Tuberculosis cases in Russia doubled from 1991 to 2000, when the incidence reached 90 per 100,000; and Russian TB mortality rates are the highest in Europe. Implementation of WHO-recommended DOTS strategy is in the preliminary stages. The epidemiological data are presented as is the implementation strategy for stabilizing the epidemic.
Цель исследования: анализ эпидемической ситуации по туберкулезу, сочетанному с ВИЧ-инфекцией (ТБ/ВИЧ-и), в субъектах РФ, определение взаимосвязанных эпидемиологических показателей. Материалы и методы. Использованы сведения из отчетных форм Росстата № 61, 8 и 33, а также данные Росстата о численности населения за 2017 г. Данные о структуре популяции пациентов с сочетанной патологией получены из отчетной формы Росстата № 61 за 2017 г. Для поиска взаимосвязей между показателями, характеризующими популяции пациентов с туберкулезом и пациентов с ТБ/ВИЧ-и, проведен корреляционный анализ (стандартные пакеты статистической программы Statistica). Результаты исследования. Распространение ТБ/ВИЧ-и в России характеризуется выраженной неравномерностью и наибольшей распространенностью в 35 субъектах РФ. Результаты корреляционного анализа показали, что в 37 субъектах РФ, где доля пациентов с ТБ/ВИЧ-и была низкой (< 7% от общего числа пациентов с туберкулезом), смертность от туберкулеза в течение 1-го года наблюдения зависела от уровня распространения туберкулеза с множественной лекарственной устойчивостью возбудителя (МЛУ-ТБ) среди всех пациентов с туберкулезом. В 48 субъектах РФ, где доля пациентов с сочетанной патологией была высокой (≥ 7% от общего числа пациентов с туберкулезом), распространение МЛУ-ТБ и смертность среди пациентов с сочетанной патологией в течение 1-го года наблюдения напрямую зависели от уровня распространения сочетанной патологии. В 16 субъектах РФ, в которых доля впервые выявленных пациентов с сочетанной патологией с МЛУ-ТБ составляла от 23 до 60%, риск развития летальных исходов был наиболее высоким. Наличие сопутствующего гепатита установлено у 42% впервые выявленных пациентов с сочетанной патологией и у 43%-в контингентах.

Р у с а к о в а , в. в. т е с т о в Ф ГБН У «Центральный Н И И туберкулеза», м оск ва Представлены результаты анализа эпидемической ситуации по туберкулезу в курируемых институтом 15 регионах Р Ф за 2014-2015 гг. Сни жение показателя заболеваемости туберкулезом за 2014-2015 гг. произошло в 11 из 15 регионов. Н а 4 территориях отмечен незначительный рост. Показатель заболеваемости туберкулезом детей в возрасте 0-17 лет снизился в 10 регионах, в 5-отмечен рост. Показатель смертно сти от туберкулеза снизился, но на 3 территориях он превышает среднестатистический показатель по стране (9,2 на 100 тыс. населения). Эффективность лечения впервые выявленных больных туберкулезом легких, зарегистрированных для лечения в 2014 г. с положительной бактериоскопией мокроты, колебалась от 69,8% в Орловской области до 32,6% в Ульяновской области, по посеву мокроты-от 70,6% в О р ловской области до 34,2% в Ульяновской области, по закрытию полостей распада-от 72,3% в Орловской области до 40,2% в Ульяновской области. Основными причинами низкой результативности химиотерапии являю тся отрыв от лечения, множественная лекарственная устойчивость микобактерий туберкулеза (М Б Т), низкая хирургическая активность. Д ля повышения эффективности лечения необходимо повсеместно внедрить быстрые методы определения лекарственной чувствительности М Б Т ко всем противотуберкулезным препаратам и проводить адекватную химиотерапию только теми препаратами, к которым М Б Т сохранили чувствительность, активнее применять хирургические методы вмешательства.
