INTRODUÇÂO Os medicamentos são importantes insumos para a assistência à saúde. Sendo assim a farmacovigilância surge como necessidade para um mundo que usa amplamente a intervenção medicamentosa. OBJETIVO Compreender os resultados obtidos com a implementação da busca de reações adversas através da metodologia de medicamentos gatilho (Trigger tool) e consolidar os tipos de reações adversas, os medicamentos mais envolvidos nelas e possíveis pontos de melhoria na implementação da busca. METODOLOGIA Trata-se de um estudo transversal e exploratório com coleta de dados retrospectiva. O período de estudo fora de março de 2017 até fevereiro de 2018, foram incluídos todos os pacientes atendidos que utilizavam algum dos medicamentos da lista de medicamentos gatilho. RESULTADOS Foram analisados 6068 eventos de detecção em 2726 prontuários, do qual, em 22 deles ocorreram RAMs. A população do estudo foi caracterizada como adultos de meia idade, com média de 47 anos (DP ± 21 anos). Sendo a população igualmente dividida quanto ao gênero, com 11 indivíduos para cada sexo. A maioria das reações foram desencadeadas por antimicrobianos ou analgésicos, e a via endovenosa fora predominante para a administração desses medicamentos, essas reações foram caracterizadas em sua maioria como do tipo B – Bizarras, com causalidade provável e gravidade leve. CONCLUSÃO As seguintes melhorias foram sugeridas: promoção do uso racional de antimicrobianos e analgésicos, utilização de algoritmos de causalidade que envolvam o uso de variáveis mais abrangentes e alinhadas com as reações adversas ao qual se investiga.
Objetivo: Realizar uma revisão sistemática dos estudos com os análogos sintéticos da Aniba riparia, as riparinas A, B, C, D, E e F, com o intuito de evidenciar os potenciais farmacológicos já elucidados. Metodologia: Utilizou-se três bases de dados para seleção dos artigos a serem incluídos na revisão Embase, Pubmed e Web of Science, com a seguinte estratégia de busca simplificada (Riparin or Aniba riparia), ao final da seleção, resultou em 10 artigos para a elaboração deste trabalho. Resultados: As publicações dos estudos com derivados sintéticos foram identificadas entre os anos de 2014 a 2021. Sendo que a maioria das pesquisas utilizavam a riparina A, seguida pela B, e todos tratavam-se de estudos pré-clínicos. Foram relatados importantes efeitos farmacológicos, como as atividades antibacteriana, antifúngica, anti-inflamatória, antinociceptiva, antitumoral, imunomoduladora, antiprotozoário, antiparasitária, ansiolítica e antioxidante. Discussão: Os estudos realizados com os análogos sintéticos têm colocado as mesmas como possíveis ferramentas terapêuticas, considerando as importantes atividades farmacológicas já relatadas. Conclusão: Contudo, considerando que a literatura já relata seis riparinas sintéticas, o número de artigos publicados voltados para elucidação de possíveis atividades farmacológicas parece reduzido, mesmo sendo promissoras candidatas para o desenvolvimento de pesquisas direcionadas para descobertas de novos fármacos.
Esta revisão integrativa teve por objetivo determinar quais são as principais bactérias que possuem correlações positivas entre o aumento do consumo de antimicrobianos e o aumento da resistência bacteriana em âmbito hospitalar. Resumiram-se os artigos, através de revisão integrativa, encontrados nas bases Pubmed® e Embase®, utilizando as seguintes estratégias de busca: (("Microbial Sensitivity Tests"[Mesh]) AND "Hospitals"[Mesh]) AND "Drug Utilization"[Mesh] e 'drug utilization'exp AND 'hospital'exp AND 'antibiotic sensitivity'exp, respectivamente. Sintetizando o autor, ano de publicação, local de estudo, população estudada, metodologia de avaliação do consumo e da relação estatística e as correlações positivas encontradas entre as espécies ou gêneros bacterianos associados com os dados de consumo de uma determinada classe de antimicrobianos. As estratégias de busca encontraram no total 414 artigos, sendo que após a aplicação dos critérios de inclusão restaram 10 artigos que atendiam a todos os critérios utilizados. As principais bactérias ou gêneros bacterianos envolvidos com correlações positivas foram P. aeruginosa (42,6%), E. coli (24,1%) e Acinetobacter spp. (9,3%). A maioria das correlações positivas encontradas foram relacionadas a mesma classe, todavia cerca de 48,2% foram cruzadas. Destacaram-se a P. aeruginosa, E. coli e o gênero Acinetobacter spp., como as bactérias com maiores quantidade de correlações positivas. Bancos de dados internacionais como o GLASS, podem fortalecer a evidência estatística dessa relação, permitindo amostras maiores e mais diversas, contudo eles dependem de cooperação internacional. Esse trabalho pode amparar a farmacovigilância em hospitais destacando correlações já elucidadas, ao qual podem contribuir na otimização dos recursos.
Este trabalho tem por objetivo avaliar a resistência a carbapenêmicos da Acinetobacter spp. reportada ao GLASS (Global Antimicrobial Resistance and Use Surveillance System), estratificando esses dados por faixa de renda per capita. Foi conduzido um estudo observacional retrospectivo sobre a sensibilidade a carbapenêmicos da Acinetobacter spp., reportada ao GLASS, fora incluído todos os dados disponíveis no momento da coleta, fevereiro de 2021, estratificando esses achados pelo nível de renda de cada país e a quantidade de testes de sensibilidade a carbapenêmicos realizados. Os países de maior renda tiveram a menor não-suscetibilidade relatada, contudo dentro deles existem exemplos que apresentam uma alta incidência de não-suscetibilidade. Para todos os quatro grupos de renda existiu uma alta variância do desvio padrão para a média dos testes de sensibilidade a carbapenêmicos, assim revelando pontos de inconsistência. Os dados trabalhados neste artigo mostram um nível preocupante de resistência a carbapenêmicos observados no GLASS. Dentro mesmo dos países ricos há aqueles que possuem níveis alarmantes de não-susceptibilidade e, para os países mais pobres, destaca-se uma escassez de dados mais completos, pois naqueles com os níveis mais preocupantes de não-susceptibilidade já são encontradas cepas de Acinetobacter spp. com fenótipo de multirresistência.
