The conducted analysis of modern literature shows that most of the establishednormative parameters of the placement of kidneys in healthy people have not beensufficiently studied, with researchers practically do not take into account the constitutionalfeatures of the organism. The purpose of the study was to determine the topographicanatomical position of the kidneys in the frontal, sagittal and horizontal planes on thebasis of MRI in patients of different somatotypes without any kidney and urinary tractdiseases. Complex examination of 65 patients of the first and second mature age ofdifferent somatotypes, which did not have kidney and urinary tract diseases, wasperformed. To determine the somatotype, we used the mathematical scheme forB.Heath and J.Carter (1990), with the definition of the endomorphic, mesomorphic andectomorphic components of the somatotype. The renal topography was conducted ona Philips Intera-1.5T magnetic resonance imaging (standard magnetic resonance protocolincluded scanning in sagittal, frontal, and axial projections to obtain T1 weighted imaging).The angles of inclination were measured in the frontal, sagital and horizontal planes tomeasure spatial position of the kidneys. The statistical analysis of the obtained resultswas carried out using the "STATISTICA 5.5" program, using parametric and non-parametricmethods for evaluating the obtained results. It was established that the angle ofinclination of the kidney on the right side in the frontal and sagittal planes was greater in1.23-1.41 times than in men and in women of representatives of all somatotypes. Theangle between the axis and the line drawn through the middle of the vertebral bodiesdid not statistically significantly change, depending on the somatotype, sex and on theside of the study in a horizontal plane. The angles of inclination of the kidney axis aredefined in three planes: the frontal, horizontal, and sagittal in the patients, with thekidney axis directed downwards outward and forward. Мета роботи-на основі магнітно-резонансної томографії визначити характеристики кутів єдиної нирки (після нефректомії контралатеральної) у фронтальній, сагітальній і горизонтальній площинах у пацієнтів різних соматотипів. Матеріали та методи. Виконали комплексне обстеження 189 хворих першого та другого зрілого віку, серед них 124 пацієнтів з єдиною ниркою та різним строком післяопераційного періоду після видалення нирки і 65 хворих із двома нирками, які не мали захворювань нирок і сечових шляхів. Дослідження здійснили на магнітно-резонансному томографі Philips Intera-1,5T. Результати. Виконані дослідження кутів нахилу єдиної нирки, що залишилася після нефректомії контралатеральної, у фронтальній, сагітальній і горизонтальній площинах за допомогою магнітно-резонансної томографії показали певні закономірності залежно від соматотипів. Встановили, що найменший кут нахилу створюється в лівій єдиній нирці жінок ектоморфного соматотипу у фронтальній площині (19,6° ± 2,1°), найбільший-у лівій єдиній нирці чоловіків мезоморфного соматотипу в горизонтальній площині (61,4° ± 4,5°). Кути нахилу єдиної нирки у представників усіх соматотипів у горизонтальній площині були статистично значущо більшими порівняно з аналогічними параметрами осіб із двома нирками, які не мали захворювань нирок і сечових шляхів. У сагітальній площині кути нахилу лівої та правої єдиної нирки у представників усіх соматотипів статистично значущо відрізнялися, а у фронтальній площині вони відрізнялися (p < 0,05) тільки у хворих мезоморфного соматотипу. Висновки. У хворих через 2-5 років після видалення контралатеральної нирки виявляють статистично значущо менші кути нахилу правої єдиної нирки у фронтальній площині порівняно параметрами осіб із двома нирками, які не мали захворювань нирок і сечових шляхів. Кути нахилу правої та лівої єдиних нирок хворих усіх соматотипів у горизонтальній площині статистично значущо більші порівняно параметрами осіб із двома нирками, які не мали захворювань нирок і сечових шляхів. Кути нахилу лівої єдиної нирки хворих усіх соматотипів у сагітальній площині статистично значущо менші, ніж параметри правої єдиної нирки.
