Introduction : La technique du volet osseux réalisé à la micro-scie et repositionné in situ peut être une technique alternative à la décortication dans le cadre de dents incluses, de chirurgie endodontique, d'exérèse de corps étranger, de lésions kystiques ou de dépose d'implants ostéo-intégrés non fonctionnels. La micro-scie permet un geste rapide qui pourrait remplacer la décortication à la fraise boule ou l'utilisation du piézotome. Technique : Un bloc cortical a été délimité par quatre traits d'ostéotomie à la micro-scie et clivé en regard de la zone à opérer. En fin d'intervention, le volet cortical a été réinséré et impacté en position initiale. En cas de manque de stabilité, il a pu être maintenu, soit par des vis et/ou des plaques d'ostéosynthèse, soit par des ligatures résorbables. Discussion : L'émergence de cette technique est en partie liée au progrès de l'instrumentation chirurgicale. Le repositionnement in situ du volet en fin d'intervention, garant de la conservation osseuse, s'oppose à l'incarcération épithéliale et favorise la régénération osseuse. Toutefois, la fixation du volet osseux en fin d'intervention est une étape délicate qui conditionne la réussite de l'intervention. L'utilisation de la micro-scie permet d'obtenir une bonne visibilité opératoire, moins de perte osseuse lors des ostéotomies et un gain de temps par rapport à la décortication et la piézochirurgie.
Introduction: Ewing's sarcoma is a primary malignant bone tumor that affects children and young adults. In rare cases, it affects the bones of the face. The diagnosis is difficult because signs are non-specific. Positive diagnosis is based on a cytogenetic examination showing a specific translocation of the EWS gene. Treatment involves a sequence including chemotherapy, oncological surgery and radiotherapy. Observation: The case presented is that of a 4-year old girl initially sent for suspicion of dental cellulitis. On clinical examination, she had recent mandibular dental mobility and a gingival mucosa swelling. Signs of bone lysis interesting the mandibular symphysis was observed on the orthopantomogram. She had no pain or symptoms indicating neoplasia. This atypical situation led to a bone biopsy who revealed the diagnostic of mandibular Ewing's sarcoma. A suitable medical care in a specialized unit was able to be started without delay. Discussion: The literature reports other cases also wrongly diagnosed as an infectious pathology of dental origin. Conclusion: This case shows that the diagnosis of mandibular Ewing's sarcoma must be evoked in front of any atypical mandibular lesion of the child, regardless of his age or sex. Physicians and dental surgeons specialized in oral surgery are then in the foreground to help establish early diagnosis and ensure rapid management by limiting the functional and aesthetic sequelae related to localization.
CoMMUNICATIoNLa compression du nerf alvéolaire inférieur (NAI) par du matériel d'obturation endodontique est une situation rare qui entraîne une hypoesthésie voire une anesthésie dans le territoire du NAI. Elle doit être prise en charge rapidement pour minimiser le risque de troubles sensitifs irréversibles.Une patiente de 25 ans, aux antécédents de déficit en protéine C et S et d'embolie pulmonaire suite à un épisode de thrombophlébite du post-partum, sous anticoagulant, a été prise en charge par son chirurgien-dentiste pour le traitement endodontique de la dent 38. Le cliché rétro-alvéolaire de contrôle a montré une fuite de la pâte d'obturation le long du canal mandibulaire gauche. La dent 38 a alors été immédiatement avulsée par le praticien. Devant l'existence d'une anesthésie non-régressive à la levée du bloc nerveux dans le territoire du NAI gauche, la patiente a été adressée dans le service de chirurgie maxillo-faciale. À l'examen initial, 3 jours après le traitement endodontique, la patiente présentait une anesthésie complète dans le territoire du NAI gauche. Après équilibration du traitement anticoagulant, une chirurgie de décompression nerveuse a été réalisée au huitième jour consistant, sous anesthésie locale, en un nettoyage du canal mandibulaire gauche après dépose de la corticale latérale au piézotome. Le matériel de curetage du canal mandibulaire a été envoyé pour