Introdução: A tomossíntese (DBT) tem sido cada vez mais utilizada para o diagnóstico precoce do câncer de mama. Estudos demonstram que sua integração pode levar a aumento nas taxas de detecção de cânceres e a menor taxa de reconvocação das pacientes. A biópsia aspirativa a vácuo (BAV) tem sido método de preferência como alternativa à biópsia cirúrgica para o diagnóstico das lesões mamárias, especialmente no caso de microcalcificações. Surgiu então a necessidade de se acessarem as lesões evidenciadas pela DBT, assim como de facilitar a abordagem daquelas evidenciadas na mamografia digital. A associação dos métodos (BAV + DBT) corresponde a fator promissor nas taxas de diagnósticos de câncer nessas lesões. Objetivo: Demonstrar a primeira experiência em casos de BAV guiados por tomossíntese em mesa dedicada (BAV-GTM), em lesões evidenciadas na mamografia. Métodos: Realizaram-se na instituição, de novembro de 2018 a fevereiro de 2019, 197 BAV-GTM. O método de obtenção dos fragmentos foi o mesmo do utilizado nas BAV guiadas por estereotaxia digital em mesa (GEDM). Resultados: Analisaram-se retrospectivamente 197 BAV-GT em 194 pacientes encaminhadas por seus médicos assistentes, com alterações mamográficas categorias 3, 4 e 5 BI-RADS, a saber, massas, microcalcificações, assimetrias ou distorções da arquitetura do parênquima. A idade média das pacientes foi de 54 anos (variando entre 31 e 90 anos). Utilizaram-se, para tais procedimentos, mesa dedicada Affirm Prone Hologic e dispositivos Attec ou Encore, com respectivas agulhas de 9 Gauge. O tempo de procedimento nas BAV-GTM foi menor (8 a 17 minutos - média 12 minutos), quando comparado com as BAV-GEDM (média 22 minutos). Observamos maior facilidade em acessar as áreas mais conspícuas das lesões, além de poder acessar mais facilmente aquelas lesões somente evidenciadas na tomossíntese. Pode-se ainda introduzir o clipe marcador no mesmo corte escolhido para acessar a lesão, possibilitando maior precisão em sua localização. Dispomos ainda do uso do braço lateral para mamas com muito pequena espessura após a compressão, possibilitando a realização do procedimento. Lesões somente identificadas na tomossíntese foram acessadas com maior facilidade. A identificação imediata do número de microcalcificações nos espécimes obtidos foi maior na tomossíntese. Não se observaram maiores complicações do que as habituais. Conclusão: A BAV-GTM tem demonstrado resultados promissores, especialmente nas lesões somente evidenciadas na tomossíntese. Estudos têm demonstrado superioridade do método em relação à BAV-GEDM, sendo ainda esperado que as desvantagens observadas no equipamento acoplado ao mamógrafo sejam superadas com o uso do equipamento em mesa dedicada.
Objectives:To demonstrate the first Brazilian experience with Prone Tomosynthesis-guided (PTG) vacuum-assisted breast biopsies (VABB) in lesions identified on mammography or tomosynthesis. Material and methods: From November 2018 to February 2019, 197 PTG VABBs in this institution. Results: We retrospectively reviewed 197 PTG VABBs in patients referred from their doctors, with lesions categories 3, 4 and 5 BI-RADS ® , observed in their mammograms or tomosynthesis. The mean age was 54 years (age range 31-90 years). For this we used the Affirm ® prone Hologic system, and 9 Gauge dedicated needles. Mean time procedure was 12 minutes (time range 8-17 minutes), shorter than usually with Prone stereotactic vacuum-assisted breast biopsy (22 minutes) and fewer exposures were acquired. The greatest conspicuity area of the lesions were easier to achieve, as well as that lesions identified only on DBT. The clip placement was introduced in the same slice as the lesion was accessed, providing more precision for its location. It's possible to use the Lateral arm to access lesions in very thin breast after compression, and the tomosynthesis of the specimens shows greater number of calcifications than in the digital mammography of them. Conclusion: PTG VABB showed promising results, especially in that cases identified only on DBT and because they can be performed in less time, and less exposures than with the prone stereotactic system.
