipos de tumores necessitam de uma avaliação singular de marcadores tumorais. Relatamos um paciente de 12 anos, sexo masculino com sinais e sintomas de hipertensão intracraniana, diminuição da acuidade visual que evoluiu com hemiparesia esquerda e distúrbios do equilíbrio e coordenação. A ressonância magnética de crânio mostrou massa expansiva em linha média, bem delimitada, de contornos lobulados, localizada na topografia do tecto do mesencéfalo com extensão para o interior do III ventrículo. Foi realizada derivação ventrículo-peritoneal e ressecção parcial da lesão. A análise imunohistoquímica revelou presença de alfafetoproteína (AFP) e corpúsculos de Schiller-Duval sendo, assim, coerente com tumor de seio endodérmico. O paciente foi submetido à quimioterapia e radioterapia. No seguimento de cinco meses após o tratamento, seguiu com cefaleia de leve intensidade e monoparesia em membro superior direito. Também foi realizada revisão sobre o tema: epidemiologia, exames de imagem, diagnóstico laboratorial, histológico e imuno-histoquímico, diferentes modalidades de tratamento e abordagens cirúrgicas.
RESUMOO Linfoma Anaplásico de Grandes Células (LAGC) é um subtipo raro de Linfoma não-Hodgkin (LNH). Citogeneticamente, tem forte expressão do marcador de ativação CD30 e, em cerca de 65-80% dos LAGCs, há expressão da quinase do linfoma anaplásico (ALK). Classificam-se em LAGC ALK-positivos quando expressam a quinase, relacionando-se a uma melhor resposta à terapia e uma melhor sobrevida global, ou em LAGC ALK-negativos em caso de não expressão. A incidência de LAGC no SNC é muito rara, com menos de 1% dos casos correspondentes a primários do SNC. Apresenta-se, no seguinte relato, um caso bastante raro de LAGC primário do SNC, ALK-negativo, em paciente jovem do sexo feminino, características epidemiológicas opostas às descritas na literatura para esse tipo de apresentação. O diagnóstico diferencial de primeira hipótese era de neoplasia glial primária do SNC de alto grau, entretanto, a imuno-histoquímica definiu como LAGC ALK-negativo, conforme o padrão de expressão citogenético (CD30 e CD3 positivos, alto índice de proliferação celular e negatividade para ALK-1). O tratamento de escolha foi quimioterapia em quatro ciclos com metotrexato e citarabina, em associação com radioterapia.Palavras-chave: Linfoma Anaplásico de Células Grandes; ALK-1; Linfoma primário do sistema nervoso central. ABSTRACTThe Anaplastic Large Cell Lymphoma (ALCL) is a rare subtype of non-Hodgkin's lymphoma (NHL). Citogenetically, it has a strong expression of the activation marker CD30 and around 65-80% of ALCL shows the expression of anaplastic lymphoma kinase (ALK). They are classified in ALK-positive ALCL when express the kinase, relating to a better therapy response and greater overall sur-vival, or in ALK-negative ALCL in case of non-expression. The incidence of CNS ALCL is highly rare, with less of 1% of cases corresponding to primary CNS anaplastic large cell lymphoma. In the following report, we present a rather unusual case of a primary CNS ALCL, ALK-negative, in a young fe-male patient, which has epidemiological characteristics opposite to the ones described in the literature for this kind of presentation. The first differential diagnostic hypothesis was a high-grade primary CNS glial neoplasia. However, the immunohistochemical defined it as ALK-negative ALCL, according to the cytogenetic expression pattern (positives CD30 and CD3, high cell proliferation rate and negative for ALK-1). The therapy of choice was 4 cycles of chemotherapy with methotrexate and cytarabine, associated with radiotherapy.
