Мета дослідження-вивчити морфологічні зміни тканин у ділянці шовного та додаткових матеріалів, механічну міцність та бактеріологічне середовище після накладання товстокишкового анастомозу в ділянці низхідної ободової кишки у щурів. Матеріал і методи. При проведенні експериментального дослідження у роботі визначені морфологічні зміни тканин у ділянці накладеного товстокишкового анастомозу на низхідній ободової кишці у щурів, а також механічна міцність і бактеріологічне середовище на 3-, 7-, 14-, 21-шу і 30-ту добу після оперативного втручання. Досліджено дві групи тварин: перша-щури, яким накладався товстокишковий анастомоз однорядовим швом, з використанням шовного матеріалу «Вікрил 5.0»; тваринам другої групи, після накладення однорядного шва, додатково додавали сучасний адгезив для тканин «Катсил». Результати. У групі щурів, яким накладався товстокишковий анастомоз однорядовим швом, з використанням шовного матеріалу «Вікрил 5.0» з додатковим використанням адгезиву для тканин «Катсил», за даними бактеріологічного дослідження на 3-тю добу достовірно меншою була забрудненість черевної порожнини за кількістю Enterobacteriaceae Klebsiella pneumonia, Pseudomonaceae та Enterococcus faecalis, порівняно з групою тварин без додаткового укріплення зони анастомозу. За даними проведеної пневмопресії, у щурів із додатковим використанням адгезиву для тканин «Катсил» міцність анастомозу була достовірно більшою на 3-, 7-му, та 14-ту добу після накладання анастомозу, ніж у групі порівняння у 2,0, 1,3 та 2,3 раза відповідно (р<0,05). Висновки. Використання адгезиву для тканин «Катсил» знижує інфікування у ділянці товстокишкового анастомозу за результатами бактеріологічного дослідження, а також сприяє збільшенню міцності анастомозу за даними проведеної пневмопресії.
Мета роботи-дослідити особливості накладання та укріплення товстокишкового анастомозу (ТКА) у хворих з інсулінорезистентністю (ІР) з використанням сучасних адгезивних матеріалів. Матеріал і методи. З метою визначення особливостей накладання та укріплення товстокишкового анастомозу у хворих з ІР з використанням сучасних адгезивних матеріалів обстежено та прооперовано 80 осіб (медіана віку-64 (57; 71) роки). Усі пацієнти розподілені на дві групи, залежно від методу укріплення шва товстокишкового анастомозу: 1-ша група-40 осіб, яким виконане накладання однорядового безперервного шва (ОБШ) ТКА (медіана віку-65 (57; 75) років, 2-га група-40 осіб, яким виконане накладання однорядового безперервного шва ТКА, та з метою герметизації та укріплення зони анастомозу додавали сучасний адгезив для тканин Н-бутилціаноакрилат («Катсил») (медіана віку хворих-63,5 (58,5; 70,5) року. Проведено визначення спектра мікробної флори виділень із дренажів поблизу анастомозу та аналіз післяопераційних ускладнень. Результати. У хворих з ІР, яким з метою додаткового укріплення зони ТКА після накладання ОБШ використовували адгезив для тканин Н-бутилціаноакрилат, траплялася достовірно менша кількість пацієнтів з поєднанням двох різних штамів мікроорганізмів порівняно з хворими, яким накладали ОБШ ТКА без додаткового укріплення, [2 (5,0%) хворих проти 9 (22,5%)) (χ2= 5,17, df=1; р<0,05], а також достовірно більше пацієнтів з відсутнім ростом мікроорганізмів у посівах із зони анастомозу [11 (27,5%) хворих проти 3 (7, 5%) хворих відповідно) (χ2= 5,54, df=1; р<0,05]. В обох групах, незалежно від методу накладання анастомозу, відзначалися гнійно-запальні ускладнення, проте у хворих з додатковим укріпленням зони ТКА кількість гнійно-запальних змін рани та випадків неспроможності ТКА було достовірно меншою, порівняно з пацієнтами без додаткового укріплення 2,5%, проти 17,5% відповідно (χ2= 6,65, df=1; р<0,05 та χ2= 5,00, df=1; р<0,05 відповідно). Висновок. Використання сучасного адгезиву для тканин Н-бутилціаноакрилат («Катсил») для додаткового укріплення зони товстокишкового анастомозу у хворих з інсулінорезистентністю сприяє достовірному зниженню забруднення зони довкола анастомозу, а також зменшенню кількості гнійно-запальних змін рани та випадків неспроможності товстокишкового анастомозу.
