With the aim of analysing the perception of cancer patients towards the end of life, a questionnaire was applied to 100 oncologic patients, of which 85% were unaware of living will/advance directives, 81% were unaware of the terms palliative care and 70% were unaware of order to not reanimate. After clarifying the meaning of the terms used, 62% agreed to make living will/advance directives, 82% to receive palliative care at home and 64% to be reanimated in any situation, but the majority (73%) disagreed with the practice of futility. Only 11% reported inadequacies in the communication of their diagnosis by the physician. The fact of having metastases elicited a broader reflection on the termination of life. The results allow to infer that it is necessary to encourage the discussion about termination of life to the population during patient care, as well to promote the respective training to health professionals since the undergraduate course. Resumo Percepção de pacientes oncológicos sobre terminalidade de vidaObjetivou-se analisar, por meio de pesquisa descritiva, a percepção de pacientes oncológicos diante da terminalidade da vida. Aplicou-se questionário para 100 pacientes oncológicos, dos quais 85% desconheciam testamento vital/diretivas antecipadas de vontade, 81% desconheciam cuidados paliativos e 70% desconheciam ordem de não reanimar. Após esclarecimento sobre os termos empregados, 62% concordariam em elaborar testamento vital/diretivas antecipadas de vontade, 82% concordariam em receber cuidados paliativos no domicílio e 64% em ser reanimados em qualquer situação, mas a maioria (73%) discordou da prática de distanásia. Apenas 11% referiram inadequação na forma de comunicação de seu diagnóstico pelo médico. A presença de metástases suscitou reflexão mais ampla sobre terminalidade da vida. Os resultados permitem inferir que é necessário incentivar a discussão sobre terminalidade da vida na população durante a assistência aos pacientes, bem como estimular o debate sobre o assunto durante a formação de profissionais de saúde. -Rational:The person who undergoes a definitive intestinal ostomy experience significant changes in social and family dynamics. Most of the time the patients are fragile by risk due to the new situation. Aim: To analyze the social dynamics of people with permanent intestinal stoma. Method: Qualitative research on the basis of information collected at the time of the monthly withdrawal of collecting handbags and accessories provided by the unified Health System. Were part of the study 12 users of São Miguel do Oeste, SC, Brazil who agreed to participate in the research. Results: Eight women and four men were included. Eleven were elderly and all were retired. Users lived with few people. The largest family interviewed had four people, the other only with the spouse or lived alone. All users indicated that they had someone to help them after the ostomy. Four users have reported that they needed aid routinely and eight were blaming himself alone by the exchange of necessary hygiene and care. Seven said that they had some kind of difficulty or negative feeling because of using the collection bag. Fear and embarrassment were the most mentioned feelings. On the other hand, five users have stated that did not have difficulties or negative feelings due to the ostomy, and that these feelings had been overcome. Conclusions: Coexistence with the ostomy is highlighted as causing fears, constraints, discomfort and doubts. The family and social support can promote a new identity to the person, enabling the return of lost self-esteem and social reintegration. RESUMO -
Objective: To evaluate the influence of chronic morbidities on the functional capacity of elderly persons living in the municipal region of Herval d’ Oeste, in the state of Santa Catarina, Brazil. Methods: An analytical cross-sectional study was carried out with a sample of 272 elderly people registered in the Family Health Strategy of the municipality of Herval d’ Oeste, Santa Catarina. A socio-demographic questionnaire, Katz’s Basic Activities of Daily Living Scale and Lawton’s Instrumental Activities of Daily Living Scale (IALD) were used as instruments to gather data. The socio-demographical variables were estimated in frequencies and percentages. The associations were analyzed through the chi-square test. Results: Significant statistical associations for dependence were only found in the IADL assessments. Hypertension and Diabetes Mellitus were not found to influence the dependence of the elderly. However, a greater number of elderly persons who reported pathologies of the gastrointestinal system were classified as dependent. While the number of morbidities did not influence dependence in IADL, there was a smaller percentage difference between dependent and independent individuals among those with more than three morbidities. Conclusion: No relationship was found between the number of morbidities and the functional and instrumental capacity of the elderly. However, diseases of the gastrointestinal system demonstrated an influence on the dependence of elderly persons of Herval d'Oeste, Santa Catarina.
Consiste da implementação da pesquisa convergente-assistencial (PCA) que se caracteriza pela propriedade de se articular com a prática assistencial em saúde. Desta forma, as ações de assistência vão sendo incorporadas no processo de pesquisa e vice-versa Exemplificamos a implementação da PCA pela sua utilização numa pesquisa que objetivou introduzir inovações no cuidado de enfermagem a partir de um processo de discussão e reflexão com um grupo de 12 pessoas com diabetes mellitus. A PCA consistiu-se instrumento útil no desenvolvimento do projeto da mestranda tanto no que se refere à educação em saúde dos usuários quanto na construção do conhecimento referente ao tema da pesquisa. Os usuários foram beneficiados com uma melhor compreensão de suas problemáticas e com a descoberta de alternativas para lidar com elas. O estudo contribuiu também para o desenvolvimento de conhecimento teórico referente aos modos populares de cuidar de si e prevenir complicações relacionadas a diabetes.
