In managing care for patients with rare, rapidly progressive pathologies such as pulmonary hypertension (PH) and chronic thromboembolic hypertension (CTEPH), early diagnosis and speed of routing are of particular importance. Reducing the time spent on patient routing reduces the risk of disease progression, the rate of hospitalizations, and thereby the socioeconomic burden of the disease.In the Republic of Karelia, an algorithm for routing patients with PH and CTEPH has been created, which is an example of a successful healthcare management.
Pulmonary hypertension (PH) is a common and frequent condition in the practice of a physician according to the results of echocardiography, and no longer can cause difficulties in determining the genesis, as well as further management of tactics. PH can be not only a complication of a severe heart failure or chronic lung diseases, but also be a manifestation of much more rare forms of diseases: pulmonary arterial hypertension (PAH) or chronic thromboembolic pulmonary hypertension (CTEPH). Timely diagnosis of PH and the correct interpretation of its origin with the contemporary treatment options can improve the quality of life and prognosis for previously severe PAH, as well as talk about the potential curability of some patients with CTEPH. The article describes the approaches adopted in the region to treat patients with PH, including the steps of diagnosis both: outpatient and in-patient hospital. The experience of interaction between the experts of the Almazov National Medical Research center and the specialists of the Baranov Republic Hospital on the problems of diagnostics and treatment of patients with PAH and CTEPH is presented. The purpose of the publication is to demonstrate the possibilities of examination and treatment of patients with PH in the Republic of Karelia.
Кафедра госпитальной терапии медицинского факультета Петрозаводского государственного университета Резюме Цель. Изучить особенности течения бактериальных инфекций у больных ревма тоидным артритом (РА), оценить динамику лабораторных показателей в процессе санации очага инфекции. Материал и методы. Обследовано 46 пациентов с достоверным РА в период диагнос тики коморбидной инфекции (КИ) и после санации очага инфекции. Выполняли бактериологическое исследование с определением чувствительности флоры к антибиотикам диско-диффузионным методом, а также гемограмму, определяли уровни фибриногена в сыворотке крови, ревматоидного фактора, циркулирующих иммунных комплексов (ЦИК), С-реактивного белка. У части больных оценено количество интерлейкина (ИЛ)-1р, ИЛ-6 и неоптерина сыворотки крови с помо щью иммуноферментного анализа. Результаты. Типичные клинические проявления КИ имелись у 28(60,9 %) пациен тов, лихорадка была отмечена у 13(28,3 %) чел., лейкоцитоз и омоложение лейко цитарной формулы наблюдались у 12(26,0%) и 15(32,6%) больных соответственно. При санации инфекционного очага без коррекции базисной противовоспали тельной терапии выявлено достоверное снижение ряда лабораторных показателей (р<0,05): тромбоцитов, уровней фибриногена, ЦИ К и неоптерина. Заключение. КИ, протекая нередко бессимптомно, чаще всего у пациентов с высо кой активностью РА, могут вызывать нарушения, способствующие развитию эндо телиальной дисфункции, атеротромбоза и негативно влиять на продолжительность жизни таких больных, в связи с чем пациентам с РА особенно показано своевремен ное выявление и адекватная санация очагов инфекции.
