Introducción: La distrofia simpática refleja, también es conocida como algoneurodistrofia, causalgia, fibromialgia, atrofia de Sudeck o síndrome del dolor regional complejo tipo I (SDRCT1). Relato de caso: Paciente de sexo femenino, 22 años, con reporte de caída de altura con trauma en la extensión del miembro superior derecho. Evolucionó después del evento con dolor y parestesias difusas en el miembro superior derecho. Electroneuromiografía de los miembros superiores no evidenció alteraciones relevantes. A pesar del uso de diversos medicamentos cursó con dolor intratable, impidiendo la realización de fisioterapia adecuadamente. Fue elegido la simpatectomía del ganglio cervical inferior a cielo abierto. En el postoperatorio evolucionó con el síndrome de Horner ipsilateral, como se esperaba, con moderado control del dolor. Discusión: En el SDRCT1 nótese que lesiones leves a moderadas de las extremidades o en áreas distantes de la zona de distrofia pueden preceder el surgimiento de los síntomas. No se verifica lesión de un nervio específico en los estudios electrofisiológicos y el cuadro clínico se caracteriza por una gama de síntomas sensorios motores y disautonómicos. En cuanto a los medicamentos no existe unanimidad, siendo aún posible el uso de bloqueos del ganglio estrellado y la simpatectomía de este. Conclusión: La simpatectomía del ganglio cervicotorácico (ganglio estrellado) puede ser una opción de tratamiento del SDRCT1 cuandolas modalidades del tratamiento conservador no promueven un resultado satisfactorio.