O estudo visou a analisar a qualidade de vida (QV) entre idosos da área urbana e rural, avaliando a influência de dois fatores (zona de residência e existência ou não de patologia). Pesquisa de delineamento epidemiológico transversal, foi realizada com idosos (homens) residentes no perímetro urbano e rural do município de Concórdia, SC. A amostra consistiu de 60 idosos: grupo da área urbana (GU; n=30; idade= 68±8 anos) e rural (GR; n=30; idade: 67±7,88 anos). Os dados de QV foram coletados através do formulário Medical Outcomes Study Questionnaire Health Survey (SF 36). Os resultados demonstraram que apenas as variáveis saúde (∆= 12,60; p<0,0001) e aspectos sociais (∆= 28,53; p<0,0001) obtiveram diferença significativa (p<0,05) intergrupos, sendo os valores do GR superiores (mais favoráveis) aos do GU. O teste do qui-quadrado revelou diferença significativa (χ²: 4,34; p= 0,037) intergrupos, sendo o GR o grupo que possuiu menor quantitativo de indivíduos com patologia. Desta forma infere-se que, na amostra analisada, o GR obteve melhora na qualidade de vida, em relação à saúde e aos aspectos sociais, quando comparados ao GU.
A formulação e a gestão de políticas públicas coerentes aos movimentos populacionais, podem contribuir para o desenvolvimento e proporcionar qualidade de vida aos idosos, pois, com a perspectiva da longevidade, aumenta a ocorrência de pessoas em envelhecimento ativo. Objetivo geral do estudo foi observar fatores que contribuem para a carreira de idosos nas organizações. O procedimento metodológico utilizado foi qualitativo, com alguma recorrência a dados quantitativos, sobretudo demográficos. Com este estudo observou-se a necessidade de criação de novas políticas públicas de emprego voltadas aos idosos, fazendo com que as organizações possam inseri-los no mercado de trabalho com autonomia e segurança.Palavras-chave: Carreira; Idosos; Trabalho. ____________________________________ ABSTRACTThe formulation and management of public policies that are coherent with population movements can contribute to development and provide quality of life for the elderly, since with the perspective of longevity the number of active aging people increases. Overall objective of the study was to observe factors that contribute to the career of older people in organizations. The methodological procedure used was qualitative, with some recurrence to quantitative data, especially demographic data. With this study it was observed the need to create new public employment policies aimed at the elderly, allowing organizations to insert them into the labor market with autonomy and security.
Objective: To analyze the vulnerability of non-institutionalized elderly persons. Method: A cross-sectional, descriptive and analytical study was carried out using data of the City Health Department of Palmas, Paraná, Brazil, and the Vulnerable Elders Survey (VES-13) instrument. The questionnaires of people aged over 60 years who had answered the VES-13 questionnaire between January 2016 and December 2017 were included. The quantitative data were analyzed by the Student’s T-Test and the categorical data by the Chi-square and Fisher’s Exact Test. The correlation between the quantitative variables was performed by the Pearson correlation coefficient. Results: A total of 1,062 questionnaires were analyzed, of which 57.3% were female, with a mean age 69 (±7.8) years. In total 427 individuals (40.2%) were vulnerable and 635 (59.8%) were not vulnerable according to VES-13 score. A total of 635 (59.8%) elderly persons were classified as robust, 176 (16.6%) as at risk of frailty and 251 (23.6%) as frail. Women and those over 75 years were more vulnerable (p<0.001). Conclusion: Vulnerability was related to the female sex, age over 75 years and the presence of chronic diseases, with these being the priority groups for the elaboration of intervention strategies in the health of the elderly.
Estudo qualitativo que teve como objetivo investigar a convivência dos sujeitos da pesquisa com a diabetes mellitus bem como o cuidado dispensado pela família ao diabético. Participaram deste estudo 20 pessoas com diabetes mellitus tipo 2, usuários do Programa de Atenção Básica, do Município de Concórdia – SC. Os dados foram obtidos por meio de entrevista, e organizados em quatro categorias: 1 – Reviravolta na vida, caracterizada por ter que seguir dieta, tomar os remédios, fazer atividade física e também como fator de isolamento social; 2 – Desgosto por não aceitar a doença; 3 – Cuidado familiar enumerado como apoio, dar os remédios, ajuda com a dieta, controle do resultado da glicemia, acompanhamento e participação nas consultas; 4 – Auto cuidado. Conclui-se que a diabetes mellitus requerem novas atitudes e a família é um importante fator que influencia na tomada de decisões frente ao novo estilo de vida que esta condição crônica impõe.
