ResumenIntroducción: Los niños y jóvenes migrantes internacionales enfrentan diferentes retos en salud en comparación con la población local, en particular si se enfrentan a ambientes inseguros o a condiciones sociales adversas. Este estudio busca identificar brechas existentes en resultados de salud de la niñez entre población migrante internacional y chilena. Métodos: Este estudio analiza tres fuentes de información: (i)Nacer en Chile: Datos de consulta antenatal recolectados de los registros electrónicos de las mujeres usuarias del programa Chile Crece Contigo, de todos los centros de salud familiar (CESFAM) de administración municipal de la comuna de Recoleta el año 2012; (ii)Crecer en Chile: Datos de encuesta poblacional "Caracterización Socioeconómica Nacional" CASEN 2013 y (iii)Enfermar en Chile: Datos de todos los egresos hospitalarios de 2012, proporcionada por el departamento de estadística e información en salud (DEIS) del Ministerio de Salud. Resultados: (i) Nacer en Chile: Hay mayor proporción de inmigrantes con riesgo biopsicosocial (62,3% vs 50,1% en chilenas) y con ingreso tardío al programa (63,1% vs 33,4%). Hay menos cesáreas en inmigrantes que en chilenas (24,2% vs 33,6%). (ii) Crecer en Chile: Existe una mayor proporción de niños migrantes fuera del sistema escolar y una mayor proporción en pobreza multidimensional (40% vs 23,2%). (iii) Enfermar en Chile: En migrantes entre 7-14 años es más frecuente egresar hospitalariamente por traumatismos/ otras causas externas (23,6% vs 16,7% en chilenos). Conclusiones: Este estudio entrega nueva evidencia sobre necesidades urgentes de salud de nuestros niños en Chile. Este es un imperativo ético, legal y moral, independiente de la condición migratoria. 708ARtÍCULO ORIGInAL IntroducciónEn 2015 se reportaron 244 millones de migrantes internacionales en el mundo, un 41% más que en el año 2000 1 . Se ha estimado que 37 millones de estos migrantes son menores de 20 años, de los cuales la mayoría reside en países en vías de desarrollo 2 . Recientemente se ha reconocido que la condición migratoria puede ser un indicador de vulnerabilidad social o de desigualdad social en salud al interior de un país 1-4,9,10 y que variables migratorias específicas, posibles de modificar o prevenir, pueden ser determinantes en este proceso 8 . Los principales factores que afectan a la salud de los migrantes internacionales son condiciones laborales, barreras culturales, acceso y uso a servicios de salud, condiciones previas en el país de origen, tiempo de residencia en el país de destino (asociados a fenómenos de asimilación y aculturación), procesos sociales de integración/exclusión y experiencias de estigma y discriminación 11,12,13 . Type 2 diabetes is a global epidemic, and many people feel stigmatized for having this disease. The stigma is a relevant barrier to diabetes management. However, evidence in this regard is scarce in Latin America. This study aimed to analyze the level of stigma surrounding type 2 diabetes in the Colombian population and its relationships with sociodemographic, clinical, psychosocial variables and behaviors related to management of the disease (self-management behaviors). This cross-sectional study included 501 Colombian adults with type 2 diabetes. We estimated the relation between stigma and selected variables through linear regression models. Additionally, we analyzed the mediator role of psychosocial variables in the relationship between stigma and self-management behaviors through structural equation models. A total of 16.4% of patients showed concerning levels of stigma. The time elapsed since diagnosis (β = −0.23) and socioeconomic status (β = −0.13) were significant predictors of the level of stigma. Stigma was negatively correlated with self-efficacy (β = −0.36), self-esteem (β = −0.37), and relationship with health care provider (β = −0.46), and positively correlated with stress (β = 0.23). Self-efficacy, self-esteem, and the relationships with health care providers had a mediation role in the relationship between stigma and self-management behaviors. These variables would be part of the mechanisms through which the perception of stigma harms self-management behaviors. The stigma of type 2 diabetes is frequent in the Colombian population and negatively associated with important aspects of disease management.
Objective To evaluate the psychometric properties of the Jefferson Medical Empathy Scale, Spanish version (JSE-S), its factorial structure, reliability, and the presence of invariance between genders in the behavior of empathy levels among Chilean nursing students. Method Instrumental research design. The JSE-S was applied to 1,320 nursing students. A confirmatory factor analysis was used. An invariance study between genders was carried out. Descriptive statistics were estimated. Between genders, Student’s T distribution was applied alongside a homoscedasticity analysis. The level of significance was α ≤ 0.05. Results The confirmatory factor analysis determined the existence of three dimensions in the matrix. The statistical results of the invariance tests were significant, and allowed comparison between genders. Differences were found between mean empathy values, as well as in some of its dimensions between genders. Conclusion The factor structure of empathy data and its dimensions is in correspondence with the underlying three-dimensional model. There are differences in empathy levels and their dimensions between genders, with the exception of the compassionate care dimension, which was distributed similarly. Women were more empathetic than men.