Purpose of the study. To investigate the effect of additional strengthening of the colonic anastomosis (CA) using modern adhesive materials on the severity of abdominal contamination in patients with insulin resistance (IR). Materials and methods. The study involved 80 patients with IR, who underwent surgery with the CA imposition (median age of the patients – 64 (57; 71) years). All patients were divided into 2 groups, depending on the method of strengthening the CA suture: 1 group – 40 patients who underwent the application of a onerow continuous suture (OCS) of the CA (median age of patients – 65 (57; 75) years, 2 group – 40 patients to whom OCS CA was applied, and in order to seal and strengthen the anastomosis zone a modern Nbutylcyanoacrylate tissue adhesive was added (median age of patients – 63,5 (58,5; 70,5) years. The spectrum of microbial flora of secretions from drains near the anastomosis was determined. Results. The additional use of modern Nbutylcyanoacrylate tissue adhesive to strengthen the area of CA with the imposition of a OCS in patients with IR contributes to a reliable reduction of number of patients with associations consisting of two types of microorganisms compared to the patients without additional strengthening (2 (5,0%) versus 9 (22,5%) of patients, respectively) (χ2 = 5,17, df = 1; р < 0,05), the greater number of patients with no growth of microorganisms in crops from the anastomotic zone ((11 (27,5%) of patients versus 3 (7, 5%) of patients, respectively), as well as fewer cases of high degree of anastomosis zone contamination (3,48 times (χ2 = 7,68, df = 1; р < 0,05)), with prevalence of mild contamination (3, 35 times (χ2 = 15,24, df = 1; р < 0,05)). Conclusion. The additional use of modern Nbutylcyanoacrylate tissue adhesive to strengthen the area of CA with the imposition of a onerow continuous suture in patients with IR contributes to a reliable reduction of contamination of the area around the anastomosis compared to the patients without additional strengthening.

Lack of control over the course of the disease (diabetes), ignoring measures to prevent the development of complicated forms of diabetic foot, late referral of patients with existing complications on the foot, contributes to an increase in the number of patients with destructive forms of diabetic foot. Purpose of the study. To evaluate the technique of vacuum instillation therapy (NPWTi) in comparison with the use of an isolated vacuum and the classical surgical method for treating purulentnecrotic forms of diabetic foot. Materials and methods. The results of local treatment of 41 patients with purulent-necrotic forms of diabetic foot corresponding to III and IV stages (Meggitt-Wagner) were analyzed. The patients are divided into groups depending on the method of topical treatment. The cellular composition and microflora of the wounds were studied, and the change in the area of the wound surface was assessed before the start of treatment and in the 4–5th and 8–9th days. Results and discussion. The use of NPWT showed better treatment results in comparison with group 1, where signs of phase II of the wound process were diagnosed by 8–9 days from the start of topical treatment in 83,3% of patients. The highest efficiency in topical treatment was noted in the 3rd group using the NPWTi method, where by the 4th–5th day, ½ patients were cytologically diagnosed with phase II of the wound process. Conclusions. The NPWTi technique is most effective in the treatment of purulent-necrotic forms of diabetic foot, which makes it possible to reduce the time of wound cleaning, the time of the patient's stay in the hospital, and save the limb.
