We present an elderly female patient with fever, aplastic anemia, arthralgic symptoms and atypical pneumonia. Serological and clinical findings suggested Parvovirus B19 and Chlamydophila pneumoniae infection. These supposed infections delayed the recognition of underlying sarcoidosis which definitive diagnosis was reached through a lung biopsy and histological demonstration of nonnecrotizing granulomas containing giant cells and noncaseating epithelioid cells. The present case highlights the potential difficulty to diagnose sarcoidosis in the presence of unusual infections which may complicate the course of this disease.
INTRODUZIONENumerosi studi condotti sulla etiopatogenesi dell'aterosclerosi hanno dimostrato che l'infiammazione della parete dei vasi arteriosi gioca un ruolo essenziale sia nell'innescare il danno, sia nella progressione di esso determinando una erosione, fissurazione e successiva progressione della placca (2). Sebbene l'infiammazione sia presente e dimostrata dall'evidenza di numerosi marcatori (proteina C reattiva, fibrinogeno plasmatico), non sono stati ancora definiti i potenziali fattori che la determinino o che la possano innescare (9). Un possibile ruolo etiopatogenetico potrebbe essere svolto anche da alcuni microrganismi a localizzazione intracellulare quali virus (Cytomegalovirus, Herpes virus, enterovirus) e schizomiceti (Helicobacter pylori, Chlamydophila pneumoniae, Mycoplasma pneumoniae) benché i risultati di queste ricerche sono ancora controversi ed attendono ulteriori conferme (7,10,14,(17)(18)(21)(22). Altri studi recenti hanno messo in evidenza una differente evoluzione della lesione aterosclerotica correlata al flusso emodinamico alterato che si viene a creare nell'arteria colpita (6,12). Particolare interesse è stato rivolto alla ricerca di C. pneumoniae nelle arterie ateromasiche e allo studio del suo possibile ruolo nella etiopatogenesi del danno aterosclerotico (2, 16). Le difficoltà di identificazione del patogeno con tecniche dirette al suo isolamento, hanno inizialmente limitato gli studi all'impiego di metodiche sierologiche volte ad evidenziare un titolo anticorpale significativo e, soprattutto, una sieroconversione in un periodo di circa 3-4 settimane (2,7,(16)(17)(18) SUMMARYSome difficult microorganisms, including Chlamydophila pneumoniae, are associated with the atherosclerotic tissue damage.The aim of this study was to evaluate the employment of culture together molecular techniques in order to define the possible role of C. pneumoniae in the atherosclerotic tissue damage. Atheromatous carotid plaques (ACP) were obtained by endoarterectomies from 10 patients with severe stenosis of the internal carotid artery. Each specimen was divided in three parts: a proximal tract to heart, without stenosis, a medial tract, corresponding to the atheromatous plaque, and a distal tract, above the plaque.Aliquots were employed to perform cultures for C. pneumoniae on Hep -2 cell line in DMEM. DNA and total RNA were extracted from 50-70 mg. of tissue sample and from Hep -2 10 6 cultures to investigate 16S rRNA, momp and hsp60 genes.The PCR and RT-PCR resulted negative for momp gene of C. pneumoniae in all samples. PCR and RT-PCR resulted positive for 16S rRNA or hsp60 genes of C. pneumoniae in the proximal portion of two ACPs with hemorrhagic evolution in two patients, one of which complicated with a retinal tromboembolic outcome. Molecular analyses on C. pneumoniae growing from the cultures are in progress. INTRODUZIONENonostante i progressi in campo profilattico e terapeutico, le infezioni osteo-articolari e protesiche rappresentano un problema grave e complesso per l'infettivologo ed il chirurgo ortopedico. Infatti, gli elevati tassi di successo della terapia antibiotica, che si osservano nella maggior parte delle malattie infettive, non sono stati ancora raggiunti nelle infezioni osteoarticolari, a causa delle caratteristiche anatomiche e fisiologiche dell'osso che richiedono trattamenti antibiotici protratti e spesso associati ad interventi chirurgici non sempre realizzabili o risolutivi. Inoltre, con l'incremento della popolazione anziana e dell'impiego sempre più esteso di impianti protesici ortopedici, l'incidenza di queste patologie è destinata ad aumentare. In Italia vengono ogni anno impiantate circa 50.000 protesi d'anca e 10.000 protesi di ginocchio. Mentre nel 1999 l'incidenza di infezione nelle protesi d'anca si attestava attorno a 1-1.5% all'anno, con l'aumento dei reimpianti, l'incidenza è allo stato attuale triplicata (3-5% all'anno). Si può pertanto stimare che in 100 pazienti operati si svilupperanno complessivamente 10-15 episodi infettivi importanti nell'arco di 10 anni (5). L'osteomielite cronica è caratterizzata da un'infezione persistente e recidivante che evolve in mesi o anni ed è caratterizzata, rispetto alla forma acuta, dalla presenza di un minor grado di infiammazione, sequestri, deposizione di tessuto osseo di neogenesi e tratti fistolosi. Gli schemi classificativi più utilizzati sono quelli di Waldvogel et al. e di Cierny et al. fondati, rispettivamente, sulla patogenesi e sull'anatomia dell'infezione e fisiologia dell'ospite (5,14,19). Se lo Stafilococco aureo rappresenta uno degli agenti etiologici maggiormente implicati nelle infezioni osteo-articolari, gli stafilococchi coagulasi-negativi, quali S. epidermidis, si riscontrano più frequentemente in quelle di protesi; streptococchi, anaerobi, enterococchi, batteri Gram-negativi e miceti si associano invece ad infezioni che coinvolgono principalmente l'immunocompromesso ed il trapiantato.