The evaluation of renal measurements such as length, width and thickness, volume is important in the diagnosis and treatment of many renal disorders, since there is a close relationship between the sizes of the kidneys and its function. The purpose of the study was to establish and compare during the postoperative period changes in the mass and sizes of the kidney left after nephrectomy in the sexually mature male rats. An experimental study was carried out on 84 sexually mature white male rats weighing 178-194 grams. Animals were divided into two groups: control (42 rats) and experimental (42 rats). In the control group, the animals under ketamine anesthesia were followed by an abdominal cavity dilution, after which the abdominal wall was applied in a row. All animals of the experimental group performed surgical intervention – nephrectomy of the left kidney. The animals were withdrawn from the trial by intra-pleural administration of thiopental-sodium 50 mg/kg after 7, 14, 21, 30, 90 days after nephrectomy. Macroscopic evaluation and description of the kidneys of animals was performed after their removal. Their weight was determined on the laboratory scale of HLR-200 up to 0.1 mg, and the length, width and thickness of the organ were measured with the help of a caliper to an accuracy of 0.05 mm. Calculated the volume of the kidney. The statistical analysis of the obtained results was carried out using the program STATISTICA 5.5 using parametric methods for evaluating the results. It was established that the mass, length, width, thickness and volume of the kidney of animals in the experimental group, as compared with the control group, were statistically significantly higher in all terms of observation. The fraction of the growth of the width and thickness of the single kidney of animals in the experimental group compared with the control animals in the animals was statistically significantly greater than the proportion of kidney growth during the postoperative period. It was found that the largest increase in the mass and thickness of the kidney, as compared with the control group, was observed after 30 days of the postoperative period, and the greatest increase in the length and width of the kidney, as compared with the control group, was observed after 14 days of the postoperative period.
Мета. Поділитися власним досвідом хірургічного лікування місцево-розповсюдженого раку ободової кишки з поширенням на підшлункову залозу (ПЗ). Матеріали і методи. Вивчено результати хірургічного лікування 24 хворих з місцево-розповсюдженим раком ободової кишки, що поширювався на ПЗ, за 2004 – 2015 рр. Чоловіків було 8 (33%), жінок - 16 (67%). Середній вік пацієнтів становив (65,2 ± 11,2) року. Рак ободової кишки, окрім ПЗ, проростав у паранефральну клітковину зліва – у 6 (25%) пацієнтів, селезінку – у 5 (20,8%), дванадцятипалу кишку (ДПК) – у 3 (12,5%), шлунок – у 1 (4,1%), тонку кишку – у 1 (4,1%), черевну стінку – у 1 (4,1%). Пацієнтів розподілили на дві групи. Хворим групи І - 13 (54,1%) - виконували симптоматичні оперативні втручання, групи ІІ - 11 (45,8%) - радикальні. Результати. Тривалість життя хворих після симптоматичних оперативних втручань становила (5,1 ± 3) міс. У 3 (23,0%) хворих післяопераційних ускладнень не спостерігали. Післяопераційні ускладнення (за класифікацією Clavien-Dindo) І ступеня виявлені у 5 (38,4%) хворих, ІІ – у 2 (15,3%), ІVа – у 1 (7,6%), V – у 2 (15,3%). Причинами смерті в ранні строки після операції 2 хворих з післяопераційними ускладненнями V ступеня стали профузна шлунково-кишкова кровотеча – у 1 (7,6%) та тромбоемболія легеневої артерії (ТЕЛА) - у 1 (7,6%). Тривалість життя хворих після радикальних оперативних втручань становила (19,5 ± 14) міс. Виконано такі оперативні втручання: дистальну резекцію ПЗ – у 10 (91%) хворих, панкреатодуоденальну резекцію - у 1 (9%). У 3 (27,2%) хворих післяопераційних ускладнень не спостерігали. Післяопераційні ускладнення (за класифікацією Clavien-Dindo) І ступеня виявлені у 3 (27,2%) хворих, ІІІа – у 1 (9%), ІVа – у 1 (9%), V – у 2 (18,1%). Померли 2 (18,1%) хворих від тяжких гнійно-септичних ускладнень. Висновки. Пацієнтам з раком ободової кишки, який поширюється на ПЗ, але без віддалених метастазів, низьким індексом коморбідності – Charlson Comorbidity Index (ССІ) та оцінкою за шкалою ECOG (Eastern Cooperative Onkology Group) менше 2 балів рекомендовано виконувати мультивісцеральну резекцію, у тому числі резекцію ПЗ.
проведено ультрамікроскопічне дослідження стану єдиної нирки після нефректомії. Експериментальне дослідження виконано на 60 статевозрілих щурах-самцях масою 155-160 г. Субмікроскопічні дослідження канальців нефронів кіркової речовини нирок статевозрілих тварин на сьому добу після нефректомії встановили, що їх реорганізація носить пристосувально-компенсаторний характер. Це проявляється компенсаторною гіпертрофією структурних компонентів нефронів та реорганізацією судинного русла. У пізні терміни досліду при тривалому функціональному навантажені на єдину нирку, у частини нефронів розвиваються морфофункціональні зміни деструктивного характеру.
