Objective: To carry out a review of the literature on adolescents’ participation in decision making for their own health.Data sources: Review in the Scientific Electronic Library Online (SciELO), Latin American and Caribbean Health Sciences Literature (LILACS) and PubMed databases. We consider scientific articles and books between 1966 and 2017. Keywords: adolescence, autonomy, bioethics and adolescence, autonomy, ethics, in variants in the English, Portuguese and Spanish languages. Inclusion criteria: scientific articles, books and theses on clinical decision making by the adolescent patient. Exclusion criteria: case reports and articles that did not address the issue. Among 1,590 abstracts, 78 were read in full and 32 were used in this manuscript.Data synthesis: The age at which the individual is able to make decisions is a matter of debate in the literature. The development of a cognitive and psychosocial system is a time-consuming process and the integration of psychological, neuropsychological and neurobiological research in adolescence is fundamental. The ability to mature reflection is not determined by chronological age; in theory, a mature child is able to consent or refuse treatment. Decision-making requires careful and reflective analysis of the main associated factors, and the approach of this problem must occur through the recognition of the maturity and autonomy that exists in the adolescents. To do so, it is necessary to “deliberate” with them.Conclusions: International guidelines recommend that adolescents participate in discussions about their illness, treatment and decision-making. However, there is no universally accepted consensus on how to assess the decision-making ability of these patients. Despite this, when possible, the adolescent should be included in a serious, honest, respectful and sincere process of deliberation.
As questões referentes à terminalidade da vida levantam muitas discussões no contexto da bioética. Os avanços tecnológicos na medicina têm permitido salvar a vida de doentes críticos, entretanto, naqueles sem possibilidade de recuperação, a utilização de medidas curativas, além de onerosas e sem benefícios, acabam causando dor e sofrimento. A nutrição e a hidratação artificiais são consideradas tratamento médico e o profissional da saúde deve tomar decisões de acordo com sua utilidade. Refletir sobre questões de alimentação relacionadas à finitude sempre provoca polêmicas e opiniões divergentes entre familiares e até mesmo entre os profissionais de saúde. Esse trabalho tem por objetivo, à luz da bioética, propor uma reflexão sobre a importância do cuidar quando a cura é impossível e reconhecer a terminalidade como processo humano, natural e necessário. Nutrir o paciente no fim de vida tem um significado mais simbólico do que fisiológico e no caso da demência avançada pode causar desconforto e piora clínica. A cultura e a religião do paciente também devem ser priorizadas nesta fase. O ideal é que a vontade do paciente terminal seja respeitada por meio das diretivas antecipadas de vontade, o que pode diminuir a sobrecarga emocional de familiares ou responsável legal no que tange a tomada de decisão. No fim de vida de pacientes com demência avançada, a prioridade deve ser proporcionar uma morte digna, respeitando os princípios de beneficência, não maleficências e justiça
O avanço tecnológico e científico propiciou nas últimas décadas, ou, mais precisamente, a partir da Segunda Guerra Mundial, uma revolução nas relações entre o homem e o mundo do trabalho. Da Revolução Industrial até a presente data, o estudo do ambiente de trabalho e seus efeitos sobre a saúde dos trabalhadores sofreu mudanças significativas, quer na concepção de doença ocupacional/do trabalho e as novas formas de adoecimento, quer nas relações entre as diversas hierarquias de uma empresa. Do combate aos riscos químicos e biológicos, tão importantes quanto a sua exposição e possível intoxicação/contaminação, para os riscos psicossociais, pode-se dizer que a medicina do trabalho e seus irmãos -a saúde ocupacional e a saúde dos trabalhadores -mudaram sobremaneira não só na sua complexa percepção conceitual, mas por extensão na sua abordagem e resolutividade.No Brasil, de dimensão continental, como também em vários outros países como da América Latina, a heterogeneidade das necessidades de cuidados com a saúde dos trabalhadores é imensa. Se, por um lado, há ainda empresas de pequeno, médio ou até grande porte precisando de implantação de programas de promoção e prevenção à saúde, existem outras em que o grau de complexidade esbarra nas questões