Lead belongs to the group of blood poisons that impair the synthesis of porphyrins and heme. Under industrial conditions, only chronic lead poisoning can develop. Lead belongs to the poisons that have the effect of material cumulation. The half-life of lead is 20 years. Once in the body, it is deposited in many organs in the form of the insoluble tribasic lead phosphates. A significant part of the lead is deposited in the trabeculae of the bones. Under the influence of provoking factors, an intensive lead release from the depot can be observed. In such cases, the amount of lead in the circulating blood increases sharply, and remission is replaced by an exacerbation. There is a wavy course of chronic lead intoxication. Lead and its inorganic compounds belong to the group of poisons that have a polytropic effect on the body, affecting many organs and systems. The blood system (anemia with specific characteristics) and the nervous system (polyneuropathy and encephalopathy) are primarily affected. A number of other organs and systems are also affected. The most severe specific syndrome of gastrointestinal tract damage is lead colic. Due to the impaired synthesis of porphyrins and heme in certain biological substrates of the body — in the blood, erythrocytes and urine, substances unused in the synthesis of heme are accumulated. They are markers of chronic intoxication caused by lead, in the presence of a relevant clinical picture. The diagnosis is based on data from a professional history, sanitary and hygienic characteristics of working conditions, clinical and objective characteristics of the disease and data from laboratory examination. The main thing is to stop contact with lead and remove it from the body. Antidotes for lead poisoning are chelators: tetacinum-calcium, pentacinum, D-penicillamine. In combination with technical and sanitary-hygienic measures to prevent chronic intoxication caused by lead, preliminary and periodic medical examinations of persons in contact with lead are of great importance.

The aim: To investigate the pathological changes in intra- and extra cranial blood vessels in workers of locomotive crews depending on their length of service. Materials and methods: The study involved 249 male patients, aged from 21 to 60 years who worked as locomotive drivers. We used Doppler examination and studied Doppler Indexes of cerebral vessels. Results: Results of examination showed that with increasing length of work of workers of locomotive crews (together with increasing of their age) we have established: 1) Reduction the elasticity of blood vessels (77.6% of the patients); 2) Increased violation of venous circulation (86.4%); 3) Reduced blood flow in the basilar artery (64%); 4) Reduced blood flow in the right and left vertebral arteries (65.6% and 65.5% respectively); 5) Increase in the thickness of intimate / media complex more than 1.1 in 62 % of drivers. All of that also connected with length of their work. Conclusions: we founded that speed of blood flow and elasticity in intra- and extra cranial vessels reduced with increase the length of service of workers of locomotive crews. Also we received such results: increasing violation of venous circulation, reducing blood flow in the basilar artery and reducing of blood flow in right and left vertebral arteries, increase intima / media thicknesses greater than 1,1 in locomotive drivers.
Benzene is a member of the aromatic hydrocarbon group and belongs to the group of blood poisons that cause inhibition of bone marrow hematopoiesis with the development of hypoplastic or aplastic anemia. The benzene can result in acute and chronic intoxication. Under industrial conditions, the penetration of benzene and its homologues into the human body is possible through the lungs and intact skin. The symptoms of changes in the nervous system and bone marrow hematopoiesis in acute and chronic exposure to benzene are different. Acute intoxication manifests itself with symptoms of damage to the central nervous system with the phenomena of general brain disorders, similar to poisoning with substances with narcotic properties. Chronic intoxication develops slowly, unnoticed by the patient, and is characterized by bone marrow damage with impaired blood cell formation. An atypical form of chronic benzene intoxication is the development of benzene leukemia. Therefore, benzene belongs to the group of industrial carcinogens. Typical forms of acute and chronic benzene intoxication are easy to recognize. To establish the diagnosis of benzene intoxication, it is necessary at first to have data confirming the patient’s contact with benzene. Repeated studies of peripheral blood, bone marrow puncture are important. There are no specific antidotes for benzene intoxication. Symptomatic treatment is performed only. Therefore, it is important to prevent the development of benzene intoxication in the workplace before the stage of deep irreversible disorders.

Arsenic hydrogen is a colorless gas. It enters the body through the respiratory system, without irritating the mucous membranes of the upper respiratory tract. It is a poison with a primary hemolytic effect. In the body, arsenic hydrogen is oxidized to arsenic and excreted from the body mainly through the kidneys and intestines. The consequence of rapid and massive hemolysis is anemia, hypoxia, damage to parenchymal organs (kidneys, liver), nervous and cardiovascular system. In addition to hemolysis, a significant role is played by the direct toxic effect of arsenic, which is formed during the oxidation of arsenic hydrogen, on the organs and tissues of the body. The development of acute and chronic poisoning with arsenic hydrogen is possible. Acute poisoning causes mild, moderate and severe intoxication. Chronic intoxication with arsenic hydrogen is very rare. Moderate anemia, which does not have clear clinical manifestations of hemolysis, is characteristic. Urgent hospitalization, rest, fluid intake in large quantities are necessary. Antidotes for arsenic hydrogen poisoning are unithiol and sodium thiosulfate. Prolonged inhalations of oxygen are indicated. In the absence of severe kidney damage, diuretics are used. In renal failure, hemodialysis is performed. In severe cases, an exchange transfusion is possible. The presence of a characteristic clinical picture of the disease, along with anamnesis data and an increased urine arsenic level, allows you to establish the correct diagnosis. After severe acute and significant chronic intoxications, a patient is released from work with arsenic hydrogen and other toxic substances. He is referred to the medical and social expert commission to establish a disability group due to an occupational disease. Prevention of arsenic hydrogen poisoning in industrial conditions should be reduced to careful control over the use of purer metals and acids that do not contain arsenic, the installation of effective local and general ventilation. Constant sanitary and hygienic control over the release of arsenic hydrogen into the air of working premises is necessary.

Випаровується навіть при температурі 0 °С, має високу (для металів) температуру кипіння (357 °С) і пароутворення, високу електро-і теплопровідність, значну хімічну стійкість і щільність. Завдяки цим властивостям ртуть широко застосовується в різних приладах (термометри, манометри, аерометри), випрямлячах електричного струму, ртутних і кварцових лампах. Тетраэтилсвинец и его смеси принадлежат к группе сильных нейротоксических веществ І класса опасности. Острые и хронические интоксикации тетраэтилсвинцом и его смесями характеризуются, главным образом, поражением центральной нервной системы. По тяжести течения различают три стадии острой интоксикации, вызванной тетраэтилсвинцом: начальную (астенический синдром, органический синдром по типу энцефалопатии, или псевдопаралитический, предделириозный синдром), предкульминационную и кульминационную. По степени тяжести различают три стадии хронической интоксикации: начальная, токсической энцефалопатии и психомоторных нарушений. Диагностика отравления основывается на анамнестических данных, клинической картине заболевания и наличии свинца в моче за счет распада тетраэтилсвинца в организме. При острых и хронических интоксикациях, вызванных тетраэтилсвинцом и его смесями, отсутствуют изменения в крови и порфириновом обмене, которые характерны для влияния неорганических соединений свинца. Лечение симптоматическое. Специфических антидотов нет. Необходимо обработать кожу и сменить загрязненную одежду. При заглатывании этилированного бензина следует неоднократно промывать желудок с использованием активированного угля до исчезновения запаха в промывных водах. Больным с острой и хронической интоксикацией, вызванной тетраэтилсвинцом, необходим полный покой, снотворные и седативные средства, витамины, теплые ванны (37 °С) перед сном. Внутривенно вводят раствор глюкозы с аскорбиновой кислотой.
