Latar belakang: Sectio caesarea (SC) adalah operasi darurat terbanyak pada obstetrik. Spinal anestesia direkomendasikan untuk section cesarean. Akan tetapi, ffek samping spinal anestesia tersering (hipotensi) dapat menimbulkan efek samping fetomaternal. Spinal anestesia dosis rendah diduga dapat mengurangi efek samping spinal anestesia. Tujuan penelitian ini adalah mengetahui perbandingan outcome spinal anestesia dosis rendah dibanding dosis biasa pada operasi section cesarean darurat. Metode: Penelitian ini merupakan studi retrospektif pada 119 pasien yang menjalani SC dengan spinal anestesia. Kelompok A mendapat bupivacaine heavy 5mg + adjuvant (dosis rendah), kelompok B mendapat bupivacaine heavy 7,5mg + adjuvant (dosis rendah), kelompok C mendapat bupivacaine heavy >8mg + adjuvant (dosis biasa). Outcome diteliti yaitu tekanan darah dan nadi ibu menit ke 0,3,6,9. waktu capai ketinggian blok T10-T6, Bromage score, dan Apgar score bayi. Data penelitian dianalisa statistik menggunakan uji normalisasi dan homogenitas, Korelasi Spearman, One-Way ANOVA, Kruskal wallis dan Mann Whitney dengan p≤0,05. Hasil: Tidak ada beda yang signifikan antara tekanan darah dan nadi pada menit ke 0,3,6, dan 9, waktu capai Bromege 2-3 (p ≥ 0,05), waktu capai T10-T6 (p≥ 0,05) dan Apgar score pada ketiga kelompok penelitian. Akan tetapi waktu kembali Bromage 0 kelompok spinal dosis rendah dan dosis biasa memiliki beda yang signifikan (p ≤0,05). Kesimpulan: Outcome tekanan darah, nadi, waktu capai blok T10-T6, bromege score 2-3, Apgar score tidak berbeda signifikan pada spinal anestesia dosis rendah dibanding dosis biasa. Waktu kembali bromege 0 berbeda signifikan pada spinal anestesia dosis rendah dibandingkan dosis biasa.