Introdução: A mamografia (MMG) é o método de escolha para o rastreio do câncer de mama e a ultrassonografia mamária (USM) é importante exame complementar, principalmente em mamas de densidade elevada. A USM é sensível para a detecção das lesões, mas é pobre na especificidade, pois a maioria das lesões sólidas encontradas é benigna. Essa limitação leva a um aumento da indicação de biópsia, com taxa de detecção de câncer de mama de somente 10-30%, mesmo utilizando-se rigorosamente os critérios BI-RADS®. Com a expectativa de superar as limitações da USM e obter maior acurácia na caracterização das lesões, desenvolveu-se a elastografia, tecnologia que combina a ultrassonografia com um princípio da física, avaliando a rigidez dos materiais. Sua utilização baseia-se na premissa de que existem diferenças significativas na resposta à aplicação da força entre os tecidos benignos (mais elásticos) e malignos (menos elásticos). Objetivo: Avaliar se o uso da elastografia como ferramenta associada à USM poderá contribuir para aprimorar a caracterização das lesões, aumentar a confiabilidade diagnóstica e a especificidade do método de USM, direcionando as pacientes mais precisamente para os procedimentos de biópsia. Métodos: Foram avaliadas com USM associada à elastografia, prospectivamente, 104 pacientes com lesões de categoria 3, 4 e 5 BI-RADS®, impalpáveis, com dimensões inferiores a 2 cm, com idade média de 47 anos, e encaminhadas para a realização de biópsias percutâneas guiadas por USM. Os resultados foram confrontados com o histopatológico, considerado padrão ouro. O equipamento de USM foi o GE Logiq 7 Expert, com tecnologia de elastografia strain, e adotou-se o critério de resistência descrito no capítulo de achados associados do BI-RADS®, macio ou rígido. Resultados: Encontraram-se 17 nódulos categoria 3, sendo quatro rígidos e 13 macios (0 malignos); 24 categoria 4 A, sendo cinco rígidos (cinco benignos) e 19 macios (19 benignos); 27 categoria 4 B, sendo 13 rígidos (dois malignos e 11 benignos) e 14 macios (12 benignos e dois malignos); 26 categoria 4 C, sendo 20 rígidos (13 malignos e sete benignos) e seis macios (cinco benignos e um maligno); e 10 categoria 5, sendo oito rígidos e dois macios (10 malignos). Sensibilidade 82%, especificidade 65%, VPP 46% e VPN 90%. Conclusão: A elastografia é um método que, apesar de operador dependente, pode melhorar a caracterização das lesões mamárias, fazendo uma indicação mais assertiva dos procedimentos.
Introdução: A síndrome de Mondor (SM) corresponde a uma tromboflebite esclerosante de veias superficiais da parede torácica anterior e constitui evento raro. Objetivo: Relatar caso de SM observada após biópsia aspirativa a vácuo (BAV). Métodos: Revisão de literatura publicada e comparação com o caso relatado. Resultados: A SM é uma doença rara e corresponde à tromboflebite de veia superficial da parede torácica anterior, podendo acometer a veia epigástrica superior, a veia tóraco-epigástrica ou a veia torácica lateral. O sinal característico encontrado é um cordão subcutâneo palpável, fibroso, semelhante a uma corda de violão, que pode promover retração da pele. As possíveis causas descritas na literatura apontam para trauma direto à veia, cirurgias, biópsias, esforço muscular, radiação e câncer de mama. É três vezes mais comum em mulheres e pode aparecer em qualquer idade. É uma doença autolimitada e benigna, tratada com compressas mornas, anti-inflamatórios e/ou analgésicos, com remissão dos sintomas em até seis semanas, podendo o cordão palpável desparecer em até sete meses. Paciente de 55 anos, submetida a BAV orientada por ultrassonografia em quadrante superior lateral de mama esquerda, sem intercorrências e com diagnóstico histopatológico de patologia benigna. Inseriu-se clipe marcado no local. A paciente evoluiu com queixa de palpação de cordão endurecido, indolor, desde a cicatriz do procedimento em direção à cicatriz umbilical, 15 dias após a BAV. Ao exame clínico palpamos cordão fibroso, com retração cutânea em todo o seu trajeto, quase até a cicatriz umbilical, especialmente quando a paciente elevava o membro superior homolateral. Na mamografia observamos imagem linear de baixa densidade, desde o clipe até o sulco inframamário. Na ecografia observamos ao Doppler vascularização seguindo todo o trajeto do cordão palpável. No momento da revisão a paciente refere apresentar varizes e fazer uso de Daflon regularmente por esse motivo. Se mantém em conduta habitual para o caso até a remissão do quadro. No momento se encontra sem dor e será mantida em acompanhamento clínico e imagenológico até a resolução dos sintomas. Conclusão: A síndrome de Mondor pode ocorrer após traumas diretos às veias superficiais do tórax e é uma doença autolimitada e benigna. Porém, por poder estar associada a algum tipo de patologia maligna, as pacientes devem ser acompanhadas e investigadas.
Objective: Elastography is a technology that evaluates the rigidity of tissues. The aim is to validate its use in combination with USM to improve the characterization of breast lesions, increasing the specificity of the method and, thus, targeting lesions more accurately for biopsy. Material and method: Four patients with lesions category 3, 4 and 5 BI-RADS ® , less than 2 cm, with a mean age of 47 years, were evaluated with USM and elastography. The results were confronted with the histopathological, considered gold standard. The USM equipment was GE Logic 7 Expert, with strain elastography technology, and the resistance criterion described in the associated findings chapter of BI-RADS ® , soft or rigid was used. Results: 17 rigid and 13 soft (0 malignant) 3-4 nodules were found; 24 category 4 A-5 rigid (5 benign) and 19 soft (19 benign); 27 category 4 B-13 rigid (2 malignant and 11 benign) and 14 soft (12 benign and 2 malignant); 26 category 4 C-20 rigid (13 malignant and 7 benign) and 6 soft (5 benign and 1 malignant); and 10 category 5-8 rigid and 2 soft (10 malignant). Sensitivity 82%, specificity 65%, PPV 46%, and VPN 90%. Conclusion: Elastography is a tool that, despite being a dependent operator, can improve the characterization of breast lesions, making a more precise indication of procedures.
