En la actualidad, los servicios de salud deben ofrecer atención y cuidados integrales que brinden los recursos necesarios para dar mejoría a sus pacientes, desde el diagnóstico y tratamiento, hasta su recuperación completa, la infección del sitio quirúrgico (ISQ) y las complicaciones que de ella se derivan han constituido un hecho inseparable a la práctica quirúrgica desde sus rudimentarios comienzos hasta la actualidad. La presente investigación contiene información de revisión bibliográfica de tipo documental, la técnica para la recolección de datos está constituida por materiales impresos y electrónicos estos últimos como Google Académico, Scielo, PubMed, entre otros. Las infecciones de sitio quirúrgicos no son infrecuentes dentro del ámbito hospitalario, ya que estas se pueden presentar por múltiples factores, como propios de la cirugía, así como propios del paciente. Sin embargo, existen protocolos a nivel internacional diseñados por la Organización Mundial de la Salud (OMS) para la prevención de las infecciones, pero como ya se menciono es difícil mantener el control tanto de los factores intrínsecos como exógenos, sobre estos últimos, se pueden mencionar los materiales protésicos, que son utilizados en un 10%. El conocimiento de todos los elementos que pueden ayudar a combatir y/o prevenir estas infecciones van a conducir a mejorar los procesos quirúrgicos y su calidad.
El Síndrome de Mirizzi es una de las complicaciones poco frecuente, se produce a causa de uno o varios cálculos impactados en el cuello de la vesícula o el conducto cístico, esto produce compresión extrínseca y obstrucción del conducto hepático común, generando una reacción inflamatoria, también condiciona la estenosis del conducto hepático común. Puede producir necrosis del tejido adyacente, causando una fístula colecitocoledociana. La epidemiología clásica de los cálculos biliares en general, permite tener un conocimiento global sobre los orígenes del síndrome de Mirizzi. Existen factores de riesgo como: la edad, (entre la cuarta y séptima década de vida), obesidad, sexo femenino, anticonceptivos orales, pérdidas abruptas de peso, ayunos prolongados, entre otros que aumentan estasis biliar, y fomentan la formación cálculos. El diagnóstico del síndrome de Mirizzi no es fácil de realizar, ni múltiples modalidades de estudios radiológicos disponibles para este fin, es un hallazgo incidental en el trans-operatorio de cirugías de vesícula biliar en la cual dicho síndrome no es el diagnóstico preoperatorio principal. Entre los métodos de estudio de imagen utilizables para diagnosticar un síndrome de Mirizzi se encuentran, pero no limitados a, el ultrasonido abdominal, la tomografía axial computarizada, la colangioresonancia, la colangiopancreatografía retrógrada endoscópica. Por otro lado, Csendes y otros reclasificaron el síndrome de Mirizzi en cuatro tipos, categorizando la fístula colecistocoledocal de acuerdo a su grado de destrucción. El tratamiento del Sindrome Misizzi más común utilizado es la cirugía, se basa esencialmente en la resección económica o total de la vesícula biliar y extracción del cálculo impactado, utilizándose diferentes técnicas para reparar el defecto en los casos con fístula. La técnica quirúrgica, ya sea por vía convencional o laparoscópica, varía dependiendo del tipo de lesión, siendo más compleja a medida que aumenta el grado de la lesión. La cirugía laparoscópica no es la más aconsejada, considerándola algunos autores contraindicada sobre todo en los casos con fístula. El objetivo del tratamiento incluye la descompresión de la vía biliar y prevenir la recurrencia de la afección. Descompresión temporal mediante la CPRE, usando stents biliares busca ganar tiempo mientras se mejoran las condiciones generales de algunos pacientes, facilitando la planificación de las estrategias quirúrgicas a desarrollar. Se aplicó una metodología descriptiva, con un enfoque documental, es decir, revisar fuentes disponibles en la red, con contenido oportuno y relevante para dar respuesta a lo tratado en el presente artículo.