OBJECTIVE: To validate an instrument measuring the cultural competence in health care workers from Chile. METHODS: Using Sue & Sue’s theoretical model of cultural competence, we designed a scale, which was assessed by health care workers and experts. Subsequently, the scale was applied to a sample of 483 different health care workers, during 2018 in Santiago de Chile. The analysis included: exploratory and confirmatory factor analysis, estimation of reliability, and analysis of measurement bias. Finally, the level of cultural competence was calculated for every professional who participated in this study. RESULTS: The final scale include 14 items that are grouped into three dimensions concordant with the theoretical model: sensitivity to own prejudices, cultural knowledge, and skills to work in culturally diverse environments. This scale showed good fit in factor models, adequate reliability and lack of evidence of measurement bias. Regarding the performance of health care workers, sensitivity showed a lower level compared with the other dimensions evaluated. CONCLUSION: The scale for measuring the level of cultural competence in health care workers (EMCC-14) is a reliable instrument, with initial support for its validity, which can be used in the Chilean context. Additionally, the results of this study could guide some possible interventions in the health sector to strengthen the level of cultural competence.
BackgroundAutopsies can shed light on the pathogenesis of new and emerging diseases.AimTo describe needle core necropsy findings of the lung, heart, and liver in decedents with COVID-19.MaterialCross-sectional study of needle core necropsies in patients who died with virologically confirmed COVID-19. Histopathological analyses were performed, and clinical data and patient course evaluated.ResultsChest core necropsies were performed in 71 decedents with a median age of 81 years (range 52–97); 47 (65.3%) were men. The median interval from symptoms onset to death was 17.5 days (range 1–84). Samples of lung (n = 62, 87.3%), heart (n = 48, 67.6%) and liver (n = 39, 54.9%) were obtained. Fifty-one lung samples (82.3%) were abnormal: 19 (30.6%) showed proliferative diffuse alveolar damage (DAD), 12 (19.4%) presented exudative DAD, and 10 (16.1%) exhibited proliferative plus exudative DAD. Of the 46 lung samples tested for SARS-CoV-19 by RT-PCR, 39 (84.8%) were positive. DAD was associated with premortem values of lactate dehydrogenase of 400 U/L or higher [adjusted odds ratio (AOR) 21.73; 95% confidence interval (CI) 3.22–146] and treatment with tocilizumab (AOR 6.91; 95% CI 1.14–41.7). Proliferative DAD was associated with an onset-to-death interval of over 15 days (AOR 7.85, 95% CI 1.29–47.80). Twenty-three of the 48 (47.9%) heart samples were abnormal: all showed fiber hypertrophy, while 9 (18.8%) presented fibrosis. Of the liver samples, 29/39 (74.4%) were abnormal, due to steatosis (n = 12, 30.8%), cholestasis (n = 6, 15.4%) and lobular central necrosis (n = 5, 12.8%).ConclusionProliferative DAD was the main finding on lung core needle necropsy in people who died from COVID-19; this finding was related to a longer disease course. Changes in the liver and heart were common.
Competencia Cultural en Salud: una necesidad urgente en trabajadores de la salud Cultural Competence in health: an urgent need for health workers Sr. Editor: La competencia cultural es entendida en el ámbito de la salud como la habilidad que deben poseer los trabajadores de la salud (TS) para incorporar una visión del contexto social y cultural de cada usuario, incrementando así la posibilidad de lograr una real comunicación con este último. Habitualmente, la necesidad de desarrollar la competencia cultural es entendida en la interacción que establecen los TS con la población de origen étnico o incluso en la interacción con inmigrantes internacionales, grupos en los cuales resulta más evidente la posible diferencia en el background cultural entre TS y usuario. Sin embargo, algunos autores 1-3 plantean que cada interacción en el ámbito de la salud debiera considerar una adaptación cultural de las estrategias, de tal forma que las intervenciones contemplen la individualidad y el contexto social de los usuarios, apostando de esta forma al éxito en la implementación de dichas intervenciones.Considerando el perfil epidemiológico actual de nuestro país, en que predominan condiciones crónicas de salud, dos de los grandes desafíos que enfrentan los TS son la comunicación efectiva con los usuarios y la adherencia a las intervenciones sugeridas. Tomando como referencia el modelo de determinantes sociales de la salud 4 , es posible establecer que variables tales como género, edad, nivel educacional, situación laboral, nivel de ingresos, etnia o condición de migrante, sitúan a cada individuo en un entramado social complejo, el cual influye en sus decisiones de salud. Necesariamente estas variables debieran ser tomadas en cuenta por los TS para el diseño de estrategias que apunten al éxito y adherencia deseados.En el ámbito de la atención primaria en salud esta habilidad de comunicarse efectivamente con el usuario, considerando su background cultural, pareciera tener especial relevancia, ya que es en este nivel donde debiera ocurrir el primer encuentro entre los usuarios y el sistema de salud, y generalmente es allí donde las personas controlan sus condiciones crónicas de salud. Overview of machine learning and its application in the management of emergency servicesThe processes associated with health care generate a large amount of information that is difficult to analyze using standard statistical procedures. In this context, disciplines such as Data Science became relevant, mainly through strategies such as Machine Learning (ML). The latter groups a series of tools whose purpose is to develop algorithms to extract information from data, whether for explanation, classification, or prediction. Despite its usefulness as support for clinical decisions, its potential in health care management has been less explored. Also, there are difficulties in understanding these types of studies. This work tries to offer a nontechnical overview of the ML concept and its advantages for health care management. It collects examples of ML applications in emergency department management.
