BackgroundHepatitis C virus (HCV) compromises Health-related Quality of Life (HRQL) with detriments to Physical, Mental and Social health domains. Treatment with interferon and ribavirin is associated with side effects which further impair HRQL. New treatments appear potent, effective and tolerable. However, Patient Reported Outcomes instruments that capture the impact on HRQL for people with hepatitis C are largely non-specific and will be needed in the new treatment era. Therefore, we developed a conceptually valid multidimensional model of HCV-specific quality of life and pilot survey instrument, the Patient Reported Outcome Quality of Life survey for HCV (PROQOL-HCV).MethodsHCV patients from France (n = 30), Brazil (n = 20) and Australia (n = 20) were interviewed to investigate HCV-HRQL issues raised in the scientific literature and by treatment specialists. Interviews were recorded, transcribed and translated into English and French.ResultsFifteen content dimensions were derived from the qualitative analysis, refined and fitted to four domains: (1) Physical Health included: fatigue, pain, sleep, sexual impairment and physical activity; (2) Mental Health: psychological distress, psychosocial impact, and cognition; (3) Social Health: support, stigma, social activity, substance use; (4) Treatment: management, side effects, and fear of treatment failure. The impact of some dimensions extended beyond their primary domain including: physical activity, cognition, sleep, sexual impairment, and the three treatment dimensions. A bank of 300 items was constructed to reflect patient reports and, following expert review, reduced to a 72-item pilot questionnaire.ConclusionWe present a conceptually valid multidimensional model of HCV-specific quality of life and the pilot survey instrument, PROQOL-HCV. The model is widely inclusive of the experience of hepatitis C and the first to include the treatment dimension.
Sexual motivation is a fundamental behavior in human. For a long time, this behavior has been somehow ignored from psychological and neuroscientific research. In this article – reflecting the collaboration of a clinical psychologist and a neuroscientist – we show that in the current period, sexual affiliation is one of the most promising affiliation context to articulate a debate, a dialog and convergence points between psychoanalysis and neuroscience. Recent data on healthy sexual behavior and its compulsive variant are discussed under the prism of neuroscience and psychoanalysis.
La misère de l'excès Ces dernières années, j'ai eu l'occasion de suivre en psychothérapie des jeunes gens dont les pratiques sexuelles étaient comparables dans leur fonctionnement à une prise de drogue. J'ai tenté de formaliser ce mode particulier de choix d'objet (partiel) sous différents paradigmes psychopathologiques : celui des addictions, des compulsions 1 , des phénomènes autistiques et des névroses actuelles. Ces divers éclairages ont mis en relief un dénominateur commun : celui d'un intense sentiment de solitude, de misère intérieure, d'une rare volonté d'éradiquer le sexuel 2 , accompagné d'une grande précarité. Aborder le fragile domaine de la sexualité et de ses diverses pratiques dans le registre des précarités n'est pas anodin : mal assurée, éphémère, sujette à la révocation, dépendante de l'autre, la sexualité est par essence précaire. En quoi les sexualités addictives seraient-elles plus précaires que d'autres ? Pourquoi le modèle psychopathologique transnosographique des toxicomanies est ici pertinent pour éclairer l'objet de cette étude ? L'urgence de la demande, la dépendance au shoot, la pauvreté de la créativité sexuelle, le sentiment de ne pas exister en dehors du terrain addictif, l'augmentation des doses pour parvenir à la tranquillisation, tels sont les éléments quotidiens des toxicomanes de la sexualité, ceux que j'appellerai les égarés de l'amour. Pourtant, l'on pourrait trouver dans cette quête folle et souvent auto-destructrice un appel vers le lien d'alliance, une prière pour accéder à l'amour d'objet (precarius signifie d'ailleurs « obtenu avec prière et insistance »). Étrangement, cette mendicité sexuelle, étroitement liée au fantasme de prostitution contiendrait dans sa réalisation même l'espoir fou d'une rencontre avec l'objet idéalisé. Si, depuis la Genèse, le sexuel est intriqué à la conscience de la mort, les pratiques ici décrites recouvrent des dimensions de défi, de transgression et de déliaison. Dans cette inépuisable quête de partenaires sans visage, ces sujets se trouvent dans une attente de personne dont Pierre Fédida écrivait dans un article consacré aux toxicomanies 3 : « La transparence des visages n'est autre que la manifestation de l'attente feignant le dialogue pour banaliser l'hostile ». Et lorsque les visages deviennent transparents, il ne reste plus que la turgescence des organes, qu'un corps en morceaux dépouillé de la beauté de l'être.