do público e do privado. Algumas precisam atuar fortemente na prevenção de acidentes e, por outro lado, outras são fortemente marcadas por gestões difíceis que se refletem em transtornos psicossociais.O médico do trabalho encontra no seu dia a dia um caleidoscópio de possibilidades de atuação e desafios com a diversidade presente, num só lugar (local de trabalho presencial ou virtual), de pessoas/trabalhadores com culturas múltiplas, pensares diferentes, funções distintas, riscos diversos e comportamento moral, em muitos casos esfacelados como reflexo da sociedade.Os absenteísmos/afastamentos por transtornos mentais, a conceituação de capacidade no trabalho e qualidade de vida, a prevenção por introdução de vacinação nas empresas e estudos sobre satisfação com o trabalho, além das diretrizes da Associação Nacional de Medicina do Trabalho (ANAMT) sobre Asma e Trabalho: diagnóstico por medida, seriada do Peak Flow, são temas que estão no cotidiano dos profissionais dessa importante área, assim como deste volume da Revista Brasileira de Medicina do Trabalho (RBMT).Em 2017, no Congresso Internacional sobre Organização do Trabalho e Fatores Psicossociais, na Cidade do México, México, as prioridades na saúde mental e física do trabalhador foram elencadas. Dessa maneira, todos os profissionais da área devem ter sempre em mente que o século XXI trouxe mudanças significativas pela globalização e avanços tecnológicos e da comunicação em massa que transformaram e, dinamicamente, transformam o mundo do trabalho, esfumaçando os antigos limites claros entre vida privada e vida profissional. Os estressores são de várias grandezas e estão constantemente presentes no mundo do trabalho, afastando os trabalhadores do ideal: "sociedades saudáveis produtivas, felizes e resilientes para as geraçõe...

Após 15 anos da criação da Revista Brasileira de Medicina do Trabalho (RBMT), e tendo exercido o cargo de editor chefe em várias gestões da Associação Nacional de Medicina do Trabalho (ANAMT), o professor René Mendes decidiu passar o bastão para que outros colegas continuem o trabalho iniciado em 2003. A semente lançada em seu primeiro editorial 1 indicava um esforço para que a ANAMT tivesse um periódico reconhecido por seus pares, em nível nacional e internacional, e que pudesse "ser um fórum permanente de discussão de idéias relacionadas com as grandes questões de Saúde e Trabalho". O trabalho frutificou, com o permanente compartilhamento de conhecimento científico atualizado na área.O Prof. René é reconhecido como uma das mais importantes autoridades nacionais e do continente americano na área de Medicina do Trabalho. Nesse período, o Prof. René agregou colegas, professores de universidades, profissionais de instituições governamentais e não governamentais, e os inseridos no setor privado para construir um periódico que se tornasse uma "estratégia deliberada de Educação Continuada em Medicina do Trabalho" 1 .Na atual gestão da ANAMT (2016-2019), a RBMT teve como editores chefes o Prof. René e a Profa. Frida Marina Fischer, que contaram com o apoio de editores associados e do conselho editorial formado por diversos profissionais de referência no âmbito técnico-científico -mantendo uma tradição criada desde o primeiro volume do periódico 1 . Nos últimos dois anos, foram revistos critérios de avaliação, submissão e publicação; a periodicidade da publicação foi aumentada; o acesso universal gratuito foi ampliado com a versão digital; a internacionalização com produção bilíngue (português ou espanhol, e inglês); a busca pela indexação em importantes bases bibliográficas, entre outras iniciativas 2 , que revela o desejo permanente de crescimento no número de leitores e a consolidação como um veículo de publicação de materiais de qualidade.Apesar de ter deixado a chefia compartilhada da editoria científica há dois meses, o Prof. René continua a ser um importante colaborador do periódico. Por decisão da Presidência da ANAMT, com anuência da Diretoria Científica e Comitê Executivo da RBMT, foi outorgado o inédito título de Editor Honorário, um cargo vitalício e que reconhece sua contribuição histórica em prol da divulgação técnico-científica no campo da interface entre Saúde e Trabalho. A contribuição do Prof. René à RBMT foi e continua sendo muito valiosa.Assim, o sonho sonhado há 15 anos se concretizou. Abriu novas oportunidades para autores divulgarem seus estudos, apresentarem ideias, além de permitir a divulgação de importantes documentos produzidos por comitês de especialistas nacionais e internacionais, agregando conhecimento à Medicina do Trabalho e a áreas correlatas.