examen anatomopathologique et a montré des remaniements fibro-inflammatoires et hémorragiques. L'orthopantomogramme de contrôle postopératoire a montré l'ablation complète du matériel d'obturation. Les suivis successifs montrent une récupération partielle de la sensibilité. Au 6ème mois postopératoire, la patiente présente une hypoesthésie accompagnée de paresthésie et de phénomènes d'hyperesthésie dans le territoire du NAI gauche. La patiente est toujours en cours de suivi.La compression du nerf alvéolaire inférieur est une urgence chirurgicale qui nécessite un diagnostic rapide et une bonne coordination entre le chirurgien-dentiste et le chirurgien oral ou le chirurgien maxillo-facial pour espérer une récupération sensitive la plus complète possible. Elle entraîne une atteinte sensitive dans le territoire du NAI correspondant dont la prise en charge est encore mal codifi ée. Dans ce contexte, nous présentons une décompression chirurgicale du NAI par volet cortical latéral. Observation : Une patiente de 25 ans a consulté pour la prise en charge d'une anesthésie labio-mentonnière apparue 3 jours auparavant lors du traitement endodontique de la 38 et non résolutive suite à l'avulsion de cette dent. Le bilan radiologique montrait une fuite massive de pâte d'obturation dans le canal mandibulaire. Une décom-pression chirurgicale du NAI a été effectuée au piézotome au 7e jour sous anesthésie locorégionale. Discussion : La décompression chirurgicale du NAI doit être effectuée par un chirurgien expérimenté. Elle doit être réalisée rapidement (neurotoxicité des matériaux, infl ammation, ischémie nerveuse). La technique doit être la moins traumatique possible pour permettre d'exposer le NAI sans le léser et préserver le volume osseux.Abstract -Inferior alveolar nerve decompression by mandibular sagittal osteotomy. Case study. Introduction: The compression of the inferior alveolar nerve (IAN) is among the potential complications that may occur during endodontic treatment of the mandibular third molars due to a leak of the endodontic paste. It results in sensory loss in the corresponding territory of the IAN. Its treatment is not well defi ned. In this context, we present an IAN surgical decompression by lateral cortical window. Observation: A twentyfi ve-year-old patient was referred to us by her dentist on day three, to treat a numbness of the IAN that arose abruptly during endodontic treatment of the mandibular left third molar (38). The numbness remained, even following removal of the tooth. X-rays showed a signifi cant leak of the endodontic paste into the mandibular canal. Surgical decompression of the IAN was performed with a piezotome on day seven under local anesthesia. Discussion: Surgical decompression of the IAN should be performed by an experienced surgeon. It must be performed quickly (neurotoxicity materials, infl ammation, nerve ischemia), using a technique as atraumatic as possible to bring out the IAN with a minimum of additional damage and to preserve bone volume.
-Pyostomatitis vegetans: case report and literature review. Introduction: Pyostomatitis vegetans is a rare dermatosis which is manifested by yellowish-white pustules and inflammatory vesiculous lesions of the oral mucosa in the gum, the vestibule, the soft and hard palate, and sometimes the lips. PV is often associated with inflammatory bowel chronic diseases (IBDs) such as ulcerative colitis and Crohn's disease. Observation: An 80-year-old patient with Crohn's disease consulted for linear ulceration localized on the lower labial vestibule of the left cheek and on the right upper vestibule. The PV diagnosis was made after biopsy and the lesions regressed under local corticosteroid application. Discussion: The characteristic symptoms and histology (negative direct and indirect immunofluorescence) enable the diagnosis of PV and eliminate pemphigus vegetans and pyoderma gangrenosum. Peripheral eosinophilia and eosinophils on a histological section may be useful for monitoring disease activity. Treatment is based on local or systemic corticosteroids depending on the intensity of the oral and gastrointestinal involvement. Conclusion: Upon discovery of PV lesions, testing for IBDs is critical in order to establish a treatment as early as possible.
